TPF（多西他赛、顺铂、氟尿嘧啶）方案已经作为局部晚期头颈鳞癌（LAHNSCC）标准化的治疗手段,其产生的毒副作用常常增加并发症。改良TPF（m TPF）方案治疗胃癌的耐受性好,疗效显著。该文作者对2010年7月-2014年7月间来自法国3个医疗中心的48例LAHNSCC患者进行回顾分析,这些患者均接受m TPF治疗。

As with many types of cancer,immunotherapy is changing the management of squamous cell carcinoma of the head and neck(HNSCC).In a locally advanced or metastatic setting,treatment options have long been curtailed,but this paradigm is currently changing.Checkpoint inhibitors were the first to be validated in second-line treatment with PD-1 and PD-L1 inhibitors and these treatments are available in USA for first-line use.In addition,many studies are underway to use its molecules earlier in the care or try to increase their effectiveness with associations.The issues of patient selection that would benefit the most from immunotherapy and the evaluation of the response to these treatments are not completely solved.The goal is here to update the possibilities of current treatment by immunotherapy in HNSCC as well as on various development pathways in progress.

AIM: To review the characteristics of hematological malignancies in tropical areas, and to focus on the specific difficulties regarding their management. METHODS: This is a retrospective narrative review of cases of patients with hematological malignancies. All medical files of patients with malignant disease whose treatment was coordinated by the HematoOncology service of the Cayenne Hospital in French Guiana between the 1st of january 2010 and the 31 st of December 2012 were reviewed. Clinical data were extracted from the medical files and included: Demographic data, comorbidities, serological status for human immunodeficiency virus, human T-lymphotropic virus 1(HTLV1), hepatitis B virus and hepatitis C virusinfections, cytology and pathology diagnoses, disease extension, treatment, organization of disease management, and follow-up. The subgroup of patients with hematological malignancies and virus-related malignancies were reviewed. Cases involving patients with Kaposi sarcoma, and information on solid tumor occurrence in virus-infected patients in the whole patient population were included. Since the data were rendered anonymous, no informed consent was obtained from the patients for this retrospective analysis. Data were compiled using EXCEL® software, and the data presentation is descriptive only. The references search was guided by the nature of the data and discussion. RESULTS: In total, the clinical files of 594 patients(pts) were reviewed. Hematological malignancies were observed in 87 patients, and Kaposi sarcoma in 2 patients. In total, 70 patients had a viral infection, and 34 of these also had hematological malignancies. The hematological diagnoses were: Multiple myeloma in 27 pts, lymphoma(L) in 43 pts, myeloproliferative disorders in 17 pts and Kaposi sarcoma in two patients. Background: There has been a rapid global development of the horizontal drilling and hydraulic fracturing process termed fracking. This involves the dispersion of “produced water” which contains naturally occurring radioactive material (NORM) which may contaminate surface water and pose a health risk. Objectives: To investigate association between early (0-28 days) infant mortality by county in Pennsylvania and fracking. Methods: We compared early infant mortality for 2007-2010 after fracking developed with a control period 2003-2006, contrasting a group of the 10 most heavily fracked counties with the rest of Pennsylvania. Results: Whilst early infant deaths decreased by 2.4% in the State over the period, in the 82,558 births in the 10 fracked counties there was a significant increase in mortality (238 vs 193;RR = 1.29;95% CI 1.05, 1.55;p = 0.011). For the five north east fracked counties Bradford, Susquehanna, Lycoming, Wyoming and Tioga the combined early infant mortality increased from 34 deaths to 60 (RR 1.66;1.05, 2.51;p = 0.014), whereas in the south western 5 counties Washington, Westmoreland, fayette, Butler and Greene the increase was modest, 157 to 178 (RR 1.18;0.95, 1.46;p = 0.13). Increased risk was associated with exposure to groundwater, expressed as the county ratio of water wells divided by the number of births. Conclusions: Fracking appears to be associated with early infant mortality in populations living in counties where the process is carried out. There is some evidence that the effect is associated with private water well density and/or environmental law violations.

N-acetylbenzoquinoneimine (NABQI) is a toxic metabolite of the common analgesic acetaminophen (APAP). NABQI is an electrophilic intermediate formed via the oxidation of APAP within the cytochrome P450 system. Within the normally recommended low-dose use of APAP, NABQI is a minor metabolite which is either quickly reduced back to APAP or conjugated to Glutathione (GSH) producing an innocuous by-product. However, with overdose or prolonged high-dose usage of acetaminophen, GSH levels can become depleted and the bioactive NABQI is thought to form adducts with proteins and oxidize protein sulfhydryls producing intra- and intermolecular disulfide bridges in proteins. In this work we investigated the effect of NABQI on purified kidney Na,K-ATPase to see if the clinical renal insufficiencies seen in APAP overdose may be linked to inhibition of the Na,K-ATPase. Our work has shown that NABQI does indeed inhibit the Na,K-ATPase in a dose dependent (IC50 = 19.8 ± 2.9 μM) and irreversible manner. Interestingly, brief storage of NABQI at -20°C eliminates the irreversible effects of the compound, and leads to a product that remains a potent reversible inhibitor of the Na,K-ATPase (IC50 = 58.7 ± 19.5 μM). Further, the reversible inhibition produced by stored NABQI competes with para-nitrop.
