Importance of the creation of a short musculofascial tunnel in peritoneal dialysis catheter placement

BACKGROUND Peritoneal dialysis(PD)catheter migration impedes the efficacy of ***,several techniques involving additional sutures or incisions have been proposed to maintain catheter position in the *** To evaluate the efficacy of creating a short musculofascial tunnel beneath the anterior sheath of the rectus abdominis during PD catheter *** Patients who underwent PD catheter implantation between 2015 and 2019 were included in this retrospective *** patients were divided into two groups based on the procedure performed:Patients who underwent catheter implantation without a musculofascial tunnel before 2017 and those who underwent the procedure with a tunnel after *** recorded patient character istics and catheter complications over a two-year follow-up *** addition,postoperative plain abdominal radiographs were reviewed to determine the catheter angle in the event of *** The no-tunnel and tunnel groups included 115 and 107 patients,*** to the no-tunnel group,the tunnel group showed lesser catheter angle deviation toward the pelvis(15.51±11.30 vs 25.00±23.08,P=0.0002)immediately after the operation,and a smaller range of migration within 2 years postoperatively(13.48±10.71 vs 44.34±41.29,P<0.0001).Four events of catheter dysfunction due to migration were observed in the no-tunnel group,and none occurred in the tunnel *** was no difference in the two-year catheter function survival rate between the two groups(88.90%vs 84.79%,P=0.3799).CONCLUSION The musculofascial tunnel helps maintain catheter position in the pelvis and reduces migration,thus preventing catheter dysfunction.

Cell transplantation to replace retinal ganglion cells faces challenges-the Switchboard Dilemma

The mammalian retina displays incomplete intrinsic regenerative capacities;therefore,retina degeneration is a major cause of irreversible blindness such as glaucoma,agerelated macular degeneration and diabetic *** diseases lead to the loss of retinal cells and serious vision loss in the late *** cell transplantation is a great promising novel treatment for these incurable retinal degenerative diseases and represents an exciting area of regenerative *** suitable stem cell sources for transplantation including human embryonic stem cells,induced pluripotent stem cells and adult stem cells have been identified as promising target ***,the retina is an elegant neuronal complex composed of various types of cells with different *** replacement of these different types of cells by transplantation should be addressed *** far,retinal pigment epithelium transplantation has achieved the most advanced stage of clinical trials,while transplantation of retinal neurons such as retinal ganglion cells and photoreceptors has been mostly studied in pre-clinical animal *** this review,we opine on the key problems that need to be addressed before stem cells transplantation,especially for replacing injured retinal ganglion cells,may be used practically for treatment.A key problem we have called the Switchboard Dilemma is a major block to have functional retinal ganglion cell *** use the public switchboard telephone network as an example to illustrate different difficulties for replacing damaged components in the retina that allow for visual *** ganglion cell transplantation is confronted by significant hurdles,because retinal ganglion cells receive signals from different interneurons,integrate and send signals to the correct targets of the visual system,which functions similar to the switchboard in a telephone network-therefore the Switchboard Dilemma.

Training gastroenterology fellows to perform gastric polypectomy using a novel ex vivo model

AIM:To evaluate the effect of hands-on training of gastroenterology fellows in gastric polypectomy using an ex vivo ***:Eight gastroenterology fellows at Mackay Memorial Hospital,Taipei were evaluated in gastricpolypectomy techniques using a pig stomach with artificial polyps created by a rubber band ligation *** performance of four second year(year-2)fellows who had undergone one year of clinical training was compared with that of four f irst year(year-1)fellows both before and after a 4-h workshop using the ex vivo *** workshop allowed for hands-on train-ing in the removal of multiple artif icial polyps and the placement of hemoclips at the excision *** included observation of technical skills,procedure time,and the fellows' conf idence ***:One week after the workshop,the year-1 fellows were re-evaluated and had significantly im-proved mean performance scores(from 17.9 ± 1.8 to 22.5 ± 0.7),conf idence scale(from 4.5 ± 1.0 to 7.8 ± 0.5)and procedure time(from 615.0 ± 57.4 s to 357.5 ± 85.0 s)compared with their baseline *** 4 h of training using the ex vivo simulator,the skills of the year-1 fellows were statistically similar to those of the year-2 ***:Use of this ex vivo simulator significantly improved the endoscopic gastric polypectomy skills of gastroenterology fellows who had not had previous clinical training in gastric polypectomy.

Neuro-rejuvenation for neuronal function

Neurodegenerative eye diseases, such as glaucoma, cause irreversible vision loss in millions of patients worldwide, creating serious medical, economic and social issues. Like other mammalian central nervous system tracts, optic nerve intrinsically lacks the capacity for axonal growth and its surrounding environment is also non-permissive to regeneration. Any axonal damage also triggers a vicious cycle of retinal ganglion cell （RGC） death. Exploring methods that can enhance RGCs survival and promote axonal regeneration will not only enable vision restoration for millions of patients, but also shed light on the treatment of other neurodegenerative diseases. In this review article, we will go through three current approaches to cure neu- rodegenerative eye diseases, including cell based therapy, neuro-regeneration and neuro-rejuvenation.

Design concept development of a variable magnetization motorwith improved efficiency and controllable stiffness for roboticapplications

A variable magnetization(VM) motor by incorporating magnets that can be flexibly configured with variable magnetization process is proposed to meet the emerging requirements on motor efficiency and actuator compliance in robotic applications. A generalized spin torque model is established which provides a relationship between the motor torque and two different types of motor inputs, the current inputs and the magnet magnetizations. Avariable magnetization process is proposed based on the study of the hysteresis properties of the magnetic materials and the design criteria for implementing the variable magnetization process with current pulses are established. The feasibility of the variable magnetization is validated with experimental data and the motor functions and performances are numerically demonstrated and evaluated. The results show that the VM motor can maintain high efficiency by switching between two actuation modes. Controllable stiffness at different equilibria can be also achieved with the VM motor with instantaneous magnetizing current pulses.
