BACKGROUND The implementation of a colorectal cancer(CRC)screening programme may increase the awareness of Primary Care Physicians,reduce the diagnostic delay in CRC detected outside the scope of the screening programme and thus improve *** To determine the effect of implementation of a CRC screening programme on diagnostic delays and prognosis of CRC detected outside the scope of a screening *** We performed a retrospective intervention study with a pre-post *** identified 322 patients with incident and confirmed CRC in the pre-implantation cohort(June 2014–May 2015)and 285 in the post-implantation cohort(June 2017-May 2018)in the Cancer Registry detected outside the scope of a CRC screening *** each patient we calculated the different healthcare diagnostics delays:global,primary and secondary healthcare,referral and colonoscopyrelated *** addition,we collected the initial healthcare that evaluated the patient,the home location(urban/rural),and the CRC stage at *** determined the two-year survival and we performed a multivariate proportional hazard regression analysis to determine the variables associated with *** We did not detect any differences in the patient or CRC baseline-related variables.A total of 20.1%of patients was detected with metastatic *** was a significant increase in direct referral to colonoscopy from primary healthcare(25.5%,35.8%;P=0.04)in the post-implantation *** delay was reduced by 24 d(106.64±148.84 days,82.84±109.31 d;P=0.02)due to the reduction in secondary healthcare delay(46.01±111.65 d;29.20±60.83 d;P=0.02).However,we did not find any differences in CRC stage at diagnosis or in two-year survival(70.3%;P=0.9).Variables independently associated with twoyear risk of death were age(Hazard Ratio-HR:1.06,95%CI:1.04-1.07),CRC stage(II HR:2.17,95%CI:1.07-4.40;III HR:3.07,95%CI:1.56-6.08;IV HR:19.22,95%CI:9.86-37.44;unknown HR:9.24,95%CI:4.27-19.99),init

Plants experience different abiotic/biotic stresses,which trigger their molecular machinery to cope with them. Besides general mechanisms prompted by many stresses, specific mechanisms have been introduced to optimize the response to individual threats. However, these key mechanisms are difficult to identify. Here, we introduce an in-depth species-specific transcriptomic analysis and conduct an extensive meta-analysis of the responses to related species to gain more knowledge about plant responses. The spider mite Tetranychus urticae was used as the individual species, several arthropod herbivores as the related species for meta-analysis, and Arabidopsis thaliana plants as the common host. The analysis of the transcriptomic data showed typical common responses to herbivory, such as jasmonate signaling or glucosinolate biosynthesis. Also, a specific set of genes likely involved in the particularities of the Arabidopsis-spider mite interaction was discovered. The new findings have determined a prominent role in this interaction of the jasmonateinduced pathways leading to the biosynthesis of anthocyanins and tocopherols. Therefore, tandem individual/general transcriptomic profiling has been revealed as an effective method to identify novel relevant processes and specificities in the plant response to environmental stresses.

The operational phase of a building project has increasingly gained importance with their energy performance becoming valuable and determining their operational excellence. In most heritage building projects （HBPs）, the operational energy use aspects are less considered, and a systematic way of analyzing their energy performance following project delivery is often lacking, The aim of this study is to evaluate the operational performance of refurbishment and reuse of UK listed church projects. The objective is to assess the operational energy use with a view to optimizing their sustafnabie performance. The methodology includes eight selected case study buildings refurbished and converted for multipurpose use. The case study approach provided qualitative insights into how the study contributes to a more structured requirements for energy management in HBPs with specific attention to energy-efficient building operations. The findings show the need to focus on fundamental areas of operational management （i.e. by developing and implementing more focused policy on operational energy performance of heritage buildings） to minimize the energy required to operate them. The challenges of implementing changes in operational energy performance improvement of heritage buildings are addressed in the form of recommendations that could Lead to real results. The study concludes that leveraging these areas requires commitment from all heritage building stakeholders because they all have substantial roles in harmonizing the requirement for the project＇s sustainability and not just the building operators. Meanwhile, baseline project planning, periodic updating, monitoring, and managing the energy use pattern are suggested as measures that could greatly facilitate better energy performance to optimizing their sustainable reuse compared with the traditional approach of trying to improve their thermal performance.

Non-alcoholic fatty liver disease(NAFLD)is among the most frequent etiologies of cirrhosis worldwide,and it is associated with features of metabolic syndrome;the key factor influencing its prognosis is the progression of liver *** review aimed to propose a practical and stepwise approach to the evaluation and management of liver fibrosis in patients with NAFLD,analyzing the currently available *** the assessment of NAFLD patients,it is important to identify clinical,genetic,and environmental determinants of fibrosis development and its *** properly detect fibrosis,it is important to take into account the available methods and their supporting scientific evidence to guide the approach and the sequential selection of the best available biochemical scores,followed by a complementary imaging study(transient elastography,magnetic resonance elastography or acoustic radiation force impulse)and finally a liver biopsy,when *** help with the selection of the most appropriate method a Fagan′s nomogram analysis is provided in this review,describing the diagnostic yield of each method and their post-test probability of detecting liver ***,treatment should always include diet and exercise,as well as controlling the components of the metabolic syndrome,+/-vitamin E,considering the presence of sleep apnea,and when available,allocate those patients with advanced fibrosis or high risk of progression into clinical *** final end of this approach should be to establish an opportune diagnosis and treatment of liver fibrosis in patients with NAFLD,aiming to decrease/stop its progression and improve their prognosis.

形成持久种子库的能力是预测物种在新区域成功建群的最佳指标之一。野生向日葵(Helianthus annuus L.)原产于北美,干扰促进了其种子库的形成,并对本地种群的建立和延续起着关键作用。然而,种子库在入侵种群的建立和延续中所起的作用尚不清楚。在本研究中,我们评估了种子库和干扰对几种向日葵生物类型(采集于阿根廷荒地生境中的野生向日葵、野生农田野生向日葵与作物向日葵自然杂交种,以及商业品种的后代)的建立和适应性,以及土壤中种子持久性的作用。在种子库试验中,我们评估了上述材料两年在干扰和未干扰条件下的出苗率、成活率到繁殖率、出苗成活率、每株花序数和每块地的花序数;在种子埋藏试验中,我们评估了其种子在四个春季(6个月、18个月、30个月和42个月)土壤中的持久性。研究结果表明,总体而言,幼苗在生长期(冬季)出苗较早,且受到干扰促进,尤其是第一年。尽管如此,在两种情况下,每块样地的花序数是相近的,尤其是荒地生境中。第二年,种子库出苗率较低,但成活率较高。在种子掩埋实验中,观察到遗传差异,荒地和野生生境中种子持久性达到42个月,而商业品种后代的种子持久性不超过6个月。在这两项实验中,农田野生型和商业向日葵品种后代生物型表现出一种中间行为。结果表明,野生向日葵和作物野生向日葵杂交种均能在其生长范围之外形成持久的种子库,且干扰可能有助于其在新区域的建立。
