The high penetration of distributed generation(DG)has set up a challenge for energy management and consequently for the monitoring and assessment of power quality(PQ).Besides,there are new types of disturbances owing to the uncontrolled connections of non-linear *** stochastic behaviour triggers the need for new holistic indicators which also deal with big data of PQ in terms of compression and scalability so as to extract the useful information regarding different network states and the prevailing PQ disturbances for future risk assessment and energy management *** and continuous monitoring would guarantee the report to claim for damages and to assess the risk of PQ *** this context,we propose a measurement method that postulates the use of two-dimensional(2D)diagrams based on higher-order statistics(HOSs)and a previous voltage quality index that assesses the voltage supply waveform in a continous monitoring *** suitable for both PQ and reliability applications,the results conclude that the inclusion of HOS measurements in the industrial metrological reports helps characterize the deviations of the voltage supply waveform,extracting the individual customers’pattern fingerprint,and compressing the data from both time and spatial *** method allows a continuous and robust performance needed in the SG ***,the method can be used by an average consumer as a probabilistic method to assess the risk of PQ deviations in site characterization.

The role of clinical diagnosis criteria on the frequency of accommodative insufficiency

AIM: To estimate and compare the frequency of accommodative insufficiency(AI) within the same clinical population sample depending on the type of clinical criteria used for diagnosis. Comparing the frequency within the same population would help to minimize bias due to sampling or methodological variability. METHODS: Retrospective study of 205 medical records of symptomatic subjects free of any organic cause and symptoms persisting despite optical compensation evaluated. Based on the most commonly clinical diagnostics criteria found in the literature, four diagnostics criteria were established for AI(Ⅰ, Ⅱ, Ⅲ and Ⅳ) based on subjective accommodative tests: monocular accommodative amplitude two or more diopters below Hofstetter's minimum value [15-(0.25×age)](Ⅰ, Ⅱ, Ⅲ, Ⅳ); failing monocular accommodative facility with minus lens, establishing the cut-off in 0 cycles per minute(cpm)(Ⅰ) and in 6 cpm(Ⅱ, Ⅲ); failing binocular accommodative facility with minus lens, establishing the cut-off in 0 cpm(Ⅰ) and in 3 cpm(Ⅱ).RESULTS: The proportion of AI(95%CⅠ) for criteria Ⅰ, Ⅱ, Ⅲ and Ⅳ were 1.95%(0.04%-3.86%), 2.93%(0.31%-4.57%), 6.34%(1.90%-7.85%) and 41.95%(35.14%-48.76%) respectively, with a statistically significant difference shown between these values(χ2=226.7, Plation varies with the clinical diagnostic criteria selected. The variation is statistically significant when considering the monocular accommodative amplitude as the only clinical diagnostic sign.

In this paper we study some van den Bergh open clusters combining photometry and astrometry. A model which analyses the proper motion distribution and the stellar density is applied to find the kinematic parameters and stellar membership in the region of the open clusters vdB92, vdB146 (NGC 7129) and vdB150. The astrometric data are obtained from UCAC4 catalogue. The centre coordinates, the components of mean proper motion, the angular diameter and the astrometric members are reported, taking the following values: for vdB92: α=105.97281°±0.13113°, δ=−11.57814°±0.10575°,?μαcosδ=−3.46±0.19mas/yr,μδ=1.27±0.19mas/yr, 34', 60 members;for vdB146: α=325.78423°±0.15297°, δ=66.13575°±0.02907°, μαcosδ=−2.71±0.25mas/yr, μδ=−3.32±0.25mas/yr, 9', 5 members;and for vdB150: α=332.22519°±0.06074°, δ=73.40232°±0.0.07789°, μαcosδ=3.07±0.90mas/yr, μδ=4.65±0.90mas/yr, 15', 7 members. The incidence of the proper motion errors in the determination of the cluster parameters and of the stellar membership is analysed and it is found that they do not significantly change. We finally compare the astrometric members with the photometric ones given in the literature.

Pulmonary artery intimal sarcoma(PAIS) is a rare tumor with a very poor prognosis. Clinical and radiological findings usually mimic thromboembolic disease, leading to diagnostic delays. The treatment of choice is surgery, and adjuvant chemotherapy and radiotherapy have limited results. We report the case of a 48-year-old male patient, initially suspected with pulmonary thromboembolism. The angio-CT revealed a filling defect in the pulmonary artery trunk. The patient underwent surgery, resulting in with complete resection of the mass with a diagnosis of PAIS. The tumor progressed rapidly in the lung, requiring surgery of multiple lung metastases. The patient was treated with stereotactic body radiation therapy(SBRT) on two occasions for new pulmonary lesions. In the last follow-up(4 years after initial diagnosis), the patient was disease-free. In conclusion, SBRT proved to be an alternative treatment to metastasectomy, allowing palliative chemotherapy to be delayed or omitted, which may result in improved quality of life.

Thyroid Cartilage Chondrosarcoma Review: Management and Prognosis of a Rare Tumor

Objective: This article is a critical review of the literature concerning thyroid cartilage chondrosarcoma and is particularly focused on the management and prognosis of this rare entity. Study Design: A review of all the cases of thyroid cartilage chondrosarcoma reported in the literature up to January 2013. Methods: The search was carried out through the introduction of the MeSH terms: Chondrosarcoma, laryngeal Cartilages, Thyroid Cartilage, Therapeutics. All the studies related to thyroid cartilage chondrosarcoma were selected together with a newly presented case. Results: A total of 47 cases have been described in the literature, and 35 of these include sufficient data for statistical analysis. The age of patients ranged from 40 to 77 years, with a male predilection (88.6%). The main symptoms were neck mass and hoarseness. Grade I and II tumors were most frequent. A total of 13 cases described an extension of the tumor beyond the thyroid cartilage. The most common treatment was partial laryngectomy (50%). All patients who died were male, with grade II or II-III tumor or associated sarcoma, had metastases and received radiotherapy in addition to total laryngectomy. Conclusion: Patient age and tumor size does not influence the prognosis. Tumor grade I and I-II have good prognoses, whereas the prognosis is unpredictable in tumors between grade II and III. Patients with associated sarcoma, or metastasis, have a worse prognosis. Partial laryngectomy is a good therapeutic option, although total laryngectomy is sometimes required in order to ensure complete resection.

Thoracic Unilateral Spinal Cord Syndrome Following Neuraxial Anaesthesia for Periprosthetic Knee Fracture: A Case Report

Neuraxial anaesthesia is widely used in surgical procedures;overall, epidural and intrathecal techniques. Nevertheless, several outcomes should be considered. The incidence of neurologic complications after neuraxial anaesthesia is not perfectly clear (0% - 0.08%), although there are several described cases of spinal cord ischemia. We present a case of thoracic unilateral spinal cord syndrome following lumbar spinal anaesthesia for periprosthetic knee fracture. Our patient suffered monoparesis in her left lower limb as well as decreasing of muscle strength and loss of tendon reflexes. The MNR showed left hyperintense intra-cord images from T7 to T12 attributed to spinal cord oedema and a lineal hypointensity related to minimal haematic component. What made this case surprising was the fact that spinal anaesthesia was performed between L3 and L4 and the patient did not suffer paraesthesia associated with local anaesthetic injection. She was treated with glucocorticoids, gabapentin and amitriptyline. She also was checked by physical rehabilitators, neurologists and Pain Unit physicians. We have found another case reported in the literature about thoracic cord injury after lumbar spinal puncture. In this paper, we report possible aetiologies according to a review and neurological evolution of the patient seven months later.
