Noninvasive methods for prediction of esophageal varices in pediatric patients with portal hypertension

AIM: To evaluate clinical and laboratory parameters for prediction of bleeding from esophageal varices (EV) in children with portal hypertension. METHODS: Retrospective study of 103 children (mean age: 10.1 ± 7.7 years), 95.1% with intrahepatic portal hypertension. All patients had no history of bleeding and underwent esophagogastroduodenoscopy for EV screening. We recorded variceal size (f1, f2 and f3), red-color signs and portal gastropathy, according to the Japanese Research Society for Portal Hypertension classification. Patients were classified into two groups: with and without EV. Seven noninvasive markers were evaluated as potential predictors of EV: (1) platelet count; (2) spleen size z score, expressed as a standard deviation score relative to normal values for age; (3)platelet count to spleen size z score ratio; (4) platelets count to spleen size (cm) ratio; (5) the clinical prediction rule (CPR); (6) the aspartate aminotransferase to platelet ratio index (APRI); and (7) the risk score. RESULTS: Seventy-one children had EV on first endoscopy. On univariate analysis, spleen size, platelets, CPR, risk score, APRI, and platelet count to spleen size z score ratio showed significant associations. The best noninvasive predictors of EV were platelet count [area under the receiver operating characteristic curve (AUROC) 0.82; 95%CI: 0.73-0.91], platelet: spleen size z score (AUROC 0.78; 95%CI: 0.67-0.88), CPR (AUROC 0.77; 95%CI: 0.64-0.89), and risk score (AUROC 0.77; 95%CI: 0.66-0.88). A logistic regression model was applied with EV as the dependent variable and corrected by albumin, bilirubin and spleen size z score. Children with a CPR fold more likely to have EV compared to children with CPR > 114. A risk score > -1.2 increased the likelihood of EV (odds ratio 7.47; 95%CI: 2.06-26.99). CONCLUSION: Children with portal hypertension with a CPR below 114 and a risk score greater than -1.2 are more likely to have present EV. Therefore, these two tes

In this study, the ambient seismic noise cross-correlation technique is applied to estimate the upper structure of the crust beneath Great Island of Tierra del fuego（Tdf）, Argentina, by the analysis of shortperiod Rayleigh wave group velocities. The island, situated in the southernmost South America, is a key area of investigation among the interaction between the South American and Scotia plates and is considered as a very seismically active one. Through cross-correlating the vertical components of ambient seismic noise registered at four broadband stations in Tdf, we were able to extract Rayleigh waves which were used to estimate group velocities in the period band of 2.5-16 s using a timefrequency analysis. Although ambient noise sources are distributed in homogeneously, robust empirical Green＇s functions could be recovered from the cross-correlation of 12 months of ambient noise, The observed group velocities were inverted considering a non-linear iterative damped least-squares inversion procedure and several 1-D shear wave velocity models of the upper crust were *** to the inversion results, the S-wave velocity ranges between 1.75 and 3,7 km/s in the first10 km of crust, depending on the pair of stations considered. These results are in agreement to the major known surface and sub-surface geological and tectonic features known in the area. This study represents the first ambient seismic noise analysis in Tdf in order to constraint the upper crust beneath this *** can also be considered as a successful feasibility study for future analyses with a denser station deployment for a more detailed imaging of structure.

Portopulmonary Hypertension in Patients Undergoing Liver Transplantation: A Case Report

Portopulmonary hypertension (PPH) is a form of pulmonary arterial hypertension, characterized by elevation of pulmonary arterial pressure due to increased resistance to pulmonary blood flow. It is a consequence of portal hypertension, associated or not with liver disease. A retrospective study of patients undergoing liver transplantation was carried out in the Liver Transplantation Unit of the Clinics Hospital of the State University of Campinas (UNICAMP) from 1991 to 2012, covering 389 transplants. Although 50 (12.9%) patients presented with mean pulmonary arterial pressure (MPAP) > 25 mmHg, only one case (0.25%) met the diagnostic criteria for PPH: a 43-year-old female patient with advanced liver disease, caused by primary biliary cirrhosis with portal hypertension, diagnosed as mild portopulmonary hypertension and diagnosed in the operating room. PPH is a serious complication of portal hypertension. It is related to the great difficulty in diagnosis and the high mortality rate due to right heart failure. PPH is an entity with a low frequency of occurrence in individuals undergoing liver transplantation, although there is a wide variation in the prevalence rate presented in the literature and the definitive diagnosis should be confirmed with right heart catheterization.

This study assessed the quality parameters and consumer perception and behavior of butter processed with different ratios of cow and sheep milk *** milk cream butter(CMCB)showed lower moisture content and higher fat content,smaller fat globules,and higher concentrations of long-chain saturated fatty acids and monounsaturated fatty acids,resulting in improved potential health *** addition,CMCB showed yellow color,little salt,and butter taste as the main sensory characteristics,resulting in grateful and good digestibility *** milk cream addition improved the rheological properties of the products(lower viscosity,consistency index,and rigidity(J0),and higher fluidity and viscoelastic behavior(J1)),mainly due to the increased concentrations of short-chain and medium-chain saturated fatty acids and polyunsaturated fatty ***,sheep milk cream butter(SMCB)was characterized by a greasy and striking aroma,with a lower perception of good digestibility and good appearance,and a higher bitter *** association of cow and sheep milk cream could be an exciting option to obtain products with improved fatty acid profiles,health indices,and yet maintain desired rheological and sensory *** formulation with 60%sheep milk cream and 40%cow milk cream would be the suggested combination.

CONSPECTUS: future medicine is primarily aiming at the development of novel approachesfor an early diagnosis of diseases and a personalized therapy for patients. for achieving theseobjectives, a key role is played by medical imaging. Among available noninvasive imagingtechniques, fluorine-19 (^(19)f) Magnetic Resonance Imaging (MRI) is emerging as a powerfulquantitative detection modality for clinical use both for molecular imaging and for cell *** strength of using ^(19)f-MRI is mainly related to the lack of endogenous organic fluorine intissues, with no background, enabling the visualization of fluorinated tracers as hot-spot images,adding secondary independent information to the anatomical features provided by thegrayscale ^(1)H-MRI. The main challenge for ^(19)f-MRI clinical application is the intrinsic reducedsensitivity of MRI. To improve sensitivity, undoubtedly the use of a high field MRI scanner andcryogenic radiofrequency probes is advantageous, but there is a clear need of developingincreasingly effective fluorinated *** ideal tracer should bear as many as possible magnetically equivalent fluorine atoms andshow optimal magnetic resonance relaxivity properties (i.e., T_(1) and T_(2)), which enable reduced acquisition time with the possibility toapply fast imaging methods. Moreover, it should be biocompatible with reduced tendency to bioaccumulate in tissues, which is oneof the main drawbacks in using perfluorocarbons (PfCs), together with their difficulty to be chemically modified with functionalgroups. In fact, PfCs such as perfluorooctyl bromide (PfOB), perfluoro-15-crown-5-ether (PfCE), and linear perfluoropolyethers(PfPE) are currently the most used tracers in ^(19)f-MRI preclinical and clinical studies, with the above-mentioned limitations. This aim of the present study was to analyze the hemodynamic responses during resistance exercise performed at different intensities and with different recovery intervals. This study was conducted on twenty-four apparently healthy male individuals (25.50 ± 3.72 years and 76.50 ± 4.50 kg) experienced in strength training. The volunteers performed a 1RM test to determine the training load for the study. Blood pressure and Rate Pressure Product were measured before and at the end of the exercise training. The only significant difference observed was in SBP during strength training at 70% intensity (121.7 ± 8.68, p = 0.039), which was lower than SBP at the remaining intensities of 80% (126.3 ± 7.11) and 90% (127.1 ± 7.51). It was concluded that strength training performed at different intensities and recovery intervals did not significantly alter most variables, changing only the SBP due to the intensity employed.
