Deprescribing antihypertensive treatment in nursing home patients and the effect on blood pressure

Background It is debatable whether treating multimorbid nursing home patients with antihypertensive drugs produces beneficial effects. Most cardiovascular guidelines promote treatment; few have advice on how to deprescribe when treatment may no longer be necessary. We investigated the effect of medication review on antihypertensive drug use and the association between cognition, blood pressure, and prescribing. Methods From August 2014 to December 2015, 765 patients from 72 units （clusters） in 32 Norwegian nursing homes were included in a 4-month, multicentre, cluster-randomized, controlled trial, with 9-month follow-up. Patients ≥ 65 years old with antihypertensive treatment （n = 295, 39%） were randomized to systematic medication review where the physician received support from peers （collegial mentoring） or were given care as usual （control condition）. Outcome measures were the number of antihypertensive drugs, systolic blood pressure, and pulse. We used hospitalizations and deaths as criteria to assess harm. Results At baseline, each patient used 9.2 ± 3.5 regular drugs, and 1.6 ± 0.7 antihypertensives. Mean blood pressure was 128/71 mmHg and 9% had a systolic pressure ≥ 160 mmHg. Between baseline and month four, antihypertensives were deprescribed to a significantly higher extent in the intervention group （n = 43, 32%） compared to control （n = 11, 10%）; Incidence Rate Ratio = 0.8, 95% CI = 0.7？0.9. In the intervention group, there was an immediate increase in systolic blood pressure when antihypertensives were reduced, from baseline 128 ± 19.5 mmHg to 143 ± 25.5 mmHg at month four. However, at month nine, the blood pressure had reverted to baseline values （mean 134 mmHg）. Deprescription did not affect pulse and systolic pressure. The number of hospitalizations was higher in control patients at month four （P = 0.031） and nine （P = 0.041）. Conclusion A systematic medication review supported by collegial mentoring significantly decreased the use of antihypertensive

Protein import into chloroplasts has been a focus of research for several decades. The first publications dealing with this fascinating topic appeared in the 1970s. From the initial realization that many plastid proteins are being encoded for in the nucleus and require transport into their target organelle to the identification of import components in the cytosol, chloroplast envelopes, and stroma, as well as elucidation of some mechanistic details, more fascinating aspects are still being unraveled. With this overview, we present a survey of the beginnings of chloroplast protein import research, the first steps on this winding road, and end with a glimpse into the future.

Can mouse models mimic sporadic Alzheimer’s disease?

The biology and ecology of biological soil crusts,a soil surface community of mosses,lichens,cyanobacteria,green algae,fungi,and bacteria,have only recently been a topic of *** efforts began in the western U.s.(Cameron,Harper,Rushforth,and ***),Australia(Rogers),and Israel(Friedmann,Evenari,and Lange)in the late 1960s and 1970s(e.g.,Friedmann et al.1967;Evenari 1985,reviewed in Harper and Marble 1988).However,these groups worked independently of each other and,in fact,were often not aware of each other’s *** addition,biological soil crust communities were seen as more a novelty than a critical component of dryland eco*** then,researchers have investigated many different aspects of these communities and have shown that although small to microscopic,biological soil crusts are critical in many ecological processes of *** often cover most of desert soil surfaces and substantially mediate inputs and outputs from desert soils(Belnap et al.2003).They can be a large source of biodiversity for deserts,as they can contain more species than the surrounding vascular plant community(Rosentreter 1986).These communities are important in reducing soil erosion and increasing soil fertility through the capture of dust and the fixation of atmospheric nitrogen and carbon into forms available to other life forms(Elbert et al.2012).

Tree-ring:a suitable implement for spatial and temporal fire distribution analysis in savanna woodland and dry forest

stability of Ni/siO2-ZrO2 catalysts towards steaming and coking in the dry reforming of methane with carbon dioxide

Ni/siO2-ZrO2 catalysts with Ni loadings of 1 to 13 wt-% were prepared, characterized by elemental analysis, X-ray diffraction, N2 sorption, temperature programmed oxidation, temperature programmed reduc- tion, and tested for their activity and stability in the dry reforming of methane with carbon dioxide at 850 ℃, gas hourly space velocity of 6000 and 1800 h^-1 and atmo- spheric pressure. The siO2-ZrO2 support as obtained through a simple and efficient sol-gel synthesis is highly porous （ABET = 90 m^·g^-1, dp= 4.4 nm） with a homogeneously distributed si-content of 3 wt-%. No loss of si or formation ofmonoclinic ZrO2, even after steaming at 850 ℃ for 160 h, was detectable. The catalyst with 5 wt-% Ni loading in its fully reduced state is stable over 15 h on-stream in the dry reforming reaction. If the catalyst was not fully reduced, a reduction during the early stages of dry reforming is accompanied by the deposition of up to 44 mg· g^-1 carbon as shown by experiments in a magnetic suspension balance. Rapid coking occurs for increased residence times and times-on-stream starting at 50 h. The Ni loading of 5 wt-% on siO2-ZrO2 was shown to provide an optimal balance between activity and coking tendency.

Longitudinal monitoring and prediction of long-term outcome of scar stiffness on pediatric patients

Background:Hypertrophic scarring after burn injury is one of the greatest unmet challenges in patients with burn injuries.A better understanding of the characteristics of scar maturation and early prediction of the long-term outcome of scarring are prerequisites for improving targeted therapies and pivotal for patient ***:Repeated measurements of scar stiffness in 11 pediatric patients were performed over the course of 1 year using 2 suction devices,the Cutometer and the *** addition,the observer pliability score of the Patient and Observer scar Assessment scale was *** longitudinal study allowed quantification of the ability of each of the measured parameters to reflect scar maturation,as indicated by change in skin pliability/stiffness,over time(using linear regression);the ability to distinguish individual patients(intraclass correlation coefficient(ICC));the correlation of the devices(spearman correlation coefficient);and the ability to predict longterm scar maturation based on early scar assessment(using receiver operating characteristic).Results:All the tools used showed significant longitudinal decrease of scar stiffness from 3 months until 12 months after the *** Nimble(ICC_(patient)^(Nimble)=0.99)and the Cutometer(ICC_(patient)^(Cuto)=0.97)demonstrated an excellent ability to distinguish between individual *** Nimble seemed to be able to predict the 12-month pliability of scars based on early(3-month)measurements(area under the curve(AUC)_(12m)^(POsAs)=0.67;AUC_(12m)^(C)=0.46;AUC_(12m)^(N)=0.79).Conclusions:The results of this preliminary study suggest that all 3 tools provide suitable means to quantify alterations in scar stiffness over *** evidence suggests the Nimble is most favorable for predicting changes in stiffness associated with long-term scar *** studies with a larger sample size are required to validate tissue suction as a clinical tool for analysis of changes of scar stiffne
