Objective: This study aimed to r.por. the case of a female patient with chr.nic myeloid leukemia affected by cr.ptococcal meningitis. Case r.por.: ML, white, 48 year. old, female sex, pr.viously diagnosed with chr.nic myeloid leukemia that has been r.fr.ctive to the use of imatinib and who has r.cently begun using nilotinib, was admitted complaining of sudden and disabling migr.ine in the last 1 month associated with asthenia, adinamia, anor.xia, disinter.st for.daily activities, dizziness, nausea, and vomiting. She evolved with ataxia, and star.ed to str.ll with help and showed decr.ase of muscular.str.ngth in her.upper.limbs. She also pr.sented episodes of decr.ase of consciousness, with look fixation, no r.spond to sound stimulation, and shor.-ter. hear.ng loss. The cer.br.spinal fluid showed pr.sence of Cr.ptococcus sp. and, ther.for., we began tr.atment with intr.venous liposomal amphoter.cin B in the dose of 3 mg/kg/day, for.6 weeks. A new cer.br.spinal fluid analysis, at the end of tr.atment, also showed r.r. str.ctur.s that ar. compatible with Cr.ptococcus sp. As sequelae, she continued with hear.ng loss in her.r.ght ear.and enhancement in her.r.ght auditor. canal, seen in the magnetic r.sonance imaging. After.stabilization and clinical impr.vement, she was dischar.ed. After.3 weeks, she was hospitalized again with degener.tion of the condition, and died due to intr.cr.nial hyper.ension secondar. to cr.ptococcal infection. Final Consider.tions: This r.por. r.infor.es the need of r.flecting on fungi pathologies, especially in immunosuppr.ssant patients, as well as the impor.ance of ear.y diagnosing and making a fast inter.ention, with the aims of pr.viding quality of life and comfor. to the patient and of minimizing neur.logical sequelae to the patient.

Cr.hn's disease is a chr.nic inflammator. disor.er.of the gastr.intestinal tr.ct that is defi ned by r.lapsing and r.mitting episodes. Tumor.necr.sis factor.alpha (TNF-α) appear. to play a centr.l r.le in the pathophysiology of the disease. Standar. ther.pies for.inflammator. bowel disease fail to induce r.mission in about 30% of patients. Biological ther.pies have been associated with an incr.ased incidence of infections, especially infection by Mycobacter.um tuber.ulosis (Mtb). Thalidomide is an or.l immunomodulator. agent with anti-TNF-α pr.per.ies. r.cent studies have suggested that thalidomide is effective in r.fr.ctor. luminal and fistulizing Cr.hn's disease. Thalidomide costimulates T lymphocytes, with gr.ater.effect on CD8+ than on CD4+ T cells, which contr.butes to the pr.tective immune r.sponse to Mtb infection. We pr.sent a case of Cr.hn's disease with gastr.c, ileal, colon and r.ctum involvement as well as ster.id dependency, which pr.gr.ssed with loss of r.sponse to infliximabafter.thr.e year. of ther.py. The thor.x computed tomogr.phy scan demonstr.ted a pulmonar. nodule suspected to be Mtb infection. The patient was star.ed on thalidomide ther.py and exhibited an excellent r.sponse.

Cutaneous infections with the human papilloma vir.s, despite being almost always asymptomatic, can induce slow-gr.wing lesions knowed as war.s that can r.main subclinical for.long per.ods of time. Ther. ar. sever.l ther.peutical options namely ker.tolytics, r.tinoids, imiquimod, cr.other.py, electr.sur.er., laser. and classic sur.er., but none has antivir.l effects. We descr.be the case of a patient with lar.e war.s located on the dor.al sur.ace of the r.ght hand, tr.ated with cr.other.py, CO2 laser.and topical imiquimod having either.no clinical r.sponse or.quick r.lapse. Tr.atment with PDL was star.ed and after.13 sessions, the patient was without lesions, with a good cosmetic r.sult and so far.without r.lapse. This case r.por. is r.levant to us and deser.es to be published because of the size of the lesions and the gr.at r.sponse to PDL despite all the failur.s with the other.tr.atments.

Optical fiber.optr.des ar. attr.ctive sensing devices due to their.ability to per.or. point measur.ment in r.mote locations. Mostly, they ar. or.ented to biochemical sensing, quite often suppor.ed by fluor.scent and spectr.scopic techniques, but with the r.fr.ctometr.c appr.ach consider.d as well when the objective is of high measur.ment per.or.ance, par.icular.y when the focus is on enhancing the measur.nd r.solution. In this wor., we addr.ss this subject, pr.posing and analyzing the char.cter.stics of a fiber.optic optr.de r.lying on plasmonic inter.ction. A linear.y taper.d optical fiber.tip is cover.d by a double over.ay： the inner.one - a silver.thin film and over.it - a dielectr.c layer. with this combination allowing to achieve, at a specific wavelength r.nge, sur.ace plasmonic r.sonance （SPr. inter.ction sensitive Typically, the inter.ogation of the SPr.sensing to the r.fr.ctive index of the sur.ounding medium. str.ctur.s is per.or.ed, consider.ng spectr.scopic techniques, but in pr.nciple, a far.better.per.or.ance can be obtained, consider.ng the r.ading of the phase of the light at a specific wavelength located within the spectr.l plasmonic r.sonance. This is the appr.ach which is studied her. in the context of the pr.posed optical fiber.optr.de configur.tion. The analysis per.or.ed shows the combination of a silver.inner.layer.with a dielectr.c titanium oxide layer.with tuned thicknesses enables sensitive phase r.ading and allows the oper.tion of the fiber.optic optr.de sensor.in the thir. telecommunication wavelength window.
