Anatomical features associated with venous congestion in DIEP flap using CT angiography with three-dimensional reconstruction

Aim: Computed tomography angiography (CTA) using three-dimensional (3D) virtual reconstruction has been increasingly used in planning deep inferior epigastric artery perforator (DIEP) breast reconstruction. Although the most common complication associated with this surgery is diffuse venous congestion, its origin remains unclear. The aim of this study was to assess the anatomical characteristics of the anterior abdominal wall vessels that could predict venous congestion, using CTA with 3D virtual reconstruction. Methods: A retrospective case-control study was conducted and a total of 169 DIEP flaps were reviewed. An abdominal CTA with 3D virtual reconstruction was analyzed with regard to anatomical features of the abdominal wall vessels. Seven venous congestive cases were identified. For each case, 3 controls that had not exhibited any vascular complications were randomly selected. Results: The global venous congestion rate was 4.14%. No statistically significant differences were found between the groups' superficial inferior epigastric vein (SIEV) diameter (P = 0.915), number of branches of SIEV (P = 0.371), number of perforators per flap (P = 0.255), flap subcutaneous tissue thickness (P = 0.652), direct communications between SIEV-perforators (P = 0.418), and communications of both SIEVs across the abdominal midline (P = 0.371). Conclusion: The present study provided new information concerning the identification of the controversial anatomical features associated with venous congestion in DIEP flaps. CTA and 3D virtual reconstruction were useful tools for evaluating the abdominal wall anatomy and for planning DIEP breast surgery, but neither for predicting nor preventing the diffuse congestive phenomenon.

Current role in facial allograft transplantation:what have we learned?

Face transplant(FT)has evolved enormously in the last 10 years since the successful completion of the first facial transplant.This procedure has become a new reconstructive option for complex facial deformities to restore the anatomy of patients with severely disfigured faces.The authors review the literature and discuss the main surgical,immunological,and ethical aspects as well as the results described in patients undergoing FT.To date there have been more than thirty FT worldwide.The main indication was post-traumatic deformity.In all cases a standard immunosuppression was performed with three drugs,although acute rejection episodes were observed,that could be controlled with conventional immunosuppressive regimen.Overall,functional and aesthetic results have been excellent at short-term and high satisfaction rate exceeded initial expectations,although long-term data are still scarce.Major complications were opportunistic infections.Five deaths that occurred have reopened the ethical debate about the potential complications and concerns of providing informed consent to recipients.Continuous progresses in microsurgical techniques and preoperative planning have promoted the evolution from partial to full FT.All these are on the basis of accurate and careful selection of well-motivated candidates.The next challenge will be getting new immunosuppressive treatment strategies.Although clinical experience has demonstrated the FT viability,it is still considered an experimental procedure in which we have much to learn to define its true role in the current reconstructive surgery and resolve major technical,medical and ethical problems involved.

AIM:To provide a comprehensive and more representative national data on the disease,especially on treatment options and outcomes,and to determine access of retinoblastoma patients from Luzon,Visayas and Mindanao to eye care,and determine if access is associated with delay in consultation,staging and outcomes.METHODS:Cohort study of retinoblastoma patients seen in eleven institutions located in the three major areas of the Philippines namely Luzon,Vizayas and Mindanao from 2010-2020.RESULTS:Totally 636 patients,involving 821 eyes,were included.Majority(57%)were from Luzon and were seen in institutions in Luzon(72%).Annually,58±10 new cases were seen with 71%having unilateral disease.Median delay of consultation remained long at 9(3,17)mo,longest in patients with unilateral disease(P<0.02)and those from the Visayas(P<0.003).Based on the International Retinoblastoma Staging System,only 35%of patients had Stage 1 while 47%already had extraocular disease.Enucleation was the most common treatment received by 484 patients while intravenous chemotherapy was received by 469.There were 250(39%)patients alive,195(31%)dead,85(13%)abandoned,17(3%)refused and 89(14%)with no data.CONCLUSION:This study presents the largest cohort of retinoblastoma patients in the Philippines in terms of patients’and participating institutions’number and geographical location and type of institution(private and public).It also presents more comprehensive data on the treatments used and outcomes(survival,globe salvage,and vision retention rates).Delay in consultation was still long among patients leading to advanced disease stage and lower survival rate.Despite increasing capacity to diagnose and manage retinoblastoma in the country,the delay of consultation remains long primarily due to accessibility issues to eye care institutions especially in the Visayas and financial concerns.The delay was still significant that overall survival rate remain low.

Role of the combination of biologics and/or small molecules in the treatment of patients with inflammatory bowel disease

Inflammatory bowel disease(IBD)is a group of chronic diseases that includes ulcerative colitis,Crohn’s disease,and indeterminate colitis.Patients with IBD require prolonged treatment and high utilization of healthcare resources for proper management.The treatment of patients with IBD is focused on achieving therapeutic goals including clinical,biochemical,and endoscopic variables that result in improvement of the quality of life and prevention of disability.Advanced IBD treatment includes tumor necrosis factor inhibitors,integrin antagonist,antagonist of the p40 subunit of interleukin 12/23,and small molecule drugs.However,despite the multiple treatments available,about 40%of patients are refractory to therapy and present with persistent symptoms that have a great impact on their quality of life,with hospitalization and surgery being necessary in many cases.Dual therapy,a strategy sometimes applicable to refractory IBD patients,includes the combination of two biologics or a biologic in combination with a small molecule drug.There are two distinct scenarios in IBD patients in which this approach can be used:(1)Refractory active luminal disease without extraintestinal manifestations;and(2)patients with IBD in remission,but with active extraintestinal manifestations or immune-mediated inflammatory diseases.This review provides a summary of the results(clinical response and remission)of different combinations of advanced drugs in patients with IBD,both in adults and in the pediatric population.In addition,the safety profile of different combinations of dual therapy is analyzed.The use of newer combinations,including recently approved treatments,the application of new biomarkers and artificial intelligence,and clinical trials to establish effectiveness during long-term followup,are needed to establish new strategies for the use of advanced treatments in patients with refractory IBD.

Lilly Reich:The architecture and critique of an invisibilized woman

The architectural modernity that prevailed in Germany during the first half of the twentieth century cannot be understood without an appreciation of the work of Lilly Reich,a brilliant designer of interiors,furniture,and ephemeral architecture who exerted significant influence from her active position as a member of the Deutscher Werkbund.Despite the critical recognition she acquired in the media of the time during her solo career,her association with the architect Mies van der Rohe,far from positioning her in the field of contributions,relegated her to the role of a mere collaborator.For this reason,this article aims to shed light on the extensive legacy of Lilly Reich,a woman whose figure is being acknowledged as a result of recent research but who was nevertheless omitted from the leading books on the history of architecture.The methodology employed in the research analyses comparatively“what she did”and“what was said about her”,highlighting the convergences and divergences between“work”and“criticism”.The study and its results yield interesting conclusions regarding both the recognition that Lilly Reich received during her lifetime and the criticism she received after her death and up to the present day.

Drones are increasingly used for fauna monitoring and wildlife tracking;however,their application for wildlife tracking is restricted by developing such systems.Here we explore the potential of drones for wildlife tracking using an off-the-shelf system that is easy to use by non-specialists consisting of a multirotor drone,smartphones,and commercial tracking devices via Bluetooth and Ultra-Wide Band(UWB).We present the system configuration,explore the operational parameters that can affect detection capabilities,and test the effectiveness of the system for locating targets by simulating target animals in savanna and forest environments.The self-contained tracking system was built without hardware or software customization.In 40 tracking flights carried out in the Brazilian Cerrado,we obtained a detection rate of 90%in savanna and 40%in forest areas.Tests for targets in movement(N=20),the detection rates were 90%in the savanna and 30%in the forest areas.The spatial accuracy obtained by the system was 14.61 m,being significantly more accurate in savanna(x=10.53)than in forest areas(x=13.06).This approach to wildlife tracking facilitates the use of drones by non-specialists at an affordable cost for conservation projects with limited resources.The reduced size of the tags,the long battery life,and the iower cost compared to GPS-tags open up a range of opportunities for animal tracking.

The USLE soil erodibility nomograph revisited

The nomograph by Wischmeier et al.(1971)for calculating the K-factor in the USLE was extremely useful when there was low access to calculators.However,the generalised calculation of this factor requires the development of analytic procedures.This paper presents a detailed analysis of the nomograph and its underlying equation,which is applicable only when the silt plus very find sand fraction does not exceed 70%.We also examined the quality of fit on the nomograph of the adaptations to the equation that have been proposed,as a means of dealing with those areas where the original equation is not applicable.All models are shown to have areas where the fit is deficient or even unacceptable.Besides,the family of curves on the nomograph for the various values taken by the organic matter are not coincident with the mathematical function from which they presumably derive.The study also identifies those areas of the textural triangle in which the soils originally used in developing the USLE are located,with a view to according a lower predictive value to the contrasting areas in which calculations of the K-factor will necessarily be extrapolations.Finally,a new equation for calculating the K-factor is presented,which accurately reproduces the different sections of the nomograph,and allows the poorly functioning graph to be dispensed with.The paper ends with a link to a tool in R for simplifying the procedure for calculating the K-factor,taking into account varying situations of data availability.

A graph-based approach for the structural analysis of road and building layouts

A better understanding of the relationship between the structure and functions of urban and suburban spaces is one of the avenues of research still open for geographical information science.The research presented in this paper develops several graph-based metrics whose objective is to characterize some local and global structural properties that reflect the way the overall building layout can be cross-related to the one of the road layout.Such structural properties are modeled as an aggregation of parcels,buildings,and road networks.We introduce several computational measures(Ratio Minimum Distance,Minimum Ratio Minimum Distance,and Metric Compactness)that respectively evaluate the capability for a given road to be connected with the whole road network.These measures reveal emerging sub-network structures and point out differences between less-connective and moreconnective parts of the network.Based on these local and global properties derived from the topological and graph-based representation,and on building density metrics,this paper proposes an analysis of road and building layouts at different levels of granularity.The metrics developed are applied to a case study in which the derived properties reveal coherent as well as incoherent neighborhoods that illustrate the potential of the approach and the way buildings and roads can be relatively connected in a given urban environment.Overall,and by integrating the parcels and buildings layouts,this approach complements other previous and related works that mainly retain the configurational structure of the urban network as well as morphological studies whose focus is generally limited to the analysis of the building layout.

Nanostructured biohybrid material with wide-ranging antiviral action

Respiratory pathogens kill more people than any other infectious agent each year worldwide.Development of novel,economically friendly,sustainable,and highly efficient materials against viruses is a major challenge.Herein,we describe a nanostructured material composed of very small crystalline phosphate copper nanoparticles synthesized using a new biohybrid technology that employs a biological agent for its formation at room temperature in aqueous media.The evaluation of different enzymes in the final preparation of the nanomaterial or even in synthetic methods was performed.Biochemical characterization revealed the formation of Cu species in the protein network.The best biomaterial synthesized using a lipase called BioCuNPs showed excellent inhibition capacity against functional proteins of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2);for example,assent 3-chymotrypsin like protease(3CLpro)complete inhibition was achieved by using 5µg/mL,or acetone(ACE)–spike protein interaction was inhibited by more than 80%in the presence of 400µg/mL of BioCuNPs.Taking these in vitro results into account,an efficacy analysis against human coronavirus 229E(HCoV-E229)coronavirus was performed.A virus reduction of 99%was obtained in 5 min.Additionally,SARS-CoV-2 virus was tested to demonstrate high efficiency,with>99%inhibition in 15 min using 500 microgram of material.To determine the wide applicability of this nanohybrid against viruses,an evaluation was carried out against a non-enveloped virus such as Human Rhinovirus(HRV-14),obtaining a virus reduction of 99.9%in 5 min.Finally,the virucidal capacity against different bacteriophages was also evaluated,obtaining an excellent inhibition effect against PhageΦX174(99.999%reduction in 5 min).

Convergence of optimal control problems governed by second kind parabolic variational inequalities

We consider a family of optimal control problems where the control variable is given by a boundary condition of Neumann type. This family is governed by parabolic variational inequalities of the second kind. We prove the strong convergence of the optimal control and state systems associated to this family to a similar optimal control problem. This work solves the open problem left by the authors in IFIP TC7 CSMO2011.
