Anatomical features associated with venous congestion in DIEP flap using CT angiography with three-dimensional reconstruction

Aim: Computed tomography angiography (CTA) using three-dimensional (3D) virtual reconstruction has been increasingly used in planning deep inferior epigastric artery perforator (DIEP) breast reconstruction. Although the most common complication associated with this surgery is diffuse venous congestion, its origin remains unclear. The aim of this study was to assess the anatomical characteristics of the anterior abdominal wall vessels that could predict venous congestion, using CTA with 3D virtual reconstruction. Methods: A retrospective case-control study was conducted and a total of 169 DIEP flaps were reviewed. An abdominal CTA with 3D virtual reconstruction was analyzed with regard to anatomical features of the abdominal wall vessels. Seven venous congestive cases were identified. For each case, 3 controls that had not exhibited any vascular complications were randomly selected. Results: The global venous congestion rate was 4.14%. No statistically significant differences were found between the groups' superficial inferior epigastric vein (SIEV) diameter (P = 0.915), number of branches of SIEV (P = 0.371), number of perforators per flap (P = 0.255), flap subcutaneous tissue thickness (P = 0.652), direct communications between SIEV-perforators (P = 0.418), and communications of both SIEVs across the abdominal midline (P = 0.371). Conclusion: The present study provided new information concerning the identification of the controversial anatomical features associated with venous congestion in DIEP flaps. CTA and 3D virtual reconstruction were useful tools for evaluating the abdominal wall anatomy and for planning DIEP breast surgery, but neither for predicting nor preventing the diffuse congestive phenomenon.

Cost Deployment Tool for Technological Innovation of World Class Manufacturing

This article brings a discussion about using the Cost Deployment methodology for technological innovation in the World Class Manufacturing (WCM) systems at Fiat Group Automobiles Production System (FAPS). It aims to show how this tool acts in the technical pillars of the WCM, and its proper use as an alternative to innovate in production processes, achieving a drastic reduction in wastes and cost optimization during specific activities in production systems. The Cost Deployment builds a distinctive transversal method of WCM which helps to promote and provide extremely effectiveness in the activation of more specific methods that have been tried successfully in the Japanese manufacturing improvements. It also allows to link the operational performances, usually measured with indicators such as efficiency, providing number of defects, hours of desaturation. The used methodology was based on a literature review about the proposed topic. It ends up finding that the Cost Deployment tool is one of the most sophisticated technological innovations existing for the production systems of the World Class Manufacturing.

Laurel forests are quite relevant for biodiversity conservation and are among the island ecosystems most severely damaged by human *** the past,Canary laurel forests have been greatly altered by logging,livestock and *** remains of laurel forests are currently protected in the Canary Islands(Spain).However,we miss basic information needed for their restoration and adaptive management,such as tree longevity,growth potential and responsiveness to natural and anthropogenic *** dendrochronological methods,we studied how forest dynamic is related to land-use change and windstorms in two well-preserved laurel forests on Tenerife *** cores were collected from over 80 trees per stand at three stands per *** used ring-width series to estimate tree ages and calculate annual basal area increments(BAI),cumulative diameter increases,and changes indicative of released and suppressed *** tree species were found in all stands,with Laurus novocanariensis,Ilex canariensis and Morella faya being the most common *** some individuals were over 100 years old,61.8%-88.9% of the trees per stand established between 1940 and 1970,coinciding with a post-war period of land abandonment,rural exodus and the onset of a tourism *** trees have shown growth rates larger than 1 cm diameter per year and most species have had increasing BAI trends over the past *** growth releases occurred after windstorms at both sites,but the effects of windstorms were site-dependent,with the 1958 storm affecting mainly the eastern tip of the island(Anaga massif)and the 1991 storm the western tip(Teno massif).Given the great ability of laurel forest trees to establish after land use cessation and to increase growth after local disturbances such as windstorms,passive restoration may be sufficient to regenerate this habitat in currently degraded areas.

Antileishmanial assessment of leaf extracts from Pluchea carolinensis,Pluchea odorata and Pluchea rosea

Endovascular treatment of paravisceral mycotic aneurysm: Chimmeny endovascular sealing the end of de road

Open surgery is the elective treatment for mycotic aneurysms of the aorta. This surgery consists of resection of the aneurysm, debridement and revascularization with an in situ or extra-anatomic bypass. Even when surgery has been successful, the morbimortality is raised and the endovascular treatment has become an alternative for specific patients. When mycotic aneurysms involved the visceral arteries, more complex techniques are necessary such as fenestrated endovascular aortic repair or chimmeny endovascular aortic repair and the most frequent complications of this are endoleaks and oclussion the visceral arteries. We present a case of a pacient with a paravisceral abdominal mycotic aneurysms that was result with 2 chimney technique(in the right renal and superior mesenteric arteries) and a single Nellix EVAS(Endologix, Irvine, Calif) of 12 cm long without evidence of endoleaks in the follow-up.

Renin-Angiotensin System (RAS) is involved with hypertension and other cardiovascular diseases. However, the association of RAS components to cancer still causes suspicion. To try to clarify this, here we aimed to show this association for three important components: Angiotensin Converting Enzyme 1 (ACE1), Angiotensin Type 1 Receptor (AGTR1) and Angiotensin Type 2 Receptor (AGTR2). The first articles show that association of RAS components with cancer dates back to the 70’s. ECA1 and AGTR1 have close association with cancer and ACE1 inhibitors or AGTR1 blockers are candidates to treatment of some tumors. Moreover, the action of AGTR2 is still controversial, but most studies show that the increased expression of AGTR2 can attack the cancer cells. In breast cancer, these components have also been widely studied and many works have shown that the correlation exists. Therefore specific target using these RAS components could be a beneficial, novel therapy to various tumors.

Species richness and community structure of class Polyplacophora at the intertidal rocky shore on the marine priority region no.33,Mexico

Marine Priority Region No. 33 (MPR 33) lies in the State of Guerrero, where the National Commission for Knowledge and Use of Biodiversity has declared a lack of information on marine species, Class POLYPLACOPHORA among others. This program was carried out on the rocky intertidal zone of MPR 33 with the objectives to: 1) determine species richness, 2) community composition based on families represented, 3) estimate the density, 4) index of diversity, 5) analyze body proportions and 6) geographic distribution of species. Three surveys on five sites were conducted during 2009 and 2012, using one mt2 quadrant for systematic sampling procedure;collecting all POLYPLACOPHORA specimens found within the quadrants, then proceeding with their preservation. 441 organisms were analyzed where 17 species were identified, on nine genera within six families. Fifteen new species were recorded for the MPR 33. Family ISCHNOCHITONIDAE Dall, 1889 was the best represented in species richness and CHITONIDAE Rafinesque, 1815 in abundance. Density of 8.82 organisms/m2, the H′ index was 2.38 bits/individuals and J′ = 0.57. The species Chiton (Chiton) articulatus Sowerby, 1832 showed the highest density, greater size and was found in all five collection sites. Three species, (Chiton (C.) articulatus, Ischnochiton (Ischnochiton) muscarius (Reeve, 1847) and Chiton (Chiton) alboli-neatus Broderip & Sowerby, 1829), were considered wide. Intertidal species richness was associated with habitat stability and wave intensity;the more unstable sites had greater richness while most stable sites lower. Species inventory and understanding the communities’ interactions of the marine fauna of the State of Guerrero are important;therefore these efforts must be continued.

An important part of the fishing activity in Mexico is carried out in the coastal strip and has a strong social, economic and ecological importance. In the state of Guerrero, mollusks fishing is coastal and craftwork activity focused on a small number of species and without information on biology and reproductive or ecological aspects for most species caught. This research was carried out in the Costa Chica region, Guerrero, Mexico and the aims were: 1) To analyze the socioeconomic features of cooperative societies of fishing production;and 2) To know the diversity of mollusks caught by coastal fishing and the species that have the greatest importance in fishing. From February to April 2013-2015, seven fish coop units, four piers and places where seafood is sold were visited. For the socio-economic analysis and the determination of the species that have more importance in the coastal fishing, a total of 67 surveys were applied. In order to determine the cast of species, the specimens captured by the fishermen were used, which were identified and counted. A total of 67 interviews were analyzed and an average of $1806.00 was estimated as weekly economic income just as a product of fishing, it was also estimated that on average the fishermen work 4.3 days a week. Fisheries cooperatives market most of their produce directly to restaurateurs and locals on the piers. Replacement of older adults by young fishermen from the same region was observed, most are adults between 22 and 39 years old. 25 species of mollusks of economic importance were registered for the coastal fishermen of the Costa Chica. Six species were identified as the main target for catching fishermen. The fishing of mollusks in Costa Chica is not subject to any type of control, which has led to overfishing reflected in the decline sizes and banks in populations of commercial importance.
