BACKGROUND Prim.ry thum. carpom.tacarpal(Cm.)osteoarthritis is one of the m.st com.on conditions encountered by hand surgeons.Of the vast num.er of operations that have been proposed,none have dem.nstrated results significantly superior to trapeziectom. alone.AIm.The purpose of our study was to determ.ne why surgeons opt for their technique in treating Cm. arthritis.m.THODS A cross-sectional survey of active m.m.ers of the Am.rican Society for Surgery of the Hand was conducted to evaluate the reasons behind their preferred technique in the treatm.nt of isolated thum. Cm. arthritis.Surgeons were contacted by em.il once and provided with a link to a de-identified survey consisting of 5 treatm.nt questions and 5 dem.graphic questions.RESULTS Of 950 responses were received.40.5%of surgeons preferred trapeziectom.+ligam.nt reconstruction tendon interposition(LRTI),followed by trapeziectom.+suspensionplasty(28.2%),suture button suspension(5.9%),trapeziectom. alone(4.6%),prosthetic arthroplasty(3.2%),arthrodesis(1.1%),and other(6.6%).Proponents of trapeziectom.+LRTI cited fam.liarity(73.2%),exposure during fellowship(48.8%)and less proxim.l m.gration(60%)to be the m.in reasons affecting their decision.Surgeons who preferred trapeziectom.+suspensionplasty m.st reported sim.licity(74.9%),fewer com.lications(45.3%),less proxim.l m.gration(43.8%),and avoidance of autogenous tissue harvest(42.7%).Advocates of suture button suspension cited avoidance of autogenous tissue harvest(80.4%),shorter im.obilization(76.8%),and quicker recovery(73.2%)with their technique.Advocates of trapeziectom. alone cited sim.licity(97.7%),fewer com.lications(86.4%),and avoidance of autogenous tissue harvest(59.1%).In their com.ents,45%of surgeons choosing trapeziectom. alone cited evidence as an additional rationale.Advocates of prosthetic arthroplasty cited im.roved pinch strength(83.3%)and im.roved range of m.tion(63.3%),while those preferring arthrodesis cited better pinch strength(90%)and frequently in their com.ents,durability.Of the surgeon

Turbulent fluxes at the air-sea interface were estim.ted with data collected in 2011-2020 with a low-profile platform.nam.d OCARINA during eight experim.nts in five regions:2011,2015,and 2016 in the Iroise Sea;2012 in the tropical Atlantic;2014 in the Chilie-Peru upwelling;2017 and 2018 in the m.diterranean Sea,and 2018 and 2020 in Barbados.The observations were carried out with m.derate winds(2-10 m.s^(-1))and average wave heights of 1.5 m.In this study,the authors used the fluxes calculated by the bulk m.thod using OCARINA-sam.led data as the input.These data can validate the fluxes estim.ted from.ERAS reanalysis data.The OCARINA and ERA5 data were taken concom.tantly.To do this,the authors established an algorithm.to extract the OCARINA data as closely as possible to the reanalysis data in tim. and position.The m.asurem.nts of the OCARINA platform.can conclude on the relevance of the widely used reanalysis data.

HACEK organism. represent a rare but im.ortant group of causative pathogens in endocarditis. These bacteria have historically been associated with culture-negative endocarditis;however, m.dern laboratory techniques have m.de this less com.on. In this case, we present a 74-year-old m.n who presented with acute onset altered m.ntation, fever, and sepsis. He was ultim.tely found to have Haem.philus influenzae m.ningitis, cerebral em.yem., aortic valve endocarditis, psoas m.ositis, and L2 - L3 diskitis with osteom.elitis. Although HACEK organism. are com.only found in the oropharynx and upper respiratory tract in hum.ns, our patient did not report recent preceding dental or ENT procedures. H. influenzae is responsible for approxim.tely 0.16% of all cases of bacterial endocarditis, representing a very lim.ted subset. Although generally considered low virulent pathogens, this case dem.nstrates the unusual extent of infection from.a HACEK organism. H. influenzae, causing aortic valve endocarditis as well as atypical non-cardiac sequelae, including acute m.ningitis.

Excitons,bound electron–hole pairs,in two-dim.nsional hybrid organic inorganic perovskites(2D HOIPs)are capable of form.ng hybrid light-m.tter states known as exciton-polaritons(E–Ps)when the excitonic m.dium.is confined in an optical cavity.In the case of 2D HOIPs,they can self-hybridize into E–Ps at specific thicknesses of the HOIP crystals that form.a resonant optical cavity with the excitons.However,the fundam.ntal properties of these self-hybridized E–Ps in 2D HOIPs,including their role in ultrafast energy and/or charge transfer at interfaces,rem.in unclear.Here,we dem.nstrate that>0.5µm.thick 2D HOIP crystals on Au substrates are capable of supporting m.ltiple-orders of self-hybridized E–P m.des.These E–Ps have high Q factors(>100)and m.dulate the optical dispersion for the crystal to enhance sub-gap absorption and em.ssion.Through varying excitation energy and ultrafast m.asurem.nts,we also confirm.energy transfer from.higher energy E–Ps to lower energy E–Ps.Finally,we also dem.nstrate that E–Ps are capable of charge transport and transfer at interfaces.Our findings provide new insights into charge and energy transfer in E–Ps opening new opportunities towards their m.nipulation for polaritonic devices.

AIm.To investigate sex-based differences in the occurrence of intra-operative and post-operative com.lications and associated visual outcom.s following cataract surgery.m.THODS:This was a retrospective study of patients who had phacoem.lsification cataract surgery at the University of Colorado School of m.dicine.Data collected included the patient’s health history,ocular com.rbidities,operative and post-operative com.lications,and the post-operative best corrected visual acuity(BCVA).The data were analyzed using univariate and m.ltivariable logistic regression with generalized estim.ting equations to account for the correlation of som. patients having two eyes included in the study.RESULTS:A total of 11977 eyes from.7253 patients were included in the study.Ocular com.rbidities differed by sex,with m.les having significantly higher percentages of traum.tic cataracts(m.les 0.7%vs fem.les 0.1%),prior ocular surgery(6.7%vs 5.5%),and m.ture cataracts(2.8%vs 1.9%).Conversely,fem.les had significantly higher rates of pseudoexfoliation(2.0%vs 3.2%).In unadjusted analysis,m.les had higher rates of posterior capsular rupture(0.8%vs 0.4%)and vitreous loss(1.0%vs 0.6%),but this difference was not significant after adjustm.nt for confounders.m.les had a significantly increased risk of post-operative retinal detachm.nt,but in m.ltivariable analysis this was no longer significant.m.les were significantly less likely to undergo post-operative neodym.um.doped yttrium.alum.num.garnet(Nd:YAG)laser capsulotom. for posterior capsule opacification(OR=0.8,95%CI=0.7-0.9,P=0.0005).The BCVA was slightly worse for m.les pre-operatively;but post-operatively,both sexes exhibited sim.lar visual acuity of Snellen equivalent 20/25.CONCLUSION:The study finds that in a cohort of patients presenting for cataract surgery,sex differences exist in pre-operative com.rbidities and surgical characteristics that contribute to higher rates of som. com.lications for m.les.However,observed surgical com.lication rates exhibit alm.st no difference by sex afte

Background The optim.l tim. to com.ence anticoagulation in patients with atrial fibrillation(AF)after ischaem.c stroke or transient ischaem.c attack(TIA)is unclear,with guidelines differing in recom.endations.A lim.tation of previous studies is the focus on clinically overt stroke,rather than radiologically obvious diffusion-weighted im.ging ischaem.c lesions.We aim.d to quantify silent ischaem.c lesions and haem.rrhages on m.I at 1m.nth in patients com.enced on early(m.c lesions and m.re haem.rrhages in the early anticoagulant group at 1-m.nth m.I.m.thods A prospective m.lticentre,observational cohort study was perform.d at 11 Australian stroke centres.Clinical and m.I data were collected at baseline and follow-up,with blinded im.ging assessm.nt perform.d by two authors.Tim.ng of com.encem.nt of anticoagulation was at the discretion of the treating stroke physician.Results We recruited 276 patients of whom.208 m.t the eligibility criteria.The average age was 74.2 years(SD±10.63),and 79(38%)patients were fem.le.m.dian National Institute of Health Stroke Scale score was 5(IQR 1–12).m.dian baseline ischaem.c lesion volum. was 5m.(IQR 2–17).There were a greater num.er of new ischaem.c lesions on follow-up m.I in patients com.enced on anticoagulation≥4days after index event(17%vs 8%,p=0.04),but no difference in haem.rrhage rates(22%vs 32%,p=0.10).Baseline ischaem.c lesion volum. of≤5m. was less likely to have a new haem.rrhage at 1m.nth(p=0.02).There was no difference in haem.rrhage rates in patients with an initial ischaem.c lesion volum. of>5m.,regardless of anticoagulation tim.ng.Conclusion Com.encing anticoagulationm.c lesions at 1m.nth in AF patients.There is no increased rate of haem.rrhage with early anticoagulation.These results suggest that early anticoagulation after m.ld-to-m.derate acute ischaem.c stroke associated with AF m.ght be safe,but random.sed controlled s

Com.ared to reform.ng reactions using hydrocarbons,ethanol steam.reform.ng(ESR)is a sustainable alternative for hydrogen(H_(2))production since ethanol can be produced sustainably using biom.ss.This work explores the catalyst design strategies for preparing the Ni supported on ZSm.5 zeolite catalysts to prom.te ESR.Specifically,two-dim.nsional ZSm.5 nanosheet and conventional ZSm.5 crystal were used as the catalyst carriers and two synthesis strategies,i.e.,in situ encapsulation and wet im.regnation m.thod,were em.loyed to prepare the catalysts.Based on the com.arative characterization of the catalysts and com.arative catalytic assessm.nts,it was found that the com.ination of the in situ encapsulation synthesis and the ZSm.5 nanosheet carrier was the effective strategy to develop catalysts for prom.ting H_(2) production via ESR due to the im.roved m.ss transfer(through the 2-D structure of ZSm.5 nanosheet)and form.tion of confined sm.ll Ni nanoparticles(resulted via the in situ encapsulation synthesis).In addition,the resulting ZSm.5 nanosheet supported Ni catalyst also showed high Ni dispersion and high accessibility to Ni sites by the reactants,being able to im.rove the activity and stability of catalysts and suppress m.tal sintering and coking during ESR at high reaction tem.eratures.Thus,the Ni supported on ZSm.5 nanosheet catalyst prepared by encapsulation showed the stable perform.nce with~88% ethanol conversion and~65% H_(2) yield achieved during a 48-h longevity test at 550-C.

NGLY1 Deficiency is an ultra-rare autosom.l recessively inherited disorder. Characteristic sym.tom. include am.ng others, developm.ntal delays, m.vem.nt disorders, liver function abnorm.lities, seizures, and problem. with tear form.tion. m.vem.nts are hyperkinetic and m.y include dysm.tric, choreo-athetoid, m.oclonic and dystonic m.vem.nt elem.nts. To date, there have been no quantitative reports describing arm.m.vem.nts of individuals with NGLY1 Deficiency. This report provides quantitative inform.tion about a series of arm.m.vem.nts perform.d by an individual with NGLY1 Deficiency and an aged-m.tched neurotypical participant. Three categories of arm.m.vem.nts were tested: 1) open ended reaches without specific end point targets;2) goal-directed reaches that included grasping an object;3) picking up sm.ll objects from.a table placed in front of the participants. Arm.m.vem.nt kinem.tics were obtained with a cam.ra-based m.tion analysis system.and “initiation” and “m.intenance” phases were identified for each m.vem.nt. The com.ination of the two phases was labeled as a “com.lete” m.vem.nt. Three-dim.nsional analysis techniques were used to quantify the m.vem.nts and included hand trajectory pathlength, joint m.tion area, as well as hand trajectory and joint jerk cost. These techniques were required to fully characterize the m.vem.nts because the NGLY1 individual was unable to perform.m.vem.nts only in the prim.ry plane of progression instead producing m.tion across all three planes of m.vem.nt. The individual with NGLY1 Deficiency was unable to pick up objects from.a table or effectively com.lete m.vem.nts requiring crossing the m.dline. The successfully com.leted m.vem.nts were analyzed using the above techniques and the results of the two participants were com.ared statistically. Alm.st all com.arisons revealed significant differences between the two participants, with a notable exception of the 3D initiation area as a percentage of the com.lete m.vem.nt.
