Effects of a Plantago ovata-based herbal compound in prevention and treatment of oral mucositis in patients with breast cancer receiving chemotherapy: A double-blind, randomized, controlled crossover trial

Background: Oral mucositis(OM) is one of the most common complications of mucotoxic cancer therapy.Mucositis induces clinically significant pain, increases the risk of infections and affects the patients’ quality of life.Objective: This study investigated the effects of an herbal preparation from Plantago ovata hydrocolloid in the prevention and treatment of OM, in breast cancer patients undergoing chemotherapy with a regimen including adriamycin.Design, setting, participants, and interventions: This research was a double-blind, randomized, controlled crossover trial. The herbal compound consisted of a mixture of 500 mg of P. ovate husk in 30 m L water plus three drops of vinegar per dose, which was used as a mouthwash. Phytochemical and physicochemical tests of the compound were also performed. Twenty-eight patients who developed mucositis during the chemotherapy screening cycle were randomized to the herbal compound(n = 14) and placebo(n = 14) groups. They received herbal compound or placebo three times per day during their next chemotherapy cycle(cycle 1 of treatment). Patients were crossed over during cycle 2 of treatment and received the alternative therapy. An oral care protocol was prescribed to all patients in cycles 1 and 2 of the treatment.Main outcome measures: The patients were visited at baseline, the end of the first and second weeks of the screening cycle, and the end of the first and second weeks of each of two treatment cycles. The degree of mucositis was used as the main treatment outcome. Other indexes, such as the severity of pain, xerostomia grade and the quality of life were also measured.Results: Compared with the placebo, the herbal compound significantly reduced the degree of mucositis,the severity of pain and the xerostomia grade;it also improved the patients’ quality of life(P < 0.05).Comparison between the screening cycle and placebo treatment group showed that the oral care protocol had a significant effect in the reduction of OM(P < 0.05).Conclusion: The oral care protocol and the he

Concept of Artificial Intelligence (AI) and Its Use in Orthopaedic Practice: Applications and Pitfalls: A Narrative Review

Background: The growth and use of Artificial Intelligence (AI) in the medical field is rapidly rising. AI is exhibiting a practical tool in the healthcare industry in patient care. The objective of this current review is to assess and analyze the use of AI and its use in orthopedic practice, as well as its applications, limitations, and pitfalls. Methods: A review of all relevant databases such as EMBASE, Cochrane Database of Systematic Reviews, MEDLINE, Science Citation Index, Scopus, and Web of Science with keywords of AI, orthopedic surgery, applications, and drawbacks. All related articles on AI and orthopaedic practice were reviewed. A total of 3210 articles were included in the review. Results: The data from 351 studies were analyzed where in orthopedic surgery. AI is being used for diagnostic procedures, radiological diagnosis, models of clinical care, and utilization of hospital and bed resources. AI has also taken a chunk of share in assisted robotic orthopaedic surgery. Conclusions: AI has now become part of the orthopedic practice and will further increase its stake in the healthcare industry. Nonetheless, clinicians should remain aware of AI’s serious limitations and pitfalls and consider the drawbacks and errors in its use.

Global health (GH) aims to improve healthcare for all people on the planet and eradicate all avoidable diseases and deaths. The inception of Artificial Intelligence (AI) is innovating healthcare practices and improving patient outcomes by shuffling enormous volumes of health data—from health records and clinical studies to genetic information analyzing it much faster than humans. AI also helps in the improvement of medical imaging and medical diagnosis. There is an increased optimism regarding the use of applications of AI locally but can these facets be translated globally in the advancement and delivery of healthcare with the help of AI. At present majority of AI developments and applications in health care provide to the needs of developed countries and there is little effort to develop programs which could help to improve healthcare delivery globally. We performed this narrative review to assess the difficulties and discrepancies in implementing AI in global health delivery and find ways to improve.

Molecular Phylogeny, Species Delimitation and Taxonomic Revision of the Australian Spider Wasp Genus Heterodontonyx Haupt, 1935 (Hymenoptera: Pompilidae)

Heterodontonyx (Pompilidae: Pepsinae) is an understudied genus with Australasiandistribution and most species are endemic to Australia. There have recently been sometaxonomic changes involving the genera Heterodontonyx and Cryptocheilus based on molecular and morphological evidence, however, phylogenetic relationships within Heterodontonyx, species delimitation and formal revision have not been conducted. Here, we construct a Maximum Likelihood phylogeny estimate of Heterodontonyx based on ITS1, 5.8S, ITS2 region of 30 samples representing seven Heterodontonyx morphospecies. bPTP species delimitation approach was employed and ultimately the taxonomic revision of the genus was carried out. Phylogenetic analyses strongly support the monophyly of Heterodontonyx and the species delimitation method recognizes eight species among which one new species is identified.

多类混凝土损伤比强度理论及塑性-损伤模型研究进展与应用

为完善混凝土强度理论和塑性-损伤模型，通过参考岩石损伤比强度理论，根据现有普通混凝土、再生混凝土、轻骨料混凝土和纤维混凝土等多类混凝土多轴强度试验数据，推荐损伤比变量中的五经验系数取值，完善多类混凝土损伤比强度理论并揭示约束混凝土工作原理。分析表明，随着静水压力的增加，混凝土压损伤比将由单轴受压时为1左右线性递减至小于0.5，八面体剪应力先增大后减小，轴向峰值应力提升为某一定值，压损伤比取值减小引发非弹性体积膨胀减小至不变，因而导致混凝土由单轴受压脆性破坏向多轴受压塑性破坏转变，该理论为钢管混凝土柱中发挥混凝土耗能潜力提供理论依据。依据混凝土损伤比强度理论，确定多类混凝土塑性-损伤模型中的三轴强度参数，包括膨胀角、拉压子午线强度比值和二轴等压与单轴抗压强度比值，并建议常温静力荷载下多类混凝土的单轴受压、受拉应力-应变曲线方程及其参数表达式，常温地震荷载下普通混凝土的单轴受压、受拉应力-应变曲线方程及其参数表达式，以及火灾升温环境下普通混凝土的单轴受压、受拉应力-应变曲线方程及其参数表达式，建立约束混凝土三轴塑性-损伤模型。介绍多类混凝土塑性-损伤模型在钢-混凝土组合结构有限元模型中的应用，模型中混凝土采用实体单元而钢管与钢梁采用壳单元，可模拟钢管与混凝土之间的界面滑移与约束作用，当采取增强约束拉筋强柱构造方法时可提升钢管混凝土柱及其结构的承载力、抗震与抗火性能。

Flow and passive scalar transfer characteristics around a row of interacting tandem cylinders

Transfer and transport of a passive scalar as well as flow structures of unbounded flow over a row of 2,3,4 and 5 equispaced identical cylinders at Re=90 and pitch ratios 1.05D≤S≤7D are investigated.Studies are performed through high fidelity simulations using a novel sharp-interface immersed boundary method.Five flow regimes are identified based on the nature of flow in the gap between the first two cylinders:dormant in the gap where fluid in the gap is motionless;symmetric in the gap where fluid in the gap behaves like a cavity flow;alternating in the gap where fluid in the gap is trapped but flaps up and down;wake in the gap where periodic vortices are shed in the gap;and bi-stable flow regime where vortices are shed in the gap for some time and then quasi-periodic twin vortices prevail for other times.A jump in values of hydrodynamic forces,pressure coefficient,Strouhal number,and Sherwood number is observed as pitch ratio increases due to change in flow structure.At high pitch ratio,a convectively unstable two-row vortical train is formed by the second cylinder.Member cylinders immersed in the two-row vortical structures have no effect on flow structure downstream or upstream and portray poor scalar transfer characteristics.The effect of configuration size on flow structures is found not to be as prominent as that of pitch ratio.

Groundwater discharge drives water quality and greenhouse gas emissions in a tidal wetland

Wetlands play an important role in the global carbon cycle as they can be sources or sinks for greenhouse gases.Groundwater discharge into wetlands can affect the water chemistry and act as a source of dissolved greenhouse gases,including CO_(2)and CH_(4).In this study,surface water quality parameters and CO_(2)and CH_(4)concentrations were evaluated in a tidal wetland(Hunter Wetlands National Park,Australia)using time series measurements.Radon(^(222)Rn),a natural ground water tracer,was used to investigate the role of groundwater as a pathway for transporting dissolved CO_(2)and CH_(4)into the wetland.In addition,water-to-air CO_(2)and CH_(4)fluxes from the wetland were also estimated.The results showed a high concentration of radon in wetland surface water,indicating the occurrence of ground water discharge.Radon concentration had a strong negative relationship with water depth with a determination coefficient(R^(2))of 0.7,indicating that tidal pumping was the main driver of groundwater discharge to the wetland.Radon concentration also showed a positive relationship with CO_(2)and CH_(4)concentrations(R^(2)=0.4 and 0.5,respectively),while the time series data revealed that radon,CO_(2),and CH_(4)concentrations peaked concurrently during low tides.This implied that groundwater discharge was a source of CO_(2)and CH_(4)to the wetland.The wetland had an average water-to-air CO_(2)flux of 99.1 mmol/(m^(2)·d),twice higher than the global average CO_(2)flux from wetlands.The average CH_(4)flux from the wetland was estimated to be 0.3 mmol/(m^(2)d),which is at the higher end of the global CH_(4)flux range for wetlands.The results showed that groundwater discharge could be an important,yet unaccounted source of CO_(2)and CH_(4)to tidal wetlands.This work has implications for tidal wetland carbon budgets and emphasizes the role of groundwater as a subsurface pathway for carbon transport.

Host-guest inclusion complexes have been extensively studied for drug delivery applications.The host molecule,typically cyclodextrin,creates a cavity,in which the guest molecule,such as a drug,can be encapsulated.This encapsulation can protect the drug from degradation,improve its solubility and stability,and enhance its bioavailability.Moreover,host-guest inclusion complexes can facilitate targeted drug delivery by selectively releasing drugs at the site of action.Various techniques,such as covalent bonding,non-covalent interactions,and self-assembly have been used to form these complexes.Host-guest inclusion complexes have shown great potential for improving the efficacy and safety of drug delivery systems.This mini review summarizes the application and recent progress of the cyclodextrin-based host-guest inclusion complex,highlighting the enhanced physicochemical and biopharmaceutical application of pharmaceutical drugs via formulation of its inclusion complex.

Quercetin protects liver injury induced by bile duct ligation via attenuation of Rac1 and NADPH oxidase1 expression in rats

BACKGROUND： Bile duct ligation （BDL） and subsequent cholestasis are correlated with oxidative stress, hepatocellular injury and fibrosis. Quercetin is a flavonoid with antifibrotic, and hepatoprotective properties. However, the molecular mechanism underlying quercetin-mediated hepatoprotection is not fully understood. The current study was to evaluate mecha- nisms of hepatoprotective effect of quercetin in BDL rat model. METHODS： We divided male Wistar rats into 4 groups （n=8 for each）： sham, sham＋quercetin （30 mg/kg per day）, BDL, and BDL＋quercetin （30 mg/kg per day）. Four weeks later, the rats were sacrificed, the blood was collected for liver enzyme mea- surements and liver for the measurement of Racl, Racl-GTP and NOX1 mRNA and protein levels by quantitative PCR and Western blotting, respectively. RESULTS： Quercetin significantly alleviated liver injury in BDL rats as evidenced by histology and reduced liver enzymes. Furthermore, the mRNA and protein expression of Racl, Racl-GTP and NOX1 were significantly increased in BDL rats compared with those in the sham group （P〈0.05）; quercetin treatment reversed these variables back toward normal （P〈0.05）. Another interesting finding was that the antioxidant markers e.g. superoxide dismutase and catalase were elevated in quercetin-treated BDL rats compared to BDL rats （P〈0.05）. CONCLUSION： Quercetin demonstrated hepatoprotective activity against BDL-induced liver injury through increasing antioxidant capacity of the liver tissue, while preventing the production of Racl, Racl-GTP and NOX1 proteins.

Persian medicine non-pharmacological therapies for headache：phlebotomy and wet cupping

OBJECTIVE: To describe the common causes of headache as well as the different methods of Phlebotomy(Fasd) and wet Cupping(Hijamat) based on Persian Medicine.METHODS: In this study, we searched Google Scholar, Science Direct, Pub Med and Scopus to get any in vitro or clinical evidence of the efficacy and possible mechanisms of phlebotomy or wet cupping on different types of headaches. Also different types of headaches, according to PM, under the title of "Sodaa"; extracted from famous Persian medical text books with key words including "Sodaa","Bayze" and "Shaqiqe".RESULTS: According to PM, diseases(dystemperament) are created by abnormal alteration in temperament. Temperament is a monolithic quality yielded by interaction between opposing qualities of four elements that are coldness, hotness, dryness and wetness. Headaches are caused by dystemperament(sue-Mizaj) in brain or in total body that are divided into simple and corporal. Common forms of the latter type are Vaporal and Migrant headache(Bokhari and Rihi). Each of these headaches is treated either through phlebotomy or wet cupping.CONCLUSION: In Persian Medicine, Fasd and Hijamat are easy and economical methods for treatment of headache with reasonable results. By describing the precise areas and indications of phlebotomy and wet cupping, this study provides the first step for future clinical trials in this field.
