Diffic.lt intubation, inadequate ventilation and esophageal intubation are the princ.pal c.uses of death or brain damage related to airway manipulation. The objec.ive of this c.oss-sec.ional study was to c.rrelate a preanesthetic.evaluation that may be c.pable of predic.ing a diffic.lt intubation with the c.nditions enc.untered at laryngosc.py and endotrac.eal intubation. Eighty-one patients submitted to general anesthesia were evaluated at a preanesthetic.c.nsultation ac.ording to the modified Mallampati c.assific.tion, the Wilson sc.re and the Americ.n Soc.ety of Anesthesiologists (ASA) diffic.lt airway algorithm. Findings were then c.rrelated with the c.rmac.-Lehane c.assific.tion and with the number of attempts at endotrac.eal intubation. No statistic.lly signific.nt c.rrelations were found between the patients’ Mallampati c.assific.tion and their c.rmac.-Lehane grade or between the Mallampati c.assific.tion and the number of attempts required to ac.ieve endotrac.eal intubation. Laryngosc.py proved diffic.lt in four patients and in all of these c.ses the Wilson sc.re had been indic.tive of a possibly diffic.lt airway, highlighting its good predic.ing sensitivity. However, the spec.fic.ty of this test was low, sinc. another 24 patients had the same Wilson sc.re but were c.assified as c.rmac.-Lehane I/II. Moreover, two patients who had a Wilson sc.re ≥ 4 were also c.assified as c.rmac.-Lehane grade I/II. The study c.nc.uded that the Wilson sc.re, although seldom used in c.inic.l prac.ic., is a highly sensitive predic.or of a diffic.lt airway;its spec.fic.ty, however, is low.

Owing to the far-reac.ing environmental c.nsequenc.s of agric.lture and food systems,suc. as their c.ntribution to c.imate c.ange,there is an urgent need to reduc. their *** and national governments set sustainability targets and implement c.rresponding ***,c.itic. of the globalized system c.aim that a territorial administrative sc.le is better suited to address sustainability ***,at the subnational level,loc.l authorities rarely apply a systemic.environmental assessment to enhanc. their ac.ion *** paper employs a territorial life c.c.e assessment methodology to improve loc.l environmental agri-food *** objec.ive is to identify signific.nt direc. and indirec. environmental hotspots,their origins,and formulate effec.ive mitigation *** methodology is applied to the administrative department of Finistere,a strategic.agric.ltural region in North-Western *** environmental c.iteria inc.uding c.imate c.ange,fossil resourc. sc.rc.ty,toxic.ty,and land use are *** findings reveal that the primary environmental hotspots of the studied loc.l food system arise from indirec. sourc.s,suc. as livestoc. feed or diesel *** reduc.ion and organic.farming c.nversion emerge as the most environmentally effic.ent strategies,resulting in a 25%dec.ease in the c.imate c.ange ***,the overall modeled impac. reduc.ion is insuffic.ent following national objec.ives and remains limited for the land use *** results highlight the innovative applic.tion of life c.c.e assessment led at a loc.l level,offering insights for the further advanc.ment of systematic.and prospec.ive loc.l agri-food ***,they provide guidanc. for loc.l authorities to enhanc. the sustainability of planning strategies.

Bac.ground: Duodenosc.pe-related multidrug-resistant organism(MDRO) infec.ions raise c.nc.rns. Disposable duodenosc.pes have been rec.ntly introduc.d in the market and approved by regulatory agenc.es with the aim to reduc. the risk of endosc.pic.retrograde c.olangiopanc.eatography(ERc.) assoc.ated infec.ions. The aim of this study was to evaluate the outc.me of proc.dures performed with single-use duodenosc.pes in patients with c.inic.l indic.tions to single-operator c.olangiopanc.eatosc.py. Methods: This is a multic.nter international, retrospec.ive study c.mbining all patients who underwent c.mplex biliopanc.eatic.interventions using the c.mbination of a single-use duodenosc.pe and a single-use c.olangiosc.pe. The primary outc.me was tec.nic.l suc.ess defined as ERc. c.mpletion for the intended c.inic.l indic.tion. Sec.ndary outc.mes were proc.dural duration, rate of c.oss-over to reusable duodenosc.pe, operator-reported satisfac.ion sc.re(1 to 10) on performanc. rating of the single-use duodenosc.pe, and adverse event(AE) rate. Results: A total of 66 patients(26, 39.4% female) were inc.uded in the study. ERc. was c.tegorized ac.ording to ASGE ERc. grading system as 47(71.2%) grade 3 and 19(28.8%) grade 4. The tec.nic.l suc.ess rate was 98.5%(65/66). Proc.dural duration was 64(interquartile range 15-189) min, c.oss-over rate to reusable duodenosc.pe was 1/66(1.5%). The satisfac.ion sc.re of the single-use duodenosc.pe c.assified by the operators was 8.6 ± 1.3 points. Four patients(6.1%) experienc.d AEs not direc.ly related to the single-use duodenosc.pe, namely 2 post-ERc. panc.eatitis(PEP), 1 c.olangitis and 1 ***: Single-use duodenosc.pe is effec.ive, reliable and safe even in tec.nic.lly c.allenging proc.dures with a non-inferiority to reusable duodenosc.pe, making these devic.s a viable alternative to standard reusable equipment.
