AIM： To evaluate the effects of indomethacin [dual cyclooxygenase （cox）-I/cox-2 inhibitor] and 3-（3,4-difluorophenyl）-4-（4-（methylsulfonyl） phenyl）- 2-（5H）-furanone （MF-tricyclic）（cox-2 selective inhibi- tor） in a rat experimental model of Barrett＇s esophagus and esophageal ***： A total of 112 surviving post-surgery rats were randomly divided into three groups： the control group （n = 48）, which did not receive any treatment; the indomethacin group （n = 32）, which were given 2 mg/kg per day of the cox-I/cox-2 inhibitor; and the MF-tricyclic group （n = 32）, which received 10 mg/kg per day of the selective cox-2 inhibitor. Randomly se- lected rats were killed either 8 wk or 16 wk after sur- gery. The timing of the deaths was in accordance with a previous study performed in our group. Only rats that were killed at the times designated by the protocol were included in the study. We then assessed the histology and prostaglandin E2 （PGE2） expression levels in the rat esophagi. An additional group of eight animals that did not undergo esophagojejunostomy were included in order to obtain normal esophageal tissue as a control. RESULTS： Compared to a control group with no treat- ment （vehicle-treated rats）, indomethacin treatment was associated with decreases in ulcerated esophageal mucosa （16% vs 35% and 14% vs 17%, 2 mo and 4 mo after surgery, respectively; P = 0.021）, length of intestinal metaplasia in continuity with anastomosis （2 4- 1.17 mm vs 2.29 ＋ 0.75 mm and 1.25 4- 0.42 mm vs 3.5 4- 1.54 mm, 2 mo and 4 mo after surgery, respec- tively; P = 0.007）, presence of intestinal metaplasia beyond anastomosis （20% vs 71.4% and 0% vs 60%, 2 mo and 4 mo after surgery, respectively; P = 0.009）, severity of dysplasia （0% vs 71.4% and 20% vs 85.7% high-grade dysplasia, 2 mo and 4 mo after surgery, re- spectively; P = 0.002）, and adenocarcinoma incidence （0% vs 57.1% and 0% vs 60%, 2 mo and 4 mo after surgery, respectively; P 〈 0.0001）. Background: According to the main prostate cancer guidelines, the response to treatment with abiraterone plus prednisone (AA+P) must be evaluated by assessing prostate-specific antigen (PSA) levels at 12 weeks. Recent studies have shown that early PSA decline, at 4 weeks, maybe a surrogate marker for survival. The objective of this work was to analyze if a decline in PSA at 4 weeks correlates with a better outcome in terms of OS (overall survival) and PFS (progression-free survival). Methods: We evaluated 168 patients (with a median age of 71 years) with prostate cancer who had started AA+P treatment between February 2012 and July 2019. Patients were divided into three different groups according to the decline of PSA (≥30%, ≥50%, and ≥90%) at 4, 8, and 12 weeks. Statistical survival analysis was performed using the Kaplan-Meier method. Results: After a follow-up of 69 months, a PSA decline ≥ 30% at 4 weeks was associated with longer median OS times (28 vs. 18 months;p = 0.027). A decline in PSA by ≥50% was also associated with increased median OS times (36 vs. 21;p = 0.003). cox univariable analysis indicated that a decrease in PSA (both by ≥30% and ≥50) were predictive of OS at 4 weeks (PSA ≥ 30%: HR = 1.568, 95%CI [1.041, 2.360], p = 0.031;PSA ≥ 50%: HR = 1.901, 95% CI [1.222, 2.956], p = 0.004);although multivariable analysis did not confirm these results. The prior administration of chemotherapy was an independent risk factor for death (HR = 2.511;p Conclusion: A decrease in PSA by ≥30% or ≥50% at 4 weeks after starting treatment with AA+P correlated with longer PFS and OS, and provides clinically meaningful information guiding the physicians towards a personalized treatment.

目的评价局部晚期乳腺癌(LABC)F-脱氧葡萄糖(FDG)PET/CT显像的纹理分析参数在疗效和预后预测中的价值。方法该前瞻性研究包括68例LABC患者,纳入者均有新辅助化疗(NC)指征,并在化疗前行F-FDG PET/CT显像。对乳腺癌穿刺标本依据分子表型进行分组[雌激素受体(ER)、孕激素受体(PR)及人表皮生长因子2(HER2)的表达、细胞增殖核抗原Ki-67指数等)],并在NC结束后将患者分为有效组和无效组。所有患者经标准的随访后得到无病生存时间(DFS)和总生存时间(OS)。利用计算机软件对乳腺癌病灶进行三维(3D)图像分割,随后进行纹理特征分析,获得行程长度矩阵(RLM)和共生矩阵(CM)参数。对不同危险度的纹理特征与各种分子表型的关系进行分析。使用Kaplan-Meier生存分析和cox比例风险分析对纹理参数、分子表型和NC的肿瘤组织学反应等在预测疗效和预后中的作用进行研究。通过受试者工作特征(ROC)曲线分析取得最佳截断值、曲线下面积(AUC)及与OS、DFS相关的灵敏度和特异性。结果 NC对18例患者有效,其DFS和OS分别为(70.87±21.85)个月和(76.77±18.80)个月。长行程增强(LRE)和长行程高灰阶增强(LRHGE)与不同的危险度分层有关,而低灰阶行程增强(LGRE)、LRHGE和行程长度不均匀性(RLNU)均与NC的疗效相关。单因素分析结果表明纹理特征分析各变量与OS和DFS相关。多因素cox回归分析结果显示,短行程高灰阶强化(SRHGE)下的PET分期与OS相关,而PET分期、高灰阶行程强化(HGRE)均与DFS相关。结论F-FDG PET/CT图像的纹理特征分析各变量可预测新辅助化疗的疗效和预后,并在预测OS和DFS时与PET分期具有相关性。

目的质子泵抑制剂(PPIs)与胃萎缩恶化相关,尤其是在幽门螺杆菌(HP)感染者中。我们确定了在接受抗HP治疗的HP感染者中,PPIs使用与胃癌(GC)之间的相关性。设计本研究以中国香港全区健康资料库为基础。我们首先确认出自2003至2012年间接受过以克拉霉素为基础的三联疗法门诊处方的成年人。排除标准:使用该处方治疗失败的患者,HP治疗后12个月内确诊为GC的患者,以及治疗后发生胃溃疡者。GC排除前的6个月内开始PPIs或H2受体阻滞剂(H2RA)治疗,以避免原始偏倚。通过C。x比例风险模型的倾向分数调整,评估GC发生风险与质子泵抑制剂二者之间的关系。H2RA作为阴性对照暴露。结果共63 397名合格受试者入选,在平均7.6年的随访时间中,有153(0.24%)名发生GC。PPIs使用与GC发生风险增高相关[危害比(HR)2.44,95%可信区间(C/)1.42-4.20],而H2RA无相关性(“R 0.72,95%CI 0.48-1.07)0GC发生的风险随PPIs使用时间的延长而增高(PPIs使用时间/年22年和年的GC发生风险分别为:HR 5.04.95%CI 1.23-20.61;6.65,1.62-27.26和8.34,95%CI 2.02-34.41)。PPIs使用相比非PPIs使用,调整的绝对风险差为每10000人年超过4.29 GC(95%C/1.25-9.54)。结论即使在HP根除治疗后,长期使用质子泵抑制剂仍与GC的发生风险增加相关。
