Chronic effects of stretching on range of motion with consideration of potential moderating variables:A systematic review with meta-analysis

Background:It is well known that stretch training can induce prolonged increases in joint range of motion(ROM).However,to date more information is needed regarding which training variables might have greater influence on improvements in flexibility.Thus,the purpose of this metaanalysis was to investigate the effects of stretch training on ROM in healthy participants by considering potential moderating variables,such as stretching technique,intensity,duration,frequency,and muscles stretched,as well as sex-specific,age-specific,and/or trained state-specific adaptations to stretch training.Methods:We searched through PubMed,Scopus,Web of Science,and SportDiscus to find eligible studies and,finally,assessed the results from 77 studies and 186 effect sizes by applying a random-effect meta-analysis.Moreover,by applying a mixed-effect model,we performed the respective subgroup analyses.To find potential relationships between stretch duration or age and effect sizes,we performed a meta-regression.Results:We found a significant overall effect,indicating that stretch training can increase ROM with a moderate effect compared to the controls(effect size=-1.002;Z=-12.074;95%confidence interval:-1.165 to-0.840;p<0.001;I^(2)=74.97).Subgroup analysis showed a significant difference between the stretching techniques(p=0.01)indicating that proprioceptive neuromuscular facilitation and static stretching produced greater ROM than did ballistic/dynamic stretching.Moreover,there was a significant effect between the sexes(p=0.04),indicating that females showed higher gains in ROM compared to males.However,further moderating analysis showed no significant relation or difference.Conclusion:When the goal is to maximize ROM in the long term,proprioceptive neuromuscular facilitation or static stretching,rather than ballistic/dynamic stretching,should be applied.Something to consider in future research as well as sports practice is that neither volume,intensity,nor frequency of stretching were found to play a significant role in ROM yields.

Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tNGCs). However, the use of tNGCs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tNGC would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tNGCs and compared the various features of these improved silk^(+) tNGCs with the tubes without holes(silk^(–) tNGCs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tNGCs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tNGCs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tNGCs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tNGCs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. Preliminary examination of early neuroconnectivity features in the R6/1 mouse model of Huntington's disease by ultra-high field diffusion MRI

During the last decades,advances in the understanding of genetic,cellular,and microstructural alterations associated to Huntington's disease(HD)have improved the understanding of this progressive and fatal illness.However,events related to early neuropathological events,neuroinflammation,deterioration of neuronal connectivity and compensatory mechanisms still remain vastly unknown.Ultra-high field diffusion MRI(UHFD-MRI)techniques can contribute to a more comprehensive analysis of the early microstructural changes observed in HD.In addition,it is possible to evaluate if early imaging microstructural parameters might be linked to histological biomarkers.Moreover,qualitative studies analyzing histological complexity in brain areas susceptible to neurodegeneration could provide information on inflammatory events,compensatory increase of neuroconnectivity and mechanisms of brain repair and regeneration.The application of ultra-high field diffusion-MRI technology in animal models,particularly the R6/1 mice(a common preclinical mammalian model of HD),provide the opportunity to analyze alterations in a physiologically intact model of the disease.Although some disparities in volumetric changes across different brain structures between preclinical and clinical models has been documented,further application of different diffusion MRI techniques used in combination like diffusion tensor imaging,and neurite orientation dispersion and density imaging have proved effective in characterizing early parameters associated to alteration in water diffusion exchange within intracellular and extracellular compartments in brain white and grey matter.Thus,the combination of diffusion MRI imaging techniques and more complex neuropathological analysis could accelerate the discovery of new imaging biomarkers and the early diagnosis and neuromonitoring of patients affected with HD.

A short P-wave duration is associated with incident heart failure in the elderly: a 15 years follow-up cohort study

BACKGROUND Early identification of patients at risk of congestive heart failure(HF)may alter their poor prognosis.The aim was therefore to test whether simple electrocardiographic variables,the P-wave and PR-interval,could predict incident HF.METHODS The PIVUS(Prospective Investigation of the Vasculature in Uppsala Seniors)study(1016 individuals all aged 70 years,50%women)was used to identify predictors of HF.Subjects with prevalent HF,QRS duration≥130 ms,atrial tachyar-rhythmias,implanted pacemaker/defibrillator,second-and third-degree atrioventricular block or delta waves at baseline were excluded.Cox proportional hazard analysis was used to relate the PR interval,P-wave duration(Pdur)and amplitude(Pamp),measured in lead V1,to incident HF.Adjustment was performed for gender,RR-interval,beta-blocking agents,systolic blood pressure,body mass index and smoking.RESULTS Out of 836 subjects at risk,107 subjects were diagnosed with HF during a follow-up of 15 years.In the multivariate analysis,there was a strong U-shaped correlation between Pdur in lead V1 and incident HF(P=0.0001)which was significant for a Pdur<60 ms[HR=2.75;95%CI:1.87-4.06,at Pdur 40 ms]but not for prolonged Pdur.There was no significant relationship between incident HF and the PR-interval or the Pamp.A Pdur<60 ms improved discrimination by 3.7%when added to the tradi-tional risk factors including sex,RR-interval,beta-blocking agents,systolic blood pressure,BMI and smoking(P=0.048).CONCLUSIONS A short Pdur,an easily measured parameter on the ECG,may potentially be a useful marker of future HF,enabling its early detection and prevention,thus improving outcomes.

One-Stage Correction with Intra- and Extraatrial Rerouting of Anomalous Systemic and Pulmonary Venous Return and Intraventricular Repair of Double Outlet Right Ventricle in a Patient with Heterotaxy Syndrome

We report a very rare case of successful intracardiac correction in a patient with heterotaxy syndrome.The cardiac malformations included dextrocardia,double outlet right ventricle,pulmonary stenosis,interrupted inferior vena cava,hemiazygos continuation and total anomalous pulmonary venous return.One-stage correction was performed.The atrial procedure consisted of intra-and extraatrial rerouting of the anomalous systemic and pulmonary venous return.The hepatic veins were detached and diverted to the left atrium via an extracardiac conduit.The correction of the double outlet right ventricle was accomplished by intraventricular redirection of the blood flow from the left ventricle to the aorta.The right ventricular outflow was ultimately remodeled using a valved conduit.For better perception of the complex morphology,a three-dimensional model was designed,using CT scan images.This proved to be very useful for surgical planning,especially with regard to the intraatrial reconstruction of the systemic and pulmonary venous rerouting.

Inflammatory bowel disease(IBD)has as a main characteristic the exacerbation of the immune system against enterocytes,compromising the individual’s intestinal microbiota.This inflammatory cascade causes several nutritional deficiencies,which further compromise immunological functioning and,as a result,worsen the prognosis.This vicious cycle can be interrupted as the patient’s dietary pattern meets their needs according to their clinical condition,acting directly on the inflammatory process of IBD through the interaction of food,intestinal microbiota,and epigenome.Specific nutritional intervention for IBD has a crucial role in preventing and managing disease activity.This review addresses epigenetic modifications through dietary compounds as a mechanism for modulating the intestinal microbiota of patients with IBD.

Obesity and its influence on liver dysfunction,morbidity and mortality after liver resection

Background:Obesity and associated steatosis is an increasing health problem worldwide.Its influence on post-hepatectomy liver failure(PHLF)and after liver resection(LR)is still unclear.Methods:Patients who underwent LR were investigated and divided into three groups[normal weight:body mass index(BMI)18.5-24.9 kg/m2,overweight:BMI 25.0-29.9 kg/m2,obese:BMI≥30 kg/m2]in this retrospective study.Primary aim of this study was to assess the influence of BMI and nonalcoholic steatohepatitis(NASH)on PHLF and morbidity.Results:Of 888 included patients,361(40.7%)had normal weight,360(40.5%)were overweight,167(18.8%)were obese.Median age was 62.5 years(IQR,54-69 years).The primary indication for LR was colorectal liver metastases(CLM)(n=366,41.2%).NASH was present in 58(16.1%)of normal weight,84(23.3%)of overweight and 69(41.3%)of obese patients(P<0.001).PHLF occurred in 16.3%in normal weight,15.3%in overweight and 11.4%in obese patients(P=0.32).NASH was not associated with PHLF.There was no association between patients’weight and the occurrence of postoperative complications(P=0.45).At multivariable analysis,solely major LR[odds ratio(OR):2.7,95%confidence interval(CI):1.83-4.04;P<0.001]remained a significant predictor for PHLF.Conclusions:Postoperative complications and PHLF are comparable in normal weight,overweight and obese patients and LRs using modern techniques can be safely performed in these patients.

Soil ecosystem changes by vegetation on old-field sites over five decades in the Brazilian Atlantic forest

Vegetation types alter soil ecosystems by changing soil fauna community activities and soil physi-cal-chemical properties.However,it is unclear how tree species(natural forest,native and exotic tree plantations)promote changes in the soil ecosystem,and if these changes alter functional groups of soil fauna and ecosystem services.To determine the effects of five decades of old-field veg-etation on soil ecosystems in the Brazilian Atlantic Forest,field sampling of three ecosystems(exotic tree species Pinus elliottii Engelm.plantation,endangered tree species Arau-caria angustifolia(Bertol.)Kuntze plantation,and a natural ecosystem)were carried out,as well using bait-lamina tests and bioassays with collembolans,earthworms and seeds of Lactuca sativa L.Field sampling evaluated the soil fauna community and soil physical-chemical properties.The bait-lamina test in situ was carried out for 14-days to deter-mine fauna feeding activity,and the bioassays evaluated the reproduction of Folsomia candida,the avoidance of Eisenia andrei,and germination of L.sativa in the soil from each ecosystem.The results are:(1)vegetation type altered the soil fauna community composition;(2)soil fauna feeding was reduced in the plantations compared to the natural eco-system;(3)a physical barrier was created by recalcitrant litter that compromised fauna community structure and seed bank germination in situ;and,(4)changes in soil physical-chemical properties promoted decomposers.
