Identification of speckle-type POZ protein somatic mutations in African American prostate cancer

The speckle-type POZ protein （SPOP） is a tumor suppressor in prostate cancer （PCa）. SPOP somatic mutations have been reported in up to 15% of PCa of those of European descent. However, the genetic roles of SPOP in African American （AA）-PCa are currently unknown. We sequenced the SPOP gene to identify somatic mutations in 49 AA prostate tumors and identified three missense mutations （p.Y87C, p.F102S, and p.G111E） in five AA prostate tumors （10%） and one synonymous variant （p.11061） in one tumor. Intriguingly, all of mutations and variants clustered in exon six, and all of the mutations altered conserved amino acids. Moreover, two mutations （p.F102S and p.G111E） have only been identified in AA-PCa to date. Quantitative real-time polymerase chain reaction analysis showed a lower level of SPOP expression in tumors carrying SPOP mutations than their matched normal prostate tissues. In addition, SPOP mutations and novel variants were detected in 5 of 27 aggressive PCa and one of 22 less aggressive PCa （P 〈 0.05）. Further studies with increased sample size are needed to validate the clinicopathological significance of these SPOP mutations in AA-PCa.

Update on Management of Portal Vein Thrombosis and the Role of Novel Anticoagulants

The clinical management of portal vein thrombosis(PVT)remains ambiguous due to its heterogeneous presentations and its associations with liver disease,malignancy,and hypercoagulable *** natural history and clinical outcome of PVT are highly variable,dependent upon size,extent and degree of the thrombotic occlusion,as well as the physiological impact of patient *** existing clinical guidelines consistently recommend low molecular weight heparin or vitamin K antagonist anticoagulation in cirrhotic patients with symptomatic acute PVT,management of asymptomatic and chronic PVT may need to be determined on a case-by-case basis,factoring in the state of underlying liver *** general,patients with PVT and underlying malignancy should be anticoagulated to alleviate symptoms and prevent recurrences that could disrupt the cancer ***,existing clinical data does not support routine anticoagulation of cirrhotic patients with asymptomatic PVT in the absence of underlying *** low molecular weight heparin and vitamin K antagonist remain the most commonly used agents in PVT,an emerging body of clinical evidence now suggests that direct-acting oral anticoagulants may be used safely and effectively in *** such,direct-acting oral anticoagulants may offer a more convenient anticoagulation alternative for PVT management in future practice.

Prostate cancer （PC） is the second most diagnosed visceral malignancy in men worldwide, with over 900 000 new diagnoses each year.1 Approximately 50% of patients treated in industrialized nations will receive androgen deprivation therapy （ADT） at some point in their lifetimes.2 The use of ADT as a treatment approach is likely to increase as new drug developments have focused on intensifying the effect of reducing androgen receptor activation.

Serrated lesions:A challenging enemy

The serrated pathway accounts for 30%-35%of colorectal cancer(CRC).Unlike hyperplastic polyps,both sessile serrated lesions(SSLs)and traditional serrated adenomas are premalignant lesions,yet SSLs are considered to be the principal serrated precursor of *** lesions represent a challenge in detection,classification,and removal–contributing to post-colonoscopy ***,it is of the utmost importance to characterize these lesions properly to ensure complete removal.A retrospective cohort study developed a diagnostic scoring system for SSLs to facilitate their detection endoscopically and subsequent *** the study,it can be ascertained that both indistinct border and mucus cap are essential in both recognizing and diagnosing serrated *** proximal colon poses technical challenges for some endoscopists,which is why high-quality colonoscopy plays such an important *** indistinct border of some SSLs poses another challenge due to difficult complete ***,it is imperative that gastroenterologists use the key features of mucus cap,indistinct borders,and size of at least five millimeters along with a high-quality colonoscopy and a good bowel preparation to improve the SSL detection rate.

Introduction of proton pump inhibitor(PPI)therapy into clinical practice has revolutionized treatment approach to acid-related *** its clinical success came a widespread use of PPI ***,several studies found that PPIs were oftentimes overprescribed in primary care and emergency setting,likely attributed to seemingly low side-effect profile and physicians having low threshold to initiate ***,now there is a growing concern over PPI side-effect profile among both patients and *** would like to bring more awareness to the currently available guidelines on PPI use,discuss clinical indications for PPIs and the evidence behind the reported *** hope that increased awareness of proper PPI use will make the initiation or continuation of therapy a well informed and an evidence-based decision between patient and *** also hope that discussing evidence behind the reported side-effect profile will help clarify the growing concerns over PPI therapy.

Cannabis,commonly known as marijuana,is a drug extracted from the Cannabis plant known for its psychotropic and medicinal *** has been used for healing purposes during ancient times,although its psychoactive components led to its restricted use in ***,cannabis is found to have modulatory effects on the endocannabinoid system exhibiting its medicinal role in the gastrointestinal(GI)*** animal and human studies demonstrate the influential effects of cannabis on a variety of GI diseases including inflammatory bowel disease,motility disorders and GI *** also has a regulatory role in GI symptoms including nausea and vomiting,anorexia,weight gain,abdominal pain,among ***,both its acute and chronic use can lead to undesirable side effects such as dependency and addiction,cognitive impairment and cannabinoid hyperemesis *** will discuss the role of cannabis in the GI system as well as dosing strategies to help guide gastroenterologists to assess its efficacy and provide patient counseling before prescription of medical marijuana.

Practical considerations for colorectal cancer screening in older adults

Recent guidelines recommend that colorectal cancer(CRC)screening after age 75 be considered on an individualized basis,and discourage screening for people over 85 due to competing causes of *** the heterogeneity in the health of older individuals,and lack of data within current guidelines for personalized CRC screening approaches,there remains a need for a clearer framework to inform clinical decision-making.A revision of the current approach to CRC screening in older adults is even more compelling given the improvements in CRC treatment,post-treatment survival,and increasing life expectancy in the *** this review,we aim to examine the personalization of CRC screening cessation based on specific factors influencing life and health expectancy such as comorbidity,frailty,and cognitive *** will also review screening modalities and endoscopic technique for minimizing risk,the risks of screening unique to older adults,and CRC treatment outcomes in older patients,in order to provide important information to aid CRC screening decisions for this age *** review article offers a unique approach to this topic from both the gastroenterologist and geriatrician perspective by reviewing the use of specific clinical assessment tools,and addressing technical aspects of screening colonoscopy and periprocedural management to mitigate screening-related complications.

Preoperative physiological esophageal assessment for anti-reflux surgery:A guide for surgeons on high-resolution manometry and pH testing

Gastroesophageal reflux disease (GERD) is one of the most commonlyencountered digestive diseases in the world, with the prevalence continuing toincrease. Many patients are successfully treated with lifestyle modifications andproton pump inhibitor therapy, but a subset of patients require more aggressiveintervention for control of their symptoms. Surgical treatment with fundoplicationis a viable option for patients with GERD, as it attempts to improve the integrityof the lower esophageal sphincter (LES). While surgery can be as effective asmedical treatment, it can also be associated with side effects such as dysphagia,bloating, and abdominal pain. Therefore, a thorough pre-operative assessment iscrucial to select appropriate surgical candidates. Newer technologies arebecoming increasingly available to help clinicians identify patients with true LESdysfunction, such as pH-impedance studies and high-resolution manometry(HRM). Pre-operative evaluation should be aimed at confirming the diagnosis ofGERD, ruling out any major motility disorders, and selecting appropriate surgicalcandidates. HRM and pH testing are key tests to consider for patients with GERDlike symptoms, and the addition of provocative measures such as straight legraises and multiple rapid swallows to HRM protocol can assess the presence ofunderlying hiatal hernias and to test a patient’s peristaltic reserve prior tosurgery.
