The Role of Ultrasound and Air Leak Measurement in Assessing Lung Expansion after Thoracic Surgery

Objective: To determine if lung ultrasound semiology is applicable to the post-surgical hemithorax, and its value in the evaluation of lung expansion in the postoperative period when paired with the use of digital chest drain systems. Methods: Prospective observational study including all patients undergoing thoracic surgery from June 2012 to March 2013. Patients undergoing pleurodesis or hemodynamically unstable were not considered candidates. final inclusion in the study was based on the availability of digital chest drain system. A transthoracic lung ultrasound evaluation of the anterior and anterolateral windows was performed 20 minutes after chest drain placement. Presence or not of lung sliding and air leak values taken from the digital chest drain system were recorded. Data were submitted to a binomial classification test for analysis. Results: forty-nine patients were included, yielding a total of 64 hemithoraces. Lung sliding was seen in 53 cases (82.8%), and an air leak value of 20 ml/min or less in 56 cases (87.5%). Sensitivity was 92.8% (95% CI: 82.6% to 97.9%) and specificity 87.5% (95% CI: 47.3% to 97.9%). Positive predictive value was 98.1%, and negative predictive value was 63.6%. Conclusions: Transthoracic lung ultrasound is a useful technique that can complement the use of digital chest drain systems in the evaluation of post-surgical lung expansion. The incorporation of lung ultrasound can greatly reduce the need for chest radiographs in thoracic surgery departments.

Levels of TNf-α,IL-1β,TGf-β,and IL-6 in patients with chronic hepatitis C virus infection and hyperinsulinemia

Background: Previous evidences support to inflammatory process associated with the IR those important factors in the development of fibrosis and hepatic damage in patients with HCV infection. Objectives: Determine the presence of immunological or histological differentces between hyperinsulinemic and non-hyperinsulinemic chronic HCV-infected patients with at least 10 years of evolution. Material and Methods: The body mass index, Child-Puigh score, liver function tests, and biochemical profile were evaluated in 30 control, 40 chronic HCV-infected patients with hyperinsulinemia, and in 40 normoinsulinemic chronic HCV-infected patients with at least 10 years of evolution. In addition, HCV genotype and viral load were established with the Amplicor System. In addition, nitrites in plasma, and TNf-α, IL-1β, TGf-β, IL-6, and insulin levels were determined. Results: No differences were found in BMI, Child-Pugh “A” scores, ALT, viral load, or genotype between the hyperinsulinemic (>25 IU/ml) and normoinsulinemic ( Hyperinsulinemia patients had a higher HOMA- IR value (13.6) than normoinsulinemia patients (3.4), and none had a glycemia > 126 mg/dl. Cy- tokines concentration did not show differences with respect to controls. Nitrites showed a slight increase only in patients with HCV infection. Conclusions: Absence of changes in the proinflammatory cytokines concentration or in some inflammation markers in chronic HCV patients with hyperinsulinemia suggests that long-term insulin levels, in presence of HCV, cannot explain by themselves the hepatic alterations observed in the patient with HCV infection, the presence of others elements (alcoholism, cirrhosis, etc.) is necessary to onset hepatic damage in these patients.

Taxifolin stability: In silico prediction and in vitro degradation with HPLC-UV/UPLCe ESI-MS monitoring

Taxifolin has a plethora of therapeutic activities and is currently isolated from the stem bark of the tree Larix gmelinni(Dahurian larch). It is a flavonoid of high commercial interest for its use in supplements or in antioxidant-rich functional foods. However, its poor stability and low bioavailability hinder the use of flavonoid in nutritional or pharmaceutical formulations. In this work, taxifolin isolated from the seeds of Mimusops balata, was evaluated by in silico stability prediction studies and in vitro forced degradation studies(acid and alkaline hydrolysis, oxidation, visible/UV radiation, dry/humid heating) monitored by high performance liquid chromatography with ultraviolet detection(HPLC-UV) and ultrahigh performance liquid chromatography-electrospray ionization-mass spectrometry(UPLC-ESI-MS). The in silico stability prediction studies indicated the most susceptible regions in the molecule to nucleophilic and electrophilic attacks, as well as the sites susceptible to oxidation. The in vitro forced degradation tests were in agreement with the in silico stability prediction, indicating that taxifolin is extremely unstable(class 1) under alkaline hydrolysis. In addition, taxifolin thermal degradation was increased by *** the other hand, with respect to photosensitivity, taxifolin can be classified as class 4(stable).Moreover, the alkaline degradation products were characterized by UPLC-ESI-MS/MS as dimers of taxifolin. These results enabled an understanding of the intrinsic lability of taxifolin, contributing to the development of stability-indicating methods, and of appropriate drug release systems, with the aims of preserving its stability and improving its bioavailability.

Experience of more than 3000 cycles of Brazilian women using the contraceptive vaginal ring (nuvaring<sup>®</sup>)

Objective: To evaluate the profile of the Brazilianvaginal ring user, reasons for choosing it, satisfaction, menstrual pattern, tolerability, efficacy, side effects and reasons for discontinuation. Methods: We reviewed the files of 148 users who attended the Hospital de Jacarepaguáand private clinics, between 2005 and 2011, in a total of 3107 cycles. Kaplan-Meier survival curves and regression models were used in statistical analysis. Results: The profile was: single, nulligravida, 30 years (mean) and with university education. The previous contraception used was mostly the oral hormonal contraception (75.7%). Women chose the ring for: practicality (32.4%), forgetting the pill (32.4%) and irregular vaginal bleeding (13.5%). The satisfaction degree was 80.5% (119). There were good cycle control and no pregnancies. Thirty-eight patients discontinued the use (25.7%). The main reasons were wish for pregnancy (6%), spotting (3.4%) and increased vaginal discharge (2.7%). At the end of the first year, use of the greatest reduction of continuity probability was observed (19.3%). Women with side effects were 68% less likely to be satisfied with the method than those without (p-value Conclusion: Nuvaring®is an excellent contraceptiveoption due to its sefficacy, cycle control, tolerability, practicality and safety.

Reviewing Self-Injuries Behavior in Macaques: The Role of Early Experience and Genetics in an Integrated Non-Human Model

Genetic differences in non-human primates and the context where they were reared are important in the determination of their behaviors. Some studies suggest that Self-Injurious Behavior results from stress or anxiety, but some macaques appear to be more vulnerable to acquiring Self-Injurious Behavior. Vulnerability and risk for socially reared macaques of developing Self-Injurious Behavior increased when they experienced early adverse events and suffered from consequent stress. Stressful events can cause alterations in the neuroendocrine and neuropeptide systems associated with the regulation of stress and anxiety. Dysregulation in these systems contributes to the occurrence of anxious episodes that lead to self-harm. This could be used by those animals to counteract the effects of anxiety, causing euphoria by releasing endogenous opioids. In developing an integrative approach of Self-Injurious Behavior, we propose the diathesis-stress model to illustrate how the environment and genetics are influenced as well, taking rhesus monkeys as an example. Rhesus macaques and humans have essentially the same gene 5-HTT as well as functional polymorphism. The allelic variation in the region promoting the serotonin transporter gene (5-HTT) was associated with significant differences in the number of depressive symptoms and contributions to social reward and punishment. Growing up without the mother and the rearing environment are important risk factors for developing abnormal behaviors. The Quaternary Structure of NADPH Thioredoxin Reductase C Is Redox-Sensitive

NADPH thioredoxin reductase C （NTRC） is a chloroplast enzyme able to conjugate NADPH thioredoxin reductase （NTR） and thioredoxin （TRX） activities for the efficient reduction of 2-Cys peroxiredoxin （2-Cys PRX）. Because NADPH can be produced in chloroplasts during darkness, NTRC plays a key role for plant peroxide detoxification during the night. Here, it is shown that the quaternary structure of NTRC is highly dependent on its redox status. In vitro, most of the enzyme adopted an oligomeric state that disaggregated in dimers upon addition of NADPH, NADH, or DTT. Gel filtration and Western blot analysis of protein extracts from Arabidopsis chloroplast stroma showed that native NTRC forms aggregates, which are sensitive to NADPH and DTT, suggesting that the aggregation state might be a significant aspect of NTRC activity in vivo. Moreover, the enzyme is localized in clusters in Arabidopsis chloroplasts. NTRC triple and double mutants, A164G- V182E-R183f and A164G-R183f, replacing key residues of NADPH binding site, showed reduced activity but were still able to dimerize though with an increase in intermediary forms. Based on these results, we propose that the catalytically active form of NTRC is the dimer, which formation is induced by NADPH.

The Strawberry fruit fra a Allergen functions in flavonoid Biosynthesis

The strawberry fra a I allergen is a homolog of the major birch pollen allergen Bet v 1. It is synthesized by red ripe fruits of fragaria x ananassa while white fruits of a mutant genotype, which is known to be tolerated by individuals affected by allergy, are devoid of it. Proteomic analyses have shown that fra a 1 is down-regulated in the tolerated white- fruited genotype along with enzymes of the anthocyanin pigment pathway. In this study, we report the spatial and tem- poral expression of three fra a genes that encode different isoforms, and the transient RNAi-mediated silencing of the fra a genes in strawberry fruits of the red-fruited cultivar Elsanta with an ihpRNA construct. As a consequence of reduced levels of fra a mRNAs, fruits were obtained that produced significantly decreased levels of anthocyanins and upstream metabolites. This effect is consistent with the parallel down-regulation of the phenylalanine ammonia lyase （faPAL） and to a lesser extent of the chalcone synthase （faCHS） transcript levels also found in these fruits. In naturally occurring white- fruited genotypes of f. chiloensis and f. vesca, fra a transcript levels are higher than those of the red-fruited varieties, likely to compensate for the low expression levels of faPAL and faCHS in these mutant genotypes. The results demonstrate that fra a expression is directly linked to flavonoid biosynthesis and show that the fra a allergen has an essential biological function in pigment formation in strawberry fruit.

Dear Editor,One of the most serious issues in modern oncology is the ineffectiveness of treatments in destroying tumours,which leads to tumour recurrence and,ultimately,patient *** phenomenon is caused by conventional therapies’fractional killing of tumour cells,which can also cause resistant cells to spread.1 We analysed chemotherapy-resistant cells and discovered that they are smaller than untreated cells(fig.1a).The fact that this phenomenon occurs in all tested cell types(Hela,A549,Huh7,and MCf7)and regardless of the chemotherapy regimen(TRAIL,Camptothecin,Doxorubicin)suggests that it is a widespread phenomenon(fig.1a,Supplementary fig.S1).
