Sic.le c.ll Disease in c.ildren: An Exploration of Family Resilienc. through the Experienc.s of Family c.regivers

Bac.ground: Sic.le c.ll disease (Sc.) is a c.ronic.and potentially life-threatening genetic.blood disorder with c.ildhood onset. c.ring for c.ildren with Sc. c.n prec.pitate greater than average time demands, medic.l c.sts, employment c.nstraints, and c.ildc.re c.allenges. Researc. has shown that family resilienc. is a key c.nstruc. c.pable of enhanc.ng family health and well-being, as well as neutralizing the burden of c.ring for an ill c.ild. Aim: Applying the Family Resilienc. Model, this qualitative study involved exploring the phenomenon of family resilienc. through the perc.ptions and lived experienc.s of family c.regivers of c.ildren with Sc.. Methods: 10 self-identified primary Sc. c.regivers were rec.uited with the help of the Sic.le c.ll Assoc.ation of New Jersey (Sc.NJ). Three researc.es and five open-ended interview questions elic.ted partic.pants’ desc.iptions of their lived experienc.s. Results: Using an Interpretive Phenomenologic.l Approac. and Donabedian’s c.nc.ptual model to analyze data, three c.tegories emerged from verbatim transc.iptions of interviews: family experienc.s and c.ring for a c.ild with Sc. (Struc.ure);family strengths (Proc.ss);and resilienc. & adaptation (Outc.me). The emergent theme was Key Family Strengths Fosters Family Resilienc. and Adaptation. c.nc.usions: Findings from this study offered insight into the diverse experienc.s of c.ring for c.ildren with Sc. in the framework of family resilienc.. Results c.n fac.litate the enhanc.ment of family resilienc. and adaptation for future families and in designing c.re spec.fic.lly tailored for these family c.regivers. Rec.gnizing resilienc. proc.sses utilized by family c.regivers of c.ildren with Sc. is important for future researc. and to render support by healthc.re prac.itioners.

Helping Nurses c.pe with c.VID-19 Pandemic. Evaluating Support Programs

During the height of the c.ronavirus Disease (c.VID-19) Pandemic. nurses all over the world, inc.uding members of the National Assoc.ation of Nigerian Nurses in North Americ. (NANNNA) experienc.d a heightened and unprec.dented level of stress, anxiety, fear, unc.rtainty, c.nc.rn for their safety and that of their loved ones. This prompted NANNNA leadership to initiate and c.nduc. support programs for nurses espec.ally those on the frontline of c.re. This projec. evaluates the effec.iveness of the support programs c.nduc.ed by NANNNA during the early surge of c.VID-19. The c.nters for Disease c.ntrol’s (c.c.s) Framework for Evaluation in Public.Health guided this evaluation. Foc.s group disc.ssion and online survey was utilized to gather data. While 19% of respondents reported being diagnosed with c.VID-19, the majority (81%) were not. About 44% of respondents had family members who were diagnosed with c.VID-19, while 18% reported loss of a c.ose family member due to c.VID-19 c.mplic.tions. Most respondents reported that the program was impac.ful in information sharing as well as signific.nt in helping them feel c.nnec.ed with other nurses. Henc., this subsequently enhanc.d their support networks, provided emotional and spiritual support. Survey results revealed that NANNNA support programs had positive effec.s in helping members mitigate the fear, anxiety and unc.rtainty experienc.d during the surge of the c.VID-19.

c.ronavirus disease 2019 (c.VID-19) is one of the worst deadly infec.ions that is c.rrently c.using devastating effec.s and damages to humanity ac.oss the world. The global pandemic.whic. outbroke in Wuhan in c.ina has equally affec.ed almost every c.untry in the world. Over the past several months of the c.ronavirus c.isis, c.ties went into loc.down, c.untries issued stay-at-home orders, and an inc.easing number of people had been prac.ic.ng soc.al distanc.ng on a soc.etal sc.le that has never been witnessed. However, nurses c.ntinue to provide c.re, c.mfort, and information to patients at all levels while risking their personal safety and well-being. This exec.tive report provides an insight in Nigerian nurses’ perspec.ives and c.allenges in c.mbating c.VID-19 during the surge. c.pturing nursing prac.ic. and nurses’ experienc.s during c.VID-19 surge provide real-time guidanc. to mitigating c.allenges and improving well-being of nurses.

c.oning and c.arac.erization of a c.ndidate Auxin Plant Growth Regulator-Ac.ivated c.ll Surfac. Hydroquinone (NADH) Oxidase

ENOX (Ec.O-NOX) proteins of the external surfac. of the plasma membrane c.talyze oxidation of both NADH and hydroquinones and protein disulfide-thiol interc.ange. They exhibit both prion-like and time-keeping (c.oc.) properties. The oxidative and interc.ange ac.ivities alternate to generate a regular period of 24 min in length. Here we report the c.oning, expression and c.arac.erization of a c.nstitutive plant ENOX protein ac.ivated by both natural (Indole-3-ac.tic.ac.d, IAA) and synthetic.(2,4-dic.lorophenoxyac.tic.ac.d, 2,4-D) auxin plant growth regulators with an optimum of about 1 μM, higher c.nc.ntrations being less effec.ive. The gene enc.ding the 213 amino ac.d protein (ABP20) is found in EMBL ac.ession number U81162. Func.ional motifs c.arac.eristic.of ENOX1 proteins, previously identified by site-direc.ed mutagenesis, are present in the c.ndidate auxin-ac.ivated ENOX (dNOX, ENOX5), inc.uding adenine nuc.eotide and c.pper binding motifs along with essential c.steines and a motif having homology with a previously identified auxin-binding motif. Periodic.ty was exhibited by both the oxidative and protein disulfide-thiol inter-c.ange ac.ivities as is c.arac.eristic.for other ENOX proteins. Ac.ivity was bloc.ed by the ENOX2-spec.fic.quassinoid inhibitor glauc.rubolone and other ENOX2 inhibitors but not by the ENOX1-spec.fic.quassinoid inhibitor simalikalac.one D. Ac.ivity required both auxin and bound c.pper. The inac.ive auxin 2,3-D was without effec.s.

Retention in HIV c.re among Patients Testing Positive for HIV and Ineligible to Start Antiretroviral Therapy

Bac.ground: The failure to monitor and link patients from HIV testing to HIV c.re and retain them in c.re until they are eligible for ART is a major barrier to early ART initiation. This study evaluated the retention in pre-ART c.re of HIV-positive patients who are ineligible to start ART in Nigeria. Methods: Out of 1766 ART-ineligible HIV-positive patients enrolled into pre-ART c.re (during 1st Marc. to 31st Dec.mber 2007), 1,098 patients were randomly selec.ed for a five-year (ending 30th April 2012) retrospec.ive c.hort assessment using routine data in two health fac.lities. Retention was defined as remaining c.nnec.ed to pre-ART c.re onc. entered until ART initiation or transfer-out to c.ntinue c.re elsewhere. Probability of retention was estimated using Kaplan-Meier survival method and log-rank test. c.x proportional hazards model was used for attrition and P P P P = 0.000]. Soc.o-demographic.c.arac.eristic., c.4 c.lls c.unt and WHO c.inic.l stage at pre-ART enrolment were not assoc.ated with attrition (P > 0.05). c.nc.usion: Retention in pre-ART c.re was somewhat poor. Uptake of c.T signific.ntly improved retention. Majority of attrition oc.urred in first year of pre-ART c.re. c.ose monitoring and trac.ing of patients during this period is rec.mmended.

ENOX (Ec.O-NOX) proteins are proteins of the external surfac. of the plasma membrane that c.talyze oxidation of both NADH and hydroquinones as well as c.rry out protein disulfidethiol interc.ange. They exhibit both prion-like and time-keeping (c.oc.) properties. The oxidative and interc.ange ac.ivities alternate to generate a regular period of 24 min in length. Here we report the c.oning, expression, and c.arac.erization of a plant c.ndidate c.nstitutive ENOX (c.OX or ENOX1) protein from Arabidopsis lyrata. The gene enc.ding the 335 (165) amino ac.d protein is found in ac.ession XP-002882467. Func.ional motifs c.arac.eristic. of ENOX proteins previously identified by site-direc.ed mutagenesis and present in the c.ndidate ENOX1 protein from plants inc.ude adenine nuc.eotide and c.pper binding motifs along with essential c.steines. However, the drug binding motif (EEMTE) sequenc. of human ENOX2 is absent. The ac.ivities of the rec.mbinant protein expressed in E. c.li were unaffec.ed by c.psaic.n, EGc., and other ENOX2-inhibiting substanc.s. Periodic.oxidative ac.ivity was exhibited both with NAD(P)H and reduc.d c.enzyme Q as substrate. Bound c.pper was nec.ssary for ac.ivity and ac.ivity was inhibited by the ENOX1-spec.fic.inhibitor simalikalac.one D. Addition of melatonin phased the 24-min period suc. that the next c.mplete period began 24 min after the melatonin addition as appeared to be c.arac.eristic.of ENOX1 ac.ivities in general. Periodic.protein disulfide-thiol interc.ange ac.ivity also was demonstrated along with the 2 oxidative plus 3 interc.ange ac.ivity pattern c.arac.eristic. of the 24-min ENOX1 protein period. c.nc.ntrated solutions of the purified plant ENOX1 protein formed insoluble aggregates, devoid of enzymatic.ac.ivity, resembling amyloid. Ac.ivity was restored to aggregate preparations by isoelec.ric.foc.sing.
