Social interaction is known to alter behavior and emotional responses to various events. It has been reported that when fear-conditioned animals are put in a fear extinction paradigm with non-fearful conspecifics (pair-exposure), freezing behavior decreases compared to a solitary situation. However, it remains unclear whether pair-exposure during fear extinction is persistently effective in reducing the freezing response. In this study, we examined whether the effect of pair-exposure could be persistently effective on cued and contextual fear extinction. The reduction of the fear compared to the solitary condition was transiently observed only in the cued fear extinction with no difference in the subsequent recall session. We also found that the correlation between corticosterone levels and freezing behavior during extinction was disrupted in the pair-exposure situation. These results suggest that pair-exposure reduces freezing behavior in cued fear extinction, although this fear response reduction is not persistent. The pair-exposure changed an association between corticosterone levels and freezing behavior during extinction.

Cognitive Behavioral Therapy as the Basis for Preventive Intervention in a Sleep Health Program: A Quasi-Experimental Study of E-Mail Newsletters to College Students

Several reports suggest that college students often have atypical sleep patterns and experience poor sleep quality. We examined the effect of a cognitive behavioral therapy (CBT) based intervention program to improve sleep quality and overall mental health among college students. The intervention was delivered in the form of e-mail newsletters. Fifty-three students participated in the intervention group, and another 50 students participated in the control group. The intervention group received a lecture on sleep hygiene;once-weekly e-mail newsletters on sleep health topics (sleep hygiene, stimulus control, sleep restriction, sleep titration, and relapse prevention) and were asked to maintain a four-week sleep diary. The Pittsburgh Sleep Quality Index (PSQI), a measure of sleep quality, and the Kessler 6, a measure of psychological distress, were administered before the lecture (as the baseline measurement) and again 16 weeks later (follow-up measurement). PSQI and K6 scores were reduced in the intervention group compared with the control group. A CBT-based sleep health program utilizing e-mail newsletters may work to improve sleep quality and mental health. This program may represent a cost effective way for Japanese students to receive treatment for poor sleep and may also serve to prevent psychiatric problems.

The diagnosis of gastric metastasis from lung cancer is relatively rare in living patients.We describe a case of Type 4 tumor-like metastasis due to primary lung cancer diagnosed with immunohistochemical staining while the patient was alive.A 68-year-old man was admitted to our hospital because of epigastric pain.Gastrointestinal endoscopy revealed a Type 4 tumor and the histological examination showed poorly differentiated adenocar-cinoma.His chest X-ray showed mass shadow in the right upper lung field.The resected specimens showed moderately differentiated adenocarcinoma.,The diagnosis of gastric metastasis from lung cancer was made by immunohistochemical staining of the lung and gastric tumors which showed positive staining for Thyroid transcriptional factor-1.Diagnosis of gastric metastasis,especially Type 4 metastasis by lung cancer is diff icult.However,immunohistochemical staining is very helpful for diagnosis of primary lung cancer metastasis at sites such as the gastrointestinal tract which are not normally prone to metastatis.

Serum Levels of Brain-Derived Neurotrophic Factor in Patients with Obsessive-Compulsive Disorder in a Japanese Population

A role of lower brain-derived neurotrophic factor (BDNF) content in the pathogenesis of several mental illnesses has been suggested, especially in major depression. It is not known whether BDNF is involved in the pathogenesis of obsessive-compulsive disorder (OCD). Herein, we assessed the serum BDNF content and its correlation with symptom severity in a Japanese population with OCD. The serum BDNF levels of OCD patients (n = 39) and healthy controls (n = 37) were measured by ELISA. The severity of OCD symptoms was assessed by the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) and the Beck Depression Inventory (BDI). The OCD patients’ BDNF levels were significantly higher than those of the controls (17.5 ± 7.3 vs. 12.7 ± 4.7) (p < 0.01). No correlation was observed between the OCD patients’ BDNF levels and their OCD symptoms as scored by the Y-BOCS. For all 76 subjects, the BDI scores were significantly negatively correlated with the serum BDNF levels. Our findings revealed that contrary to previous reports, the serum BDNF content in OCD patients could be higher than that of healthy subjects.

Correlation of prefrontal activity measured by near-infrared spectroscopy (NIRS) with mood, BDNF genotype and serum BDNF level in healthy individuals

Depression has been known to reduce the prefrontal activity associated with the execution of certain cognitive tasks, although whether a temporarily depressed or anxious mood in healthy individuals affects the prefrontal blood oxygen level during cognitive tasks is unknown. Combining the measurement of prefrontal activity with near-infrared spectroscopy (NIRS) and the two cognitive tasks, namely the letter version of the verbal fluency test (VFT-l) and the Stroop test, we measured the effect of a depressed or anxious mood and gender on the changes in the prefrontal oxygenated hemoglobin (Oxy-Hb) levels during those cognitive tests in healthy individuals. Depressed mood or anxious mood was assessed by the Hospital Anxiety and Depression Scale (HADS). Thereby we aimed to explore the possibility of NIRS measurement for detecting the early subclinical manifestation of major depression. Moreover, we examined the possible relationships between prefrontal activation and the functional Val66Met polymorphisms of the brain derived neurotropic factor (BDNF) gene and serum BDNF level. As a result, the increased prefrontal Oxy-Hb levels during cognitive tasks were significantly correlated with the severity of depressed mood in males. The course of the prefrontal Oxy-Hb increase was different depending on the cognitive tasks, i.e., the VFT-l or the Stroop test, in both genders. Correlations of BDNF genotype and serum BDNF level with the prefrontal Oxy-Hb levels during those cognitive tasks were negative. Our results suggest that the early subclinical manifestation of depressed mood in males might be detected by the NIRS measurement, which is not correlated with the individual properties of BDNF.

Closed-Loop Control of an XYZ Micro-Stage and Designing of Mechanical Structure for Reduction in Motion Errors

This paper presents a compact XYZ micro-stage driven by an impact driving mechanism.A moving body is translationally actuated along the X-,Y-,and Z-axes in millimeter-scale range and with nanometer-scale resolution.Cr-N thin-film strain sensors are integrated into the micro-stage for closed-loop positioning.Closed-loop control is also carried out.The motion errors in six degrees of freedom are also investigated for this micro-stage.It is clarified by analysis of finite element method that rotational motion errors around the driving axis are caused by torques due to tensions from the elastic hinges of microstage.The mechanical structure of XYZ micro-stage to cancel out the torque generated is proposed and the second prototype is fabricated.Although the rotational motion error is successfully suppressed in the second prototype,a rotational motion error of more than 0.1°remains due to remaining torque and assembly error of the elastic hinges.Then,in order to reduce the rotational motion error,the prototype is designed in which the location of elastic hinges is single-layered.By designing a mechanical structure in which torque is suppressed,all rotational motion errors are successfully reduced to less than 0.05°in the prototype with single-layer hinges.

Surgical resection of a solitary para-aortic lymph node metastasis from hepatocellular carcinoma

Lymph node (LN) metastases from hepatocellular carcinoma (HCC) are considered uncommon. We describe the surgical resection of a solitary para-aortic LN metastasis from HCC. A 65-year-old Japanese man with B-type liver cirrhosis was admitted for the evaluation of a liver tumor. He had already undergone radiofrequency ablation, transcatheter arterial chemoembolization, and percutaneous ethanol injection therapy for HCC. Despite treatment, viable regions remained in segments 4 and 8. We performed a right paramedian sectionectomy with partial resection of the left paramedian section of the liver. Six months later, serum concentrations of alpha-fetoprotein (189 ng/mL) and PIVKA-2 (507 mAU/mL) increased. Enhanced com- puted tomography of the abdomen revealed a tumor (20 mm in diameter) on the right side of the abdominal aorta. Fluorine-18 fluorodeoxyglucose positron emission tomography revealed an increased standard uptake value. There was no evidence of recurrence in other regions. Esophagogastroduodenoscopy and colonoscopy revealed no malignant tumor in the gastrointestinal tract. Para-aortic LN metastasis from HCC was thus diagnosed. We performed lymphadenectomy. Histopathological examination revealed that the tumor was largely necrotic, with poorly differentiated HCC on its surface, which confirmed the suspected diagnosis. After 6 mo tumor marker levels were normal, with no evidence of recurrence. Our experience suggests that a solitary para-aortic LN metastasis from HCC can be treated surgically.

BACKGROUND Inflammatory bowel disease(IBD)is a chronic,relapsing inflammation of the digestive tract.Although fecal and serum biomarkers have been extremely important and supportive for monitoring of IBD,their low sensitivity and high variability characteristics limit clinical efficacy.Thus,the establishment of better biomarkers is expected.Fucosylation is one of the most important glycosylation modifications of proteins.Fucosylated haptoglobin(Fuc-Hpt)is used as a biomarker for several cancers and inflammation-related diseases.We recently established a novel glycan monoclonal antibody(mAb),designated 10-7G,which recognizes Fuc-Hpt.We developed an enzyme-linked immunosorbent assay(ELISA)to measure serum levels of Fuc-Hpt(10-7G values).AIM To investigate the usefulness of the serum 10-7G values as a potential biomarker for monitoring disease activity in IBD.METHODS This was a case control study.Intestinal tissues of IBD patients(n=10)were examined immunohistochemically using the 10-7G mAb.We determined 10-7G values using serum from patients with ulcerative colitis(UC,n=110),Crohn’s disease(n=45),acute enteritis(AE,n=11),and healthy volunteers(HVs)who exhibited normal(n=20)or high(n=79)C-reactive protein(CRP)levels at medical check-up.We investigated the correlation between the 10-7G value and various clinical parameters of IBD patients by correlation analysis.Receiver operating characteristic(ROC)curve analysis was performed to evaluate the usefulness of the 10-7G values as a biomarker for clinical and endoscopic remission of UC compared to conventional serum biomarkers.RESULTS In the immunohistochemical analysis,positive 10-7G mAb staining was observed in lymphocytes infiltrating into inflammatory sites of the mucosal layer and lymphoid follicles.The 10-7G values were significantly higher in patients with IBD(P<0.001)and AE(P<0.05)compared with HVs.In addition,10-7G values were correlated with clinical examination parameters related to inflammation in patients with UC,particularly the CRP level(rs=0.525,P=0.003)a
