The microstructure, phase consistence and microhardness of thermal sprayed coatings were investigated. The tungsten and chromium carbide coatings and also composite NiCrSiB coating were analyzed. The microstructure of coatings were observed by using optical microscopy (MO), scanning electron microscopy (SEM) and transmission electron microscopy (TEM). Almost equiaxial carbide particles settled inside the surrounded material of coating were found. The cracks propagating thorough the particles and along boundaries between the particles and surrounded material were observed. This phenomenon was connected with the porosity of coatings. The decarburization process was detected in coatings by phase composition investigation using X-ray method. The decarburization process was the reason due to which beside initial Cr3C2 the Cr7C3 and Cr23C6 particles were found. In the tungsten coatings beside the initial WC carbides the W2Cones were found.

By enabling the probing of light-matter interactions at the functionally relevant length scales of most materials, near-field optical imaging and spectroscopy accesses information that is unobtainable with other methods. The advent of apertureless techniques, which exploit the ultralocalized and enhanced near-fields created by sharp metallic tips or plasmonic nanoparticles, has resulted in rapid adoption of near-field approaches for studying novel materials and phenomena, with spatial resolution approaching sub-molecular levels. However, these approaches are generally limited by the dominant out-of-plane polarization response of apertureless tips, restricting the exploration and discovery of many material properties. This has led to recent design and fabrication breakthroughs in near-field tips engineered specifically for enhancing in-plane interactions with near-field light components. This mini-review provides a perspective on recent progress and emerging directions aimed at utilizing and controlling in-plane optical polarization, highlighting key application spaces where in-plane near-field tip responses have enabled recent advancements in the understanding and development of new nanostructured materials and devices.

Colon cancer is the second leading cause of cancer mortality in the United States with a median age at diagnosis of 69 years. Sixty percent are diagnosed over the age of 65 years and 36% are 75 years or older. At diagnosis, approximately 58% of patients will have locally advanced and metastatic disease, for which systemic chemotherapy has been shown to improve survival. Treatment of cancer in elderly patients is more challenging due to multiple factors, including disabling co-morbidities as well as a decline in organ function. Cancer treatment of elderly patients is often associated with more toxicities that may lead to frequent hospitalizations. In locally advanced disease, fewer older patients receive adjuvant chemotherapy despite survival benefit and similar toxicity when compared to their younger counterparts. A survival benefit is also observed in the palliative chemotherapy setting for elderly patients with metastatic disease. When treating elderly patients with colon cancer, one has to consider drug pharmacokinetics and pharmacodynamics. Since chronological age is a poor marker of a patient’s functional status, several methods of functional assessment including performance status and activities of daily living (ADL) or instrumental ADL, or even a comprehensive geriatric assessment, may be used. There is no ideal chemotherapy regimen that fits all elderly patients and so a regimen needs to be tailored for each individual. Important considerations when treating elderly patients include convenience and tolerability. This review will discuss approaches to the management of elderly patients with locally advanced and metastatic colon cancer.

Congenital heart disease (CHD) is one of the risk factors for developing infective endocarditis (IE). Right-sided IEoccurs in 5%–10% of endocarditis cases, and pulmonary valve (PV) is involved in less than 2% of such *** data are few, and optimal treatment methods, indications for surgery, and types of operative techniquesare still under debate. We present an adult patient with a rare combination of the ventricular septal defect (VSD)and PV IE who underwent surgical treatment. Neocuspidization with autologous pericardium was utilized for thereconstruction of his PV. We discuss details of this novel surgical technique.

Diabetes mellitus(DM) is the third most common noninfectious disease leading to early disability and high mortality. Moreover, the number of patients is growing every year. The main symptom of DM is hyperglycemia. Increased levels of blood glucose activate polyol, hexosamine, and protein kinase metabolic pathways cause the intensification of non-enzymatic glycosylation and nitration of macromolecules. This, in turn, leads to the development of oxidative and nitrative stresses and secondary complications, such as different kinds of microand macroangiopathies. Metabolic disorders caused by insulin deficiency in diabetes significantly impede the functioning of a homeostasis system, which change the physical, biochemical, morphological, and functional properties of blood cells. As a result, the oxygen-transport function of red blood cells(RBCs), rheological properties of the blood, and functions of immunocompetent cells as well as the process of apoptosis are primarily affected. Modern pharmacotherapy focuses on the search for new preparations that aim to decrease blood glucose levels. Undesirable side effects and adverse reactions caused by synthetic medicines led to the search and investigation of new preparations of natural origin. Medicinal mushrooms play an important role among such new preparations. They are a source of a large number of high-and lowmolecular compounds with pronounced biological effects. Our investigations show pronounced hypoglycemic and anti-anemic action of submerged cultivated mycelium powder of medicinal mushrooms Agaricus brasiliensis(A. brasiliensis) and Ganoderma lucidum(G. lucidum) on streptozotocin-induced DM in rats. Also, we showed that mycelium powders have membrane protective properties as evidenced by the redistribution of RBC populations towards the growth of full functional cell numbers. A Systematic Approach to Patient-Specific Quality Assurance for Volumetric Modulated Arc Therapy

We developed the first non-commercial treatment planning system for volumetric modulated arc therapy(VMAT) in the United States. Because VMAT involves multi-parameter modulations, it is imperative to develop a comprehensive, rigorous and yet, practical procedure for routine patient-specific quality assurance(QA). In this paper, we presented our own approach as being currently implemented in our *** patient-specific QA procedure involves multi-levels: pre-treatment QA,on-treatment QA, and posttreatment QA. The pre-treatment QA focuses on dosimetry verification, which is done with the commercial Map CHECK in Map PHAN mounted on an isocentric mounting fixture(IMF). This method is also referred to the fixed-gantry technique,i.e., the beams always remain perpendicular to the detector plane. The on-treatment QA involves in vivo optically stimulated luminescent dosimetry(OSLD).Prior to the treatment, two nano Dot TM OSLD dosimeters are placed on the patient abdomen under 1 cm bolus at the isocenter location. The irradiated dosimeters are then read by a nano Dot TM reader and the average reading of the two is calculated. The post-treatment QA involves the analysis of the Dyna Log and DLog files. The Dyna Log is a treatment log file that contains the planned and actual leaf positions at a given gantry angle. The DLog is a treatment log file that contains the planned segmented treatment table(STT) and the corresponding segment boundary samples, i.e., the actual delivered MU and gantry angle increment at each control point.

Hurricane fighting devices are recommended. The devices belong to the sphere of artificial weather conditions changes over the Earth surface. The purpose of the paper is the development of the original design of devices for hurricane fighting. The decreasing of hurricane energy takes place under the reaction of directed air current against the hurricane air current mass. The air current is created by propellers, located in cylindrical frames with nozzles at a current exit. The devices function due to horizontal hurricanes air current. The essential factor lies in absence of influence on ecology. The devices are of simple constructions, it results in simplification of their production technology and lower cost,

The measuring system for estimation of power of wind flow generated by the train movement has been created. The advantages of the proposed system are the cheapness and simple design. With its simplicity of design and easy build-up of channels, designed measuring system can be used for a wide range of technical problems. This paper describes the design process, validation and conducting the first field test of this measuring system.

The arms-up position is the most common treatment position adopted for lung cancer patients treated with radiation therapy. However, many elderly or frail patients have shoulder problems and cannot tolerate such an overstretched position for an extended period. Therefore, the arms-down position becomes the only alternative for this group of patients during radiation therapy. Even though the arms-down position is not ideal, it does provide a stable and comfortable patient immobilization position for radiation treatments that require a longer delivery time, such as stereotactic body radiation therapy (SBRT). In this study, we designed a protocol to treat lung cancer patients with VMAT stereotactic body radiation therapy (VMAT SBRT) and deep inspiration breath-hold (DIBH) in the arms-down position. Our initial clinical experience with this protocol indicates that it is reliable for patient immobilization and accurate in delivered dosimetry.

Each year, 170,000 cancer patients in the United States develop brain metastases. Many of them present with multiple small lesions. Historically, Linac-based stereotactic radiosurgery (SRS) was used to treat single solitary brain metastasis with a diameter of less than 3.0 cm, while whole brain radiation therapy (WBRT) was used to treat multiple brain metastases mainly as palliative therapy. Evidence-based practices reveal that WBRT results in poor treatment outcomes, with high local recurrence rates, decreased cognitive function, and even the onset of dementia. Recently, volumetric modulated arc therapy (VMAT) SRS has been tested as an alternative treatment to WBRT. Owing to its inherent complexity and high risk, it is imperative to perform rigorous testing prior to its clinical implementation. In this paper, we present a novel technique for dosimetry validation of VMAT SRS.
