AIM:To evaluate the causes of phakic implantable collamer lens(ICL)exchange/explantation in patients with and without keratoconus(KC)at two tertiary hospitals in Riyadh,Saudi ***:A retrospective chart review of all patients who underwent ICL(model V4 c with central port)exchange/explantation was performed using the electronic medical record *** available preoperative and postoperative data were documented for each ***:Over 7 y,2283 ICL implantation procedures were performed;46 implants(2%)required exchange(21 implants)/explantation(25 implants),of which 14 cases(30.4%)were patients with *** for ICL exchange/explantation in non-KC group were vault measurement,cataract formation,increased intraocular pressure,inaccurate refraction,and patient dissatisfaction in 22(68.75%),4(12.5%),3(9.37%),2(6.25%),and 1(3.12%)case,*** most common indication for ICL exchange/explantation in the KC group was inaccurate vault sizing in 11 patients(78.57%),inaccurate refraction in 2 patients(14.28%),and patient dissatisfaction postoperatively in 1(7.14%)***:ICL implantation results in predictable refractive outcomes over the long term with exchange/explantation rates comparable to previous *** vault size is the most common cause of ICL exchange/explantation among patients with or without KC.

For a 5G wireless communication system,a convolutional deep neural network(CNN)is employed to synthesize a robust channel state estimator(CSE).The proposed CSE extracts channel information from transmit-and-receive pairs through offline training to estimate the channel state ***,it utilizes pilots to offer more helpful information about the communication *** proposedCNN-CSE performance is compared with previously published results for Bidirectional/long short-term memory(BiLSTM/LSTM)NNs-based *** CNN-CSE achieves outstanding performance using sufficient pilots only and loses its functionality at limited pilots compared with BiLSTM and LSTM-based *** three different loss function-based classification layers and the Adam optimization algorithm,a comparative study was conducted to assess the performance of the presented DNNs-based *** BiLSTM-CSE outperforms LSTM,CNN,conventional least squares(LS),and minimum mean square error(MMSE)*** addition,the computational and learning time complexities for DNN-CSEs are *** estimators are promising for 5G and future communication systems because they can analyze large amounts of data,discover statistical dependencies,learn correlations between features,and generalize the gotten knowledge.

Role of antibiotic therapy in bacterial disease：A mathematical study

Pathogenic bacteria in human system mature through the bin-synthesis of protective layer known as cell wall. This bacterial cell wall growth occurs in the presence of enzyme released by it. After maturation by the cell wall formation, pathogenic bacteria become harmful for human body as they are responsible for different diseases. Antibiotics or drugs are employed for curing bacterial diseases through the inhibition of this maturation process and it occurs by the binding progression of antibiotics with the released enzyme. But nowadays, drugs or antibiotics like β-lactum family （Amoxcillin） which are generally used for inhibition of bin-synthesis of cell wall become ineffective due to evolution of antibiotic resistance by the bacteria. Antibiotic resistance occurs when an antibiotic has lost its ability to effectively control or kill bacterial growth. As a result, the bacteria becomes ＂resistant＂ and continue to multiply for the generation of robust pathogenic bacteria in spite of drug administration. This is due to the release of another type of enzyme by the resistant bacteria which binds with the active antibiotic or drug making it ineffective. Hence, another type of drug （Clauvanic acid） is combined to resist the activity of drug hydrolyzing enzyme so that the initial drug can act effectively. Hence a combination of drug therapy is applied to cure the bacterial diseases successfully. We developed a mathematical model based on the bacterial enzyme and bacterial cell wall proliferation mechanism and showed how we can reduce the bacterial infection in the resistant cases with application of combination drugs （Amoxcillin and Clauvanic acid） to restore normal health. Based on the enzymatic activity and individual drug dynamics we studied the overall system under the single drug and combinational drug administra- tion through our formulated model analysis. One of the most commonly reported disabilities is vision loss,which can be diagnosed by an ophthalmologist in order to determine the visual system of a *** procedure,however,usually requires an appointment with an ophthalmologist,which is both time-consuming and expensive *** issues that can arise include a lack of appropriate equipment and trained practitioners,especially in rural *** on a cognitively motivated attribute extraction and speech recognition approach,this paper proposes a novel idea that immediately determines the eyesight *** proposed system uses an adaptive filter bank with weighted mel frequency cepstral coefficients for feature *** adaptive filter bank implementation is inspired by the principle of spectrum sensing in cognitive radio that is aware of its environment and adapts to statistical variations in the input stimuli by learning from the *** performance evaluation demonstrates the potential of our automated visual acuity test method to achieve comparable results to the clinical ground truth,established by the expert ophthalmologist’s *** overall accuracy achieved by the proposed model when compared with the expert ophthalmologist test is 91.875%.The proposed method potentially offers a second opinion to ophthalmologists,and serves as a cost-effective pre-screening test to predict eyesight loss at an early stage.
