Optimization of the treatment with immunosuppressants and biologics in inflammatory bowel disease

Many placebo controlled trials and meta-analyses evaluated the efficacy of different drugs for the treatment of inflammatory bowel disease (IBD), including immunosuppressants and biologics. Their use is indicated in moderate to severe disease in non responders to corticosteroids and in steroid-dependent patients, as induction and maintainance treatment. Infliximab, as well as cyclosporine, is considered a second line therapy in the case of severe ulcerative colitis, or non-responders to intravenous corticosteroids. An adequate dosage and duration of therapy with thiopurines should be reached before evaluating their efficacy. Methotrexate is a valid option in patients with Crohn’s disease but its use is confined to patients who are intolerant or non-responders to thiopurines. Evidence for the use of methotrexate in ulcerative colitis is insufficient. The use of thalidomide and mycophenolate mofetil is not recommended in patients with inflammatory bowel disease, these treatments could be considered in case of failure of all other therapeutic options. In patients with moderately active ulcerative colitis, refractory to thiopurines, the use of tacrolimus is considered an alternative to biologics. An increase of the dose or a decrease in the interval of administration of biological treatment could be useful in the presence of an incomplete clinical response. In the case of primary failure of an anti-tumor necrosis factor alpha a switch to another one should be considered. Data on the efficacy of combination therapy are up to now insufficient to consider this strategy in all IBD patients. The final outcome of the treatment should be considered the clinical remission, with mucosa healing, and not the clinical response. The evaluation of serum concentration of thiopurine methyl transferase activity, thiopurine metabolites, biologic serum levels and antibiologic antibodies could be useful for the management of the treatment but it has not been routinely applied in cl

Could JC virus provoke metastasis in colon cancer?

AIM: To evaluate the prevalence of John Cunningham virus(JC virus) in a small cohort of patients with colon cancer and to assess its presence in hepatic ***: Nineteen consecutive patients with histologically diagnosed colon cancer were included in our study, together with ten subjects affected by histologically and serologically diagnosed hepatitis C virus infection. In the patients included in the colon cancer group, JC virus was searched for in the surgical specimen; in the control group, JC virus was searched for in the hepatic biopsy. The difference in the prevalence of JC virus in the hepatic biopsy between the two groups was assessed through the χ2 ***: Four out of 19 patients with colon cancer had a positive polymerase chain reaction(PCR) test for JC virus, and four had liver metastasis. Among the patients with liver metastasis, three out of four had a positive PCR test for JC virus in the surgical specimen and in the liver biopsy; the only patient with liver metastasis with a negative test for JC virus also presented a negative test for JC virus in the surgical specimen. In the control group of patients with hepatitis C infection, none of the ten patients presented JC virus infection in the hepatic biopsy. The difference between the two groups regarding JC virus infection was statistically significant(χ2 = 9.55, P = 0.002).CONCLUSION: JC virus may play a broader role than previously thought, and may be mechanistically involved in the late stages of these tumors.

Mou Zongsan and Martin Heidegger： Reopening a Debate on Ontology and Ethics

In this paper I investigate differences and similarities in the definitions of human being and human essence as proposed by two of the most influential thinkers of 20th-century world philosophy, namely Mou Zongsan and Martin Heidegger. I first examine a number of interpretations put forth by scholars that assess the philosophical compatibility of the thought of the two philosophers. Each of these assessments tend to agree that they are incompatible with each other based on what they perceive as an absolute distance between Mou＇s and Heidegger＇s ways of thinking. Although these studies are pioneering and show an in-depth understanding of Mou＇s thought, none demonstrate a correct understanding of Heidegger＇s philosophy. Therefore, I will attempt to demonstrate that, despite their differences, the ontological and ethical theses of Mou and Heidegger have several striking points of contact. I will also put forth the claim that Heidegger＇s post-turn philosophy is more compatible with Mou＇s philosophy than Kant＇s system.

Assessment of positioning performances in Italy from GPS, BDS and GLONASS constellations

The use of multiple GNSS constellations has been beneficiary to positioning performances and reliability in recent times, especially in low cost mass-market setups. Along with GPS and GLONASS, GALILEO and BDS are the other two constellations aiming for global coverage. With ample research demonstrating the benefits of GALILEO in the European region, there has been a lack of study to demonstrate the performance of BDS in Europe, especially with mass-market GNSS receivers. This study makes a comparison of the performances between the combined GPS-GLONASS and GPS-BDS constellations in Europe with such receivers. Static open sky and kinematic urban environment tests are performed with two GNSS receivers as master and rover at short baselines and the RTK and double differenced post processed solutions are analyzed. The pros and cons of both the constellation choices is demonstrated in terms of fixed solution accuracies, percentage of false fixes, time to first fix for RTK and float solution accuracies for post processed measurements. Centimeter level accuracy is achieved in both constellations for static positioning with GPS-BDS combination having a slightly better performance in comparable conditions and smaller intervals. GPS-GLONASS performed slightly better for longer intervals due to the current inconsistent availability of BDS satellites. Even if the static tests have shown a better performance of GPS-BDS combination, the kinematic results show that there are no significant differences between the two tested configurations.

Assessment of autonomic function in untreated adult coeliac disease

AIM: Some recent studies showed that alteration of upper-gut motility in coeliac disease may be related to dysfunction of autonomic nervous system. The aim of our study was to investigate whether autonomic nervous system was altered in untreated and unselected coeliac disease ***: We studied 8 untreated and consecutive coeliac disease patients (2 males and 6 females, age range 37±14.5 years). Histological evaluation of duodenal mucosa, anti-gliadin antibodies (AGA), antiendomysial antibodies (EMA) and anti-tTG antibodies and sorbitol H2 breath test were performed in all patients. Extrinsic autonomic neuropathy was assessed by the standardized measurement of cardiovascular reflexes (lying-to-standing,Valsalva manoeuvre, deep breathing, sustained handgrip).The results obtained were compared with a healthy,asymptomatic control group (6 males and 7females, age range 42.3±13.5 years).RESULTS: Coeliac patients exhibited a lower increase of PAS as a response to isometric effort, a reduction of spectral power LF as a response to clinostatic position,but without statistical significance. Also they showed a lower tolerance to orthostatic position, associated with a latent disequilibrium of sympathetic-vagal balance, a relative prevalence of parasympathetic component of the autonomic function. However, these results were not statistically significant when compared with control group(P = n.s.). And they were unchanged after 6 and 12 mo of gluten-free ***: This study failed to confirm a significant correlation between autonomic dysfunction and coeliac disease, yet we could not exclude a role of autonomic dysfunction in the genesis of systemic symptoms in some coeliacs.

Adrenal metastasis from endometrial cancer: A case report

BACKGROUND Metastases to adrenal glands originate principally from lung, breast, or gastrointestinal cancers, followed by malignant melanoma and thyroid neoplasms. We present an unusual case of uterine cancer metastasizing to the adrenal glands with a review of the English literature on the management of this rare disease. CASE SUMMARY A 53-year-old Caucasian woman with a history of endometrial cancer (grade 2;International Federation of Gynecology and Obstetrics III A) was hospitalized in November 2017 for a left adrenal mass found on a follow-up computed tomography scan 3 years after her gynecological surgery. Laboratory test results were normal. A laparoscopic left adrenalectomy was performed. The postoperative course was uneventful, and no chemotherapy was administered. The pathological report confirmed an adrenal endometrioid metastasis. At 36 mo of follow-up, the patient is alive and well, with no evidence of recurrent disease. A literature review identified only 11 previously-published cases of adrenal metastases from uterine cancer. CONCLUSION Adrenal metastasis from uterine cancer is very rare. Laparoscopic adrenalectomy may be an effective treatment in selected cases of localized adrenal metastasis.
