To present a patient diagnosed with pancreatic carcinoidthat was extremely rare and produced an atypical carcinoidsyndrome. We reported a 58-year old male patient whopresented with long standing, prominent cervicallymphadenopathy and occasional watery *** and immunohistochemical examinationof lymph node biopsy showed a metastatic neuroendocrinetumor, which was histological type A of carcinoid (EMA+,cytokeratin+, CEA-, NSE+, chromogranin A+, synaptophysin+,insulin-). Bone marrow biopsy showed identical *** site of the tumor was pancreas and diagnosis wasmade according to cytological and immunocytochemicalanalysis of the tumor cells obtained with aspiration biopsyof pancreatic mass (12 mm in diameter) under endoscopicultrasound guidance. However, serotonin levels in bloodand urine samples were normal. it is difficulty to establishthe precise diagnosis of a "functionally inactive" pancreaticcarcinoid and aspiration biopsy of pancreatic tumor underendoscopic ultrasound guidance can be used as a newpotent diagnostic tool.

Effect of opioid dependence or abuse on opioid utilization after shoulder arthroplasty

AiM To examine whether opioid dependence or abuse has an effect on opioid utilization after anatomic or reverse total shoulder arthroplasty(TSA).METHODS All anatomic TSA(iCD-9 81.80) and reverse shoulder arthroplasty(RSA)(iCD-9 81.88) procedures from 2007 to 2015 were queried from within the Humana claims database utilizing the Pearl Diver supercomputer(Colorado Springs, CO). Study groups were formed based on the presence or absence of a previous history of opioid dependence(iCD-9 304.00 and 304.03) or abuse(iCD-9 305.50 and 305.53). Opioid utilization among the groups was tracked monthly up to 1 year post-operatively utilizing National Drug Codes. A secondary analysis was performed to determine risk factors for pre-operative opioid dependence or *** Two percent of TSA(157 out of 7838) and 3% of RSA(206 out of 6920) patients had a history of opioid dependence or abuse. For both TSA and RSA, opioid utilization was significantly higher in opioid dependent patients at all post-operative intervals(P incidence of opioid use among groups was similar within the first post-operative month. After TSA, opioid dependent patients were over twice as likely to fill opioid prescriptions during the post-operative months 1-12. Following RSA, opioid dependent patients were over 3 times as likely to utilize opioids from months 3-12. Age less than 65 years, history of mood disorder, and history of chronic pain were significant risk factors for pre-operative opioid dependence/abuse in patients who underwent TSA or *** Following shoulder arthroplasty, opioid use between opioid-dependent and non-dependent patients is similar within the first post-operative month but is greater among opioid-dependent patients from months 2-12.

Microwave-assisted irradiation was performed for esterification of chicory inulin with high degree of polymerization with acetic anhydride without a solvent only with a *** resulting esters were characterized by melting point,hydrophilic-lipophilic balance,thin-layer chromatography,ultraviolet spectroscopy,Fourier transform infrared spectroscopy(FTiR)and nuclear magnetic resonance(NMR)*** acetate demonstrated a high degree of acetylation(2.5-3.0)and presented a white,water-insoluble substance with bitter *** FTiR and NMR spectra confirmed esterification and demonstrated the incorporation of hydrophobic residue to the water soluble inulin *** capacity,water holding,oil-holding capacities,the foamability,foam stability and emulsifying properties were also *** acetate showed promising foam stability 52%for 60 min and formed stable emulsions at concentration 0.2 g/L with 50 and 80%oil *** water holding capacity was lower than the oil holding *** addition to this,for the first time,the antimicrobial potential of inulin acetate was tested against seventeen microorganisms(Gram-positive and Gram-negative bacteria,yeasts and fungi).inulin acetate(10 mg/ml)inhibited the growth of Bacillus cereus,Escherichia coli ATCC 8739,Salmonella abony,Candida albicans and Penicillium ***,inulin acetate demonstrated antimicrobial activity at concentration 1 mg/ml against Listeria monocytogenes 863,Escherichia coli 3398,Candida albicans 8673,Fusarium oxysporum and Aspergillus *** current study demonstrated the applications of“green”synthesized inulin acetate as a foaming agent,oil-in-water emulsion stabilizer and antimicrobial substance in pharmaceutical,agricultural and cosmetic preparations.
