The Impact of COVID-19 on the Mental-Emotional Wellbeing of Primary Healthcare Professionals: A Descriptive Correlational Study

The present study aimed to examine work environment related factors and frontline primary healthcare profes-sionals’mental-emotional wellbeing during the COVID-19 pandemic in school communities of Hong Kong.A total of 61(20%)school health nurses(frontline primary healthcare professionals)participated in a cross-sec-tional online survey from March to June 2020.Outcomes of mental-emotional health were measured using the Mental Health Continuum-Short Form(14-item scale with three subscales related to emotional,social and psychological wellbeing);the Perceived Stress Scale(10-item scale with two subscales related to perceived help-lessness and lack of self-efficacy);and the Coping Orientation to Problems Experienced Inventory(Brief COPE),a 28-item inventory with two subscales related to adaptive and maladaptive strategies.Almost half(42.6%)of par-ticipants experienced mental health problems.Those employed in government subsidized schools had signifi-cantly lower scores in mental health wellbeing than those who worked in private schools.Factors relating to increased mental health problems included lack of emotional support,inadequate training relating to infection prevention and control measures,disengagement and self-blame.A variety of factors inﬂuencing school health nurses’social,emotional and psychological wellbeing in their work environment during the COVID-19 pandemic were also reported.The mental-emotional wellbeing of school nurses may relate to their subjective feeling of lone-liness as participants were the sole frontline primary healthcare professional working in the school community during the COVID-19 pandemic.Studyfindings provide relevant evidence for management teams to build a cul-ture of psychological and social support into workplace policies and procedures.Continuous staff development and adequate social support are important to promote the mental-emotional wellbeing of primary healthcare pro-fessionals in school communities as they play a significant role in safeguarding resources during pandemics

Influence of South china Sea SST and the ENSO on Winter Rainfall over South china

The present study investigates the influence of South china Sea （SCS） SST and ENSO on winter （January-February-March; JFM） rainfall over South china and its dynamic processes by using station observations for the period 1951-2003, Met Office Hadley Center SST data for the period 1900-2008, and ERA-40 reanalysis data for the period 1958-2002. It is found that JFM rainfall over South china has a sig- nificant correlation with Nio-3 and SCS SST. Analyses show that in El Nio or positive SCS SST anomaly years, southwesterly anomalies at 700 hPa dominate over the South china Sea, which in turn transports more moisture into South china and favors increased rainfall. A partial regression analysis indicates that the independent ENSO influence on winter rainfall occurs mainly over South china, whereas SCS SST has a larger independent influence on winter rainfall in northern part of South china. The temperature over South china shows an obvious decrease at 300 hPa and an increase near the surface, with the former induced by Nio-3 and the latter SCS SST anomalies. This enhances the convective instability and weakens the potential vorticity （PV）, which explains the strengthening of ascending motion and the increase of JFM rainfall over South china.

Randomized controlled trial of antibiotic prophylaxis regimens for transrectal ultrasound-guided prostate biopsy

Background A prior study showed aimed to evaluate and compare the transrectal ultrasound-guided prostate Methods A prospective randomized significant antibiotic resistance to quinolone in our population. In this study we efficacy of a single versus a combined prophylactic antibiotic regimen before biopsy （TRUGPB）. study was conducted at a university hospital. Patients undergoing TRUGPB were randomized into an amoxicillin-clavulanate alone （1 mg; one dose before and two doses after biopsy） or an amoxicillin-clavulanate ＋ ciprofloxacin group （250 mg; one dose before and two doses after biopsy）. Patients were surveyed for infection symptoms by phone on days 3 and 30 after TRUGPB. We defined an infective complication as the occurrence of symptoms including fever, chills or rigor within 30 days after prostate biopsy, requiring medical treatment or hospitalization, aided by a territory-wide electronic medical record system. Results Between November 2007 and July 2009, 367 patients were randomized to either amoxicillin-clavulanate alone or amoxicillin-clavulanate ＋ ciprofloxacin group. The infection rates after TRUGPB were 3.91% in the former group （7 out of 179 patients） versus 0.53% （1 out of 188 patients） in the latter. Sixty-three percent （5/8） of patients with infective complications needed hospitalization. There was no intensive care unit admission or mortality during the study period. Conclusions Combining prophylactic antibiotics with amoxicillin-clavulanate ＋ ciprofloxacin significantly reduced the incidence of infective complications after TRUGPB. We recommended a combination regimen, especially in centre with high incidence of post-TRUGPB infection.

Visualizing recent changes in Wikipedia

Large wikis such as Wikipedia attract large numbers of editors continuously editing content.It is difficult to observe what editing activity goes on at any given moment,what editing patterns can be observed,and which are the currently active editors and articles.We introduce the design and implementation of an information visualization tool for data streams of recent changes in wikis that aims to address this difficulty.We also show examples of our visualizations from English Wikipedia,and present several patterns of editing activity that we have visually identified using our tool.We have evaluated our tool's usability,accuracy and speed of task performance in comparison with Wikipedia's recent changes page,and have obtained qualitative feedback from users on the pros and cons of our tool.We also present a review of the related literature.

Large-scale Circulation Control of the Occurrence of Low-level Turbulence at Hong Kong International Airport

This study identifies the atmospheric circulation features that are favorable for the occurrence of low-level turbulence at Hong Kong International Airport [below 1600 feet （around 500 m）]. By using LIDAR data at the airport, turbulence and nonturbulence cases are selected. It is found that the occurrence of turbulence is significantly related to the strength of the southerly wind at 850 hPa over the South china coast. On the other hand, the east-west wihd at this height demonstrates a weak relation to the occurrence. This suggests that turbulence is generated by flow passing Lantau Island from the south. The southerly wind also transports moisture from the South china Sea to Hong Kong, reducing local stability. This is favorable for the development of strong turbulence. It is also noted that the strong southerly wind during the occurrence of low-level turbulence is contributed by an anomalous zonal gradient of geopotential in the lower troposphere over the South china Sea. This gradient is caused by the combination of variations at different timescales. These are the passage of synoptic extratropical cyclones and anticyclones and the intraseasonal variation in the western North Pacific subtropical high. The seasonal variation in geopotential east of the Tibetan Plateau leads to a seasonal change in meridional wind, by which the frequency of low-level turbulence is maximized in spring and minimized in autumn.

BACKGROUND Few studies have addressed the effects of human leukocyte antigen(HLA)alleles on different clinical sub-phenotypes in childhood steroid-sensitive nephrotic syndrome(SSNS),including SSNS without recurrence(SSNSWR)and steroid-dependent nephrotic syndrome/frequently relapse nephrotic syndrome(SDNS/FRNS).In this study,we investigated the relationship between HLA system and children with SSNSWR and SDNS/FRNS and clarified the value of HLA allele detection for precise typing of childhood SSNS.METHODS A total of 241 chinese Han individuals with SSNS were genotyped using GenCap-WES Capture Kit,and four-digit resolution HLA alleles were imputed from available Genome Wide Association data.The distribution and carrying frequency of HLA alleles in SSNSWR and SDNS/FRNS were investigated.Additionally,logistic regression and mediating effects were used to examine the relationship between risk factors for disease process and HLA system.RESULTS Compared with SSNSWR,significantly decreased serum levels of complement 3(C3)and complement 4(C4)at onset were detected in SDNS/FRNS(C3,Pchildren with SSNS as mediators of immunoregulation.

Possible Roles of Diet in the Etiology of Rheumatoid Arthritis:A Systematic Review

Background:There is a growing interest in the possible role of diet in the prevention of rheumatoid arthritis(RA).However,the evidence for diet having a role in the etiology of RA is inconsistent,sometimes conflicting.Objective:To critically appraise the literature to pool the results of studies to clarify the relation between diet risk and RA.Methods:We performed a systematic review using guideline-recommended methodology to evaluate the association between pre-illness diet or diet pattern and the risk of subsequent RA diagnosis.Results:A total of 25 studies met the inclusion criteria.They include 13 cohort studies,10 case-control studies and 2 nested case-control studies,with a total of 1279810 participants.There were new evidences of a protective effect of long term consumption of alcohol.High sodium intake was associated with an increased risk of RA.Fish intake has consistently been shown to have no effect on the development of RA.Mediterranean diet pattern,Vitamin D intake,and the consumption of long-chain omega-3 acids were not associated with an increased/decreased risk of RA in every two studies.There were non-statistically significant association between RA and sugar-sweetened soda,red meat,vegetable and fruits in each single study.The association between coffee,tea and RA was consistent,and here exists an inverse relationship between olive oil and RA.Due to the heterogeneity of study designs and analyses,the results could not be pooled.Conclusion:The results of this SR indicate that alcohol consumption and sodium intake may be associated with RA risk,especially evidence from recent studies.Because of some discordant results,the debate continues on whether some other dietary intakes increase or decrease RA risk.

Large tropical cyclone track forecast errors of global numerical weather prediction models in western North Pacific basin

Although tropical cyclone(TC)track forecast errors(TFEs)of operational warning centres have substantially decreased in recent decades,there are still many cases with large TFEs.The International Grand Global Ensemble(TIGGE)data are used to study the possible reasons for the large TFE cases and to compare the performance of different numerical weather prediction(NWP)models.Forty-four TCs in the western North Pacific during the period 2007-2014 with TFEs(+24 to+120 h)larger than the 75 th percentile of the annual error distribution(with a total of 93 cases)are identified.Four categories of situations are found to be associated with large TFEs.These include the interaction of the outer structure of the TC with tropical weather systems,the intensity of the TC,the extension of the subtropical high(SH)and the interaction with the westerly trough.The crucial factor of each category attributed to the large TFE is discussed.Among the TIGGE model predictions,the models of the European Centre for Medium-Range Weather Forecasts and the UK Met Office generally have a smaller TFE.The performance of different models in different situations is discussed.
