Limited endoscopic sphincterotomy plus large balloon dilation for choledocholithiasis with periampullary diverticula

AIM: To investigate the effectiveness and safety of limited endoscopic sphincterotomy (EST) plus large balloon dilation (LBD) for removing choledocholithiasis in patients with periampullary diverticula (PAD). METHODS: A total of 139 patients with common bile duct (CBD) stones were treated with LBD (10-20 mm balloon diameter) after limited EST. Of this total, 73 patients had PAD and 66 patients did not have PAD (controls). The results of stone removal and complications were retrospectively evaluated. RESULTS: There were no significant differences between the PAD and the control groups in overall successful stone removal (94.5% vs 93.9%), stone removal in first session (69.9% vs 81.8%), mechanical lithotripsy (12.3% vs 13.6%), and complications (11.0% vs 7.6%). Clinical outcomes were also similar between the types of PAD, but the rate of stone removal in first session and the number of sessions were significantly lower and more frequent, respectively, in type B PAD (papilla located near the diverticulum) than controls [23/38 (60.5%) vs 54/66 (81.8%), P = 0.021; and 1 (1-2) vs 1 (1-3), P = 0.037, respectively] and the frequency of pancreatitis was significantly higher in type A PAD (papilla located inside or in the margin of the diverticulum) than in controls (16.1% vs 3.0%, P = 0.047). CONCLUSION: Limited EST plus LBD was an effective and safe procedure for removing choledocholithiasis in patients with PAD. However, some types of PAD should be managed with caution.

AIM To evaluate portal vein(PV) stenosis and stent patency after hepatobiliary and pancreatic surgery, using abdominal computed tomography(CT).METHODS Percutaneous portal venous stenting was attempted in 22 patients with significant PV stenosis(> 50%)-after hepatobiliary or pancreatic surgery- diagnosed by abdominal CT. Stents were placed in various stenotic lesions after percutaneous transhepatic portography. Pressure gradient across the stenotic segment was measured in 14 patients. Stents were placed when the pressure gradient across the stenotic segment was > 5 mm Hg or PV stenosis was > 50%, as observed on transhepatic portography. Patients underwent followup abdominal CT and technical and clinical success, complications, and stent patency were *** Stent placement was successful in 21 patients(technical success rate: 95.5%). Stents were positioned through the main PV and superior mesenteric vein(n = 13), main PV(n = 2), right and main PV(n = 1), left and main PV(n = 4), or main PV and splenic vein(n = 1). Patients showed no complications after stent placement. The time between procedure and final follow-up CT was 41-761 d(mean: 374.5 d). Twenty stents remained patent during the entire follow-up. Stent obstruction-caused by invasion of the PV stent by a recurrent tumor- was observed in 1 patient in a follow-up CT performed after 155 d after the procedure. The cumulative stent patency rate was 95.7%. Small in-stent low-density areas were found in 11(55%) patients; however, during successive follow-up CT, the extent of these areas had *** Percutaneous transhepatic stent placement can be safe and effective in cases of PV stenosis after hepatobiliary and pancreatic surgery. Aim： To investigate the antioxidant effects of Morinda officinalis （Morindae radix, MR） on H2O2-induced oxidative stress in cultured mouse TM3 Leydig cells. Methods： We carried out 2,2-diphenyl-1-picrylhydrazyl free radical scavenging, 3-（4,5-dimethylthiazol-2-yl）-2,5-diphenyltetrazolium bromide, lipid peroxidation, testosterone enzyme immunoassay, superoxide dismutase （SOD）, and catalase （CAT） assays in Leydig TM3 cells. Results： MR showed a 47.8% 2,2-diphenyl-1-picrylhydrazyl radical scavenging effect in TM3 cells with no significant cytotoxicity. Oxidative stress was induced in TM3 cells with 100 μmol H2O2, and treatment of the cells with 250 μg/mL MR showed the most significant protective effect （64%, P 〈 0.001） in the cell viability assay with a decreased lipid peroxidation level （1.75 nmol/mg protein, P 〈 0.05）, increased testosterone production （43.5 pg/mL）, and improvements in SOD activity （7.49 units of SOD/mg protein, P 〈 0.001） and CAT activity （74.6 units of CAT/mg protein, P 〈 0.001）. Conclusion： These findings indicate that MR, as an antioxidant, protects functions of cultured mouse TM3 Leydig cells from H2O2-induced oxidative stress.

A critical challenge hindering the practical application of lithium–oxygen batteries(LOBs)is the inevitable problems associated with liquid electrolytes,such as evaporation and safety *** study addresses these problems by proposing a modified polyrotaxane(mPR)-based solid polymer electrolyte(SPE)design that simultaneously mitigates solvent-related problems and improves ***-SPE exhibits high ion conductivity(2.8×10^(−3)S cm^(−1)at 25℃)through aligned ion conduction pathways and provides electrode protection ability through hydrophobic chain *** this mPR-SPE into solid-state LOBs resulted in stable potentials over 300 *** situ Raman spectroscopy reveals the presence of an LiO_(2)intermediate alongside Li_(2)O_(2)during oxygen *** situ X-ray diffraction confirm the ability of the SPE to hinder the permeation of oxygen and moisture,as demonstrated by the air permeability *** present study suggests that maintaining a low residual solvent while achieving high ionic conductivity is crucial for restricting the sub-reactions of solid-state LOBs.

The objective of this study was to develop equations for estimating ileal digestible crude protein(CP)and metabolizable energy(ME)contents of meat meal(MM)and meat and bone meal(MBM)as feed in-gredients for pigs based on in vitro *** ingredients were 4 sources of MM and 3 sources of *** and CP contents of the ingredients ranged from 3.8%to 33.1%and 46.8%to 82.9%(as-is basis),*** vitro ileal disappearance(IVID)of CP was determined and ileal digestible CP content was calculated by multiplying CP content by IVID of *** vitro total tract disappearance(IVTTD)of dry matter(DM)was determined and ME was calculated using gross energy,CP contents,and IVTTD of *** IVID of CP and IVTTD of DM ranged from 77.2%to 88.7%and from 82.7%to 92.4%,*** ileal digestible CP and ME contents ranged from 37.8%to 73.5%DM and 2,405 to 3,905 kcal/kg DM,*** contents were negatively correlated(P<0.001)with CP(r=-0.99),in vitro ileal digestible CP(r=-0.97),gross energy(r-1.00),in vitro digestible energy(r=-0.97),and adjusted ME(r=-0.97).The most fitting equations for ileal digestible CP and adjusted ME were:ileal digestible CP(%DM)=11.91-0.90×Ash(%DM)+0.74×IVID of CP(%)(R2=0.99)and adjusted ME(kcal/kg DM)=130.85-50.90×ash(%DM)+47.06×IVTTD of DM(%)(R2=0.99).To validate the accuracy of the prediction equations for ME,mean bias and linear bias were determined using a regression *** ME values of MM and MBM were in a good agreement with data obtained from animal ex-periments based on a statistically insignificant bias in the *** conclusion,ME concentrations of MM and MBM as swine feed ingredients can be calculated using ash concentration and in vitro disap-pearance of dry matter.

To study wing-wake interaction for various wing flexibilities, force measurements and digital particle image velocimetry were carried out on flapping hawkmoth-like wings in a water tank. Wing thickness was employed as a design variable for the wing flexi- bility distributions. Abrupt flap-down and phase delay in flexible wings influenced the behaviors of the Leading-Edge Vortex （LEV） and Trailing-Edge Vortex （TEV）, generated by the previous stroke. While the rigid wing exhibited a detached LEV at the end of the stroke, wing with specific flexibilities obtained attached LEVs. The attached LEVs induced a relatively rapid flow toward the wing surface as a result of encountering the TEV, and the flow caused a higher lift peak. On the other hand, the wings with larger wing deformations generated distinctive changes in LEV and TEV behaviors. The flap-down helped the TEV form closer to the wing surface, and it thus caused a downwash rather than wing-wake interaction. Furthermore, the most flexible wing had a newly-formed pair of LEVs above the wing during the wing reversal, thereby being not able to generate the wing-wake interaction. These results help to understand the different vortex structures generated by flexible wings during the wing reversal and the corresponding effects of wing-wake interaction.
