Grandmultiparity: Incidence, consequences and outcome in the Niger delta of Nigeria

Sub-Saharan Africa has the highest rates of maternal and neonatal mortality worldwide. Grandmultiparity has been traditionally considered as a risk factor for adverse pregnancy outcome. The aim of this study is to determine the incidence, neonatal and maternal outcome of pregnancy in grandmultiparous women. This is a retrospective cross sectional study of grandmultiparous patients that were managed at the University of Port Harcourt Teaching Hospital, Rivers State, Nigeria, from 1st April 2007 to 31st March 2009. There were a total of 6760 deliveries during the study period, out of which 130 were grandmultiparous giving an incidence of 1.92%. Their mean age was 35.99 years and mean parity 5.70. Most of the patients 62 (47.5%) were para 5. About 71 (54.5%) had seconddary education, while 30 (22.8%) had tertiary level of education, 93 (71.3%) were Booked, while 37 (28.7%) were unbooked. Hypertension, malaria, antepartum haemorrhage, diabetes mellitus, anaemia and abnormal lie and presentation occurred in 68 (52.6%) during the Antenatal period with some patients having more than one complication. Labour complications occurred in 25 (19.0%) of the patients and 36 (27.4%) had anaemia at delivery. 80 (61.4%) achieved vaginal delivery. The caesarean section rate was 34.7%, and 74.3% were performed as emergency procedure. The blood transfusion rate was 12%. The maternal mortality ratio was 1538.46/100,000 live births and the perinatal mortality rate was 107.69/1000 births. It was concluded that the incidence of Grandmultiparity is low in our center compared to other centres in Nigeria, however, they tend to have unfavorable obstetric outcome. Concrete measures must be put in place to address these through specialized antenatal care and adequate management of Unbooked Emergencies.

Prevalence, Pattern and Risk Factors of Soil Transmitted Helminth Infections amongst Children in a Tertiary Institution in South East, Nigeria

Introduction: Soil-transmitted helminthic infection (STHI) is a common public health challenge of children in the most deprived communities in low income countries. In the long-term, STHI can cause developmental and growth disorders leading to future learning defect. Objective: Our aim was to determine the prevalence and pattern of soil-transmitted helminthic infection among children attending a tertiary hospital in Imo State, Nigeria. Patients and Method: The study involved a cross-sectional survey of 268 children, aged 7 months to 18 years seen in a tertiary health facility in Nigeria;from August to December 2022. Data were collected using a structured questionnaire and stool samples were analyzed for intestinal helminths using the Kato-Katz method. Results: The prevalence of soil-transmitted helminthic infection (STHI) was 38.4%. Of all STHIs, Ascaris lumbricoides was the commonest geohelminth observed, 81 (62.1%). Multiple infections were noted in 25 (62.4%) of the specimen. The prevalence of soil-transmitted helminthic infection amongst subjects’ 5 - 9 years was high and least in children older than 15 years. This difference was not statistically significant (p = 0.3407). Statistically significant relationship was detected between STHI and low socioeconomic class. Conclusion: The high prevalence rate of soil-transmitted helminthic infection amongst the subjects is disturbing. This high rate justifies strengthening a structured and routine deworming amongst children in order to improve outcome.

Assessment of Heavy Metal Contamination in Sea Bed Sediment of the Bight of Bonny, Southern Atlantic Coast of Nigeria Using Index Model Analysis

The heavy metal contamination status of Bight of Bonny sea bed sediment was investigated. Sediment samples were collected from eleven locations grouped into three stations. The samples were analyzed to determine the heavy metals (Fe, Pb, Ni, Zn, Mn, Co, Cd, Cu, Cr) concentration using atomic absorption spectrophotometer. Index model analyses, such as geo-accumulation index (I-geo), Enrichment factor (EF), and Pollution load index (PLI) were used for pollution assessment of sea bed sediment. The most predominant heavy metals in the upstream sediment were Fe;13.57 ± 1.838 mg/kg, cobalt;7.987 ± 3.550 mg/kg, and Ni;1.020 ± 0.802 mg/kg, midstream sediment were Fe;8.554 ± 3.010 mg/kg, Co;8.520 ± 4.00 mg/kg, and Ni;1.140 ± 0.879 mg/kg, while the downstream sediment result had Fe;11.12 ± 3.825 mg/kg, Co;7.275 ± 1.700 mg/kg, and Cd;1.025 ± 0.159 /kg. The I-geo results indicate that Cu, Mn, Pb, Cr, Fe, Zn, Ni, and Co were in their background concentrations while Cd had I-geo > 3 which implies a moderately or heavily polluted environment. The EF indicates metal enrichment from anthropogenic sources for Co, Ni, Pb, Cd, Cu, and Cr while Fe and Mn predicate biogenic origin. The PLI of the heavy metals in the three sample stations of sea bed sediment was found to be generally low (cumulation index strongly suggest the Bight of Bonny sea bed sediment contamination by Cd from anthropogenic sources.

Cervical Ripening for Induction of Labor: A Randomized Comparison between Vaginal Misoprostol versus Foley’s Catheter Placement in a Nigeria Tertiary Hospital

Background: Cervical ripening is prerequisite of successful induction of labor. Vaginal misoprostol and Foley’s catheter placement have been widely used for this purpose but the data are not always sufficient. We attempted to determine which (misoprostol versus Foley’s catheter) is more effective/safer in Nigerian setting. Methods: A randomized controlled trial was performed at Federal Teaching Hospital Abakaliki, Nigeria, involving 135 term pregnant women requiring cervical ripening and labor induction. Participants were randomly allocated to misoprostol versus catheter group. The following were recorded/measured/analyzed: Bishop’s score, age, parity, body mass index, gestational age, labor duration, indication, oxytocin use, mode of delivery, and Apgar score. Chi square test and t test were used where appropriate. Results: At 24 hours, all of misoprostol group were either in labor, had ripe cervix, or had delivered, whereas 35.4% of catheter group had still unripe cervix (x2 = 29.856, P = 0.0001). Effects and correlates of continuous training programme on psychosocial status and white blood cell count in men with essential hypertension: A randomized controlled trial

Aim: The aim of this study was to determine the effect and relationship of continuous training programme on WBCc and psychosocial status of black African (Nigerian) subjects with hypertension. Methods: Nigerian subjects with diagnosis of hypertension attending the hypertensive clinic of Murtala Muhammed Specialist Hospital (MMSH), Kano, Nigeria form the population for the study. 217 subjects with mild to moderate (systolic blood pressure [SBP] between 140-180 & diastolic blood pressure [DBP] between 90-109 mmHg) essential hypertension were age matched and randomly grouped into continuous (112) & control groups (105). The continuous group involved in an 8 weeks continuous training (60%-79% HR max) of between 45 minutes to 60 minutes, 3 times per week, while the controls group remain sedentary. SBP, DBP, WBCc, VO2max and psychosocial status were assessed. Student t test and Pearson correlation test were used in data analysis. Results: The study revealed a significant beneficial effect of continuous training programmes on VO2max, SBP, DBP, WBCc and psychosocial status (p < 0.05). Psychosocial status and WBCc were positively and negatively correlated respectively with VO2max at p < 0.01. Conclusions: This study supports the recommendations of moderate intensity (continuous) training program as an adjunct multi-therapy in blood pressure, inflammatory and psychosocial stress management in hypertension.

At present there is no conclusive evidence that participation in exercise affects menstruation and is equivocal and unclear whether menstrualtion affects athletic performance. An attempt has therefore been made in this paper to discuss the effects of menstruation on exercise performance and vice versa. The paper reviewed that women experiencing pre-menstrual syndrome will not likely perform well and that menstrual irregularity is much higher in athletes than sedentary women. The paper also reviewed that athletes reach menarche later than sedentary sisters. The review concluded that competing women experiencing premenstrual syndrome might get control over their menstruation through medically recommended low dose oral contraceptive. The paper recommended that athletes experiencing menstrual irregularities and disturbances should seek medical consultation and management before further training.

The Cretaceous shallow marine Gboko limestone, Yandev, Nigeria is a component of the sedimentary fill of the 800 km NE-SW trending Benue Trough, Nigeria. The limestone is made up of thin bedded to massive limestone beds interspersed with laminated grey shale having foraminifera as the dominant fossil. The limestone has both mud supported and grain supported texture, and micrites constitute about 75% of the limestone. Bulk chemical composition analysis of the limestone reveals average CaCO3 of 92.41% and a range of 77.50% - 99.00%. Mineralogical impurities include quartz, dolomite, pyrrhotite, fluorapatite etc. Trace elements concentration analysis was carried out using Energy Dispersive X-ray (EDXRF) spectrophotometry and showed the following trace elements: Mn (841.3 ppm), Sr (444.6 ppm), Fe (470 ppm), Zn (114.6 ppm) and Pb (116.4 ppm). Calcining the limestone in a laboratory muffle furnace at 1050°C for 90 minutes produced a compact, soft burnt porous and reactive lime that does not crumble into fines. The lime so produced neither meets the requirements of the Steel Making Shop (SMS) of the Ajaokuta Steel Plant nor could it be used in the growing sugar refining industry in Nigeria. It can however be used in the food and the food by-products industry, environmental, agricultural and petroleum industries etc. The raw stone remains a major source of raw materials for cement manufacture for the ever expanding building industry.

Objective:To investigate the the analgesic,anti-inflammatory and antipyretic activities of the methanolic leaf extract of Maerua crassifolia in mice and ***:Acetic acid-induced writhing and tail immersion methods were used to assess analgesic activity,while xylene and carrageenan-induced paw oedema methods were used to evaluate the anti-inflammatory effect of the leaf *** and amphetamine-induced pyrexia were used to investigate the antipyretic *** phytochemical analysis and oral acute toxicity of the methanolic leaf extract of Maerua crassifolia were also ***:The leaf extract(100,200,and 400 mg/kg)showed a dose dependent and significant(P<0.05)inhibition of pain in acetic acid-induced writhing and tail immersion *** extract also produced significant(P<0.05)anti-inflammatory activity in both paradigms.A significant(P<0.05)reduction in hyperpyrexia was also observed with the leaf *** phytochemical screening revealed the presence of alkaloids,flavonoids,terpenoids,tannins,steroids,resins,saponins and cardiac *** oral median lethal dose of the leaf extract was estimated to be greater than 5000 mg/kg in ***:The findings confirmed its ethnomedical use in the treatment of pains and feverish conditions.
