Modified preoperative score to predict disease-free survival for hepatocellular carcinoma patients with surgical resections

BACKGROUND No prognostic models specific to hepatocellular carcinoma patients receiving surgical resection have been considered strong and convincing enough for survival prediction thus far,and there are no models including only preoperative *** derived a nomogram to predict disease-free survival in a previous *** To simplify our score and compare research outcomes among other scoring *** We retrospectively reviewed data from 1106 patients with hepatocellular carcinoma who underwent liver resection at the Linkou Chang Gung Memorial Hospital between April 2003 and December *** analyses were conducted to identify the significant survival ***,Harrell’s C-index,and Akaike information criterion were compared between our score,AJCC 8^(th)edition,Tokyo score,and Taipei Integrated Scoring System(TTV-CTPAFP model).RESULTS Among the 1106 patients,731(66.1%)had tumor recurrence at a median followup of 83.9 *** risk factors were identified:platelet count,albumin level,indocyanine green retention rate,multiplicity,and radiologic total tumor *** were divided into three risk groups,and the 5-year survival rates were 61.7%,39%,and 25.7%,*** C-index was 0.617,which was higher than the Tokyo score(0.613)and the Taipei Integrated Scoring System(0.562)and equal to the value of the AJCC 8th edition(0.617).CONCLUSION The modified score provides an easier method to predict *** treatment can be planned preoperatively by dividing patients into risk groups.

A simple method to indium tin alloy and oxide nanowires were achieved by using the extrusion molding process in the air. Eutectic indium tin bulk were firstly injected into AAO templates. After dissolving AAO, the indium tin nanowires were collected in ethanol. Furthermore, in order to have oxide nanowires, alloy nanowires were directly oxidized by heat treatment in the air below its melting point for 24 hours. Finally, crystalline indium-tin oxide and indium oxide nanowires with diameters 90-110 nm and lengths 2-20 μm were obtained.

Cost-benefit analysis of screening for diabetic retinopathy among Chinese type 2 diabetes: decision from different perspectives of a population-based study

The purpose of this study is to explore whether it is worthwhile to launch a routine diabetic retinopathy (DR) screening for blindness pre-vention among Chinese type 2 diabetes from different perspective based on the popula-tion-based study in Kinmen, Taiwan. A total of 971 community dwelling adults previously di-agnosed with type 2 diabetes in 1991-1993 un-derwent DR screening in 1999-2002 by a panel of ophthalmologists using on-site indirect oph-thalmoscopy and 45-degree color fundus retinal photographs. The cost-benefit analysis is used to evaluate the DR screening. In terms of bene-fit-cost ratio, the different screening programs for DR could save New Taiwan Dollars (NTD) from 14.38 to 36.83 in discounted costs for each dollar incurred in different screening years from the societal viewpoint for Taiwan and save NTD from 0.81 to 1.80 in different screening years from health care payer’s perspective. The av-erage estimate of willingness-to-pay to translate into benefit yields NTD from 937.8 to 4,689 be- nefits per case due to DR screening in different screening years during 10-year follow-up. The net present value of the DR screening were NTD from -167,318 to -307,251.2 in different screening years. In conclusion, it is worthwhile to initial a routine DR screening of Chinese type 2 diabetes for blindness prevention from the societal per-spective but not from consumer decision based on the willingness-to-pay perspective.

降雨是诱发坡地地质灾害的重要原因,因此掌握降雨的趋势与分布规律,可有效地用于坡地灾害预警,但由于降雨预测精度、灾害资料统计的不足与坡地地质条件的不确定性,实际应用降雨预警基准仍存在许多困难。通过分析台风诱发坡地地质灾害的降雨特性,来说明降雨预警基准在防灾减灾工作上应用的条件与限制。结果表明,对于不同类型之坡地灾害与降雨分布特性,需采用不同的雨量预警基准。唯有通过岩土力学、工程地质学与气象学的整合研究,才能有效提高降雨预警基准预测的准确率,因此各学科的整合是未来防灾减灾研究工作的发展趋势。

A Particle Swarm Optimization to Minimize Makespan for a Four-Stage Multiprocessor Open Shop with Dynamic Job Release Time

This paper considers the scheduling problem observed in chip sorting operation of LED manufacturing, where each lot (job) with release time have four operations to be processed on a set of processing stages without pre-determined necessary route. Each stage has one and more identical sorting machines. The sorting machines scheduling problem can be treated as a four-stage multiprocessor open shop problem with dynamic job release, and the objective is minimizing the makespan in the paper. This problem is formulated into a mixed integer programming (MIP) model and empirically shows its computational intractability. Due to the computational intractability, a particle swarm optimization (PSO) algorithm is proposed. A series of computational experiments are conducted to evaluate the performance of the proposed PSO in comparison with exact solution on various small-size problem instances. The results show that the PSO algorithm could finds most optimal or better solutions in one second.

Fecal calprotectin correlated with endoscopic remission for Asian inflammatory bowel disease patients

AIM: To evaluate the correlation between fecal calprotectin(f C), C-reactive protein(CRP), and endoscopic disease score in Asian inflammatory bowel disease(IBD) ***: Stool samples were collected and assessed for calprotectin levels by Quantum Blue Calprotectin High Range Rapid test. Crohn's disease endoscopic index of severity(CDEIS) and ulcerative colitis endoscopic index of severity(UCEIS) were used for endoscopic lesion scoring. RESULTS: A total of 88 IBD patients [36 patients with Crohn's disease(CD) and 52 with ulcerative colitis(UC)] were enrolled. For CD patients, f C correlated with CDEIS(r = 0.465, P = 0.005) and CRP(r = 0.528, P = 0.001). f C levels in UC patients correlated with UCEIS(r = 0.696, P < 0.0001) and CRP(r = 0.529, P = 0.0005). Calprotectin could predict endoscopic remission(CDEIS < 6) with 50% sensitivity and 100% specificity(AUC: 0.74) in CD patients when using 918 μg/g as the cutoff. When using 191 μg/g as the cut-off in UC patients, calprotectin could be used for predicting endoscopic remission(UCEIS < 3) with 88% sensitivity and 75% specificity(AUC: 0.87). CONCLUSION: f C correlated with both CDEIS and UCEIS. f C could be used as a predictor of endoscopic remission for Asian IBD patients.

To response the demand for fine line in electronic products,additive manufacturing process integrated printing techniques and deposited methods to reach fine line circuit,with the merits of reduced material wastage,low fabrication costs,and mass production advantage ***,we have developed additive process in fabricating circuit on flexible substrate through catalyst induced copper electroless deposition(ELD)*** additive processes that integrated printing,activation,and metallization were applied to produce fine-line circuit with 5μm line *** sample with ultraviolet(UV)activation shows better conductive property in comparison with the sample without activation after electroless deposited ***,the results indicated that the reaction of catalyst induced electroless copper plating strongly depends on UV *** fine-line circuit exhibits a narrow line width circuit(around 5μm),lower resistance(6.2μΩ·cm),and mass production with low pollution in comparison with lithographic processes(with photoresist and acid pollution)with a high throughput system(R2R system)for the applications of double side flexible printed circuit board(FPCB).

Complications of airway self-expandable metallic stent in benign airway diseases

Objectives: To present the complications of Ultraflex Self-Expandable Metallic Stents (SEMSs) applied in patients with benign tracheobronchial diseases. Methods: Eighty patients received 124 SEMSs were retrospectively reviewed in a tertiary hospital. Results: The baseline characteristics of patients received SEMS implantation including age (mean + standard deviation: 62.5 + 14.8 years), gender (male, 60.5%), smoking (46.8%), forced expiratory volume in first one second (FEV1) (mean + standard deviation, 0.92 +0.41 L/s), follow-up days after SEMS implantation (median (interquartile range);457 (131 - 897)) and covered SEMS (33.9%) were listed. Symptoms improved after SEMS implantation among 95 patients (76.6%). Time to complications developed was 236 (median;interquartile range, 59 - 672) days. The overall complication rate was 41.9% after SEMS implantation. The complications included SEMS migration (6.5%), granuloma (19.3%) and SEMS fracture (16.1%). Successful management rates of SEMS migration, granuloma and SEMS fracture were up to 100%, 83.3%, and 85% respectively. Conclusions: Patients received SEMS implantation due to benign conditions had poor lung function and were old. The complication rate in patients with benign conditions was high after longer follow-up period, however, successful management achieved in most patients with complications.

Patients with cancer are at high risk for thrombotic events,which are known collectively as Trousseau's syndrome. Herein,we report a 66-year-old male patient who was diagnosed with terminal stage gastric cancer and liver metastasis and who had an initial clinical presentation of upper gastrointestinal bleeding. Acute ischemia of the left lower leg that resulted in gangrenous changes occurred during admission. Subsequent angiography of the left lower limb was then performed. This procedure revealed arterial thrombosis of the left common iliac artery with extension to the external iliac artery,the left common iliac artery,the posterior tibial artery,and the peroneal artery,which were occluded by thrombi. Aspiration of the thrombi demonstrated that these were not tumor thrombi. The interesting aspect of our case was that the disease it presented as arterial thrombotic events,which may correlate with gastric adenocarcinoma. In summary,we suggested that the unexplained thrombotic events might be one of the initial presentations of occult malignancy and that thromboprophylaxis should always be considered.
