Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow up.Results:There was no significant difference in graft healing between the two groups.Postoperative COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,respectively.The difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD group.Conclusion:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

Objective: To investigate the protective effects of aqueous seed extracts of fennel"Foeniculum vulgare"(FVE) and caraway "Carum carvi"(CCE) on liver, kidney and reproductive organs in female rats against cadmium chloride(CC) intoxication.Methods: A total of 36 adult female rats were divided into six groups, six in each group.Control group(fed normal diet), CC-treated group(50 mg CC/kg diet), CCE-treated group(150 mg CCE/kg diet), CCE + CC group, FVE(150 mg/kg diet) and FVE + CC. One month later, all rats were sacrificed and all samples were collected at proestrus phase.Results: The toxic effects of CC were confirmed biochemically by significant increase of serum concentration of liver enzymes(P < 0.05), and creatinine(P < 0.001). Moreover,CC increased significantly the serum level of malondialdehyde(MDA) and decreased the total antioxidant capacity(TAC)(P < 0.001). In addition, serum concentrations of estrogen, progesterone, follicle-stimulating hormone(FSH) and luteinizing hormone(LH)were significantly decreased(P < 0.01). Histopathologically, CC-treated group revealed marked pathological changes in renal, hepatic, ovarian and uterine tissues. All toxic findings observed in liver and kidney with CC treatment were found to be ameliorated markedly after co-treatments with FVE or CCE. Furthermore, co-treatment of FVE with CC improved significantly all studied reproductive parameters(P < 0.01).Conclusions: Both FVE and CCE could be used as efficient treatments for liver and kidney against CC intoxication. Moreover, FVE could be utilized as a potent treatment to protect and improve female fertility from cadmium intoxication.

Objective: To determine effects of variations of heart girth, volume trait on embryo quality grade, and to explore the relationship between heart girth and circulating progesterone and correlations of circulating progesterone with embryo quality grade in superstimulated donor Holstein cows. Methods: Nineteen cows were subjected to a standard superstimulation protocol using follicle stimulating hormone. Blood samples were collected before superovulation, at insemination and at collection of embryos for progesterone analysis. Embryo quality grades were compared between high and low heart girth donors. Moreover, Pearson's correlations were determined between heart girth, progesterone and embryo quality. Results: Variation of heart girth was not associated with significant differences in embryo quality grade between high and low heart girth donor cows (P>0.05). However, we observed a significantly higher percentage of the third-grade embryo (24.09%) in low heart girth donors compared to 9.64% in high heart girth donors. Moreover, the percentage of the transferable embryo was numerically higher and that of the degenerated embryo was numerically lower in low heart girth cows. Donor cows with low response to superovulation (total structures ≤ 3) expressed numerically higher mean heart girth, compared to donors with high (total structures>15) and medium (total structures = 4-15) superovulation. Heart girth and body weight of donor cows were moderately correlated (r=0.45, P<0.05), but none was correlated with circulating progesterone at different sampling times except for a moderate correlation between body weight and progesterone at embryo collection (r=0.54, P=0.02). Circulating progesterone before superovulation was moderately correlated to the second-grade embryo (r=0.46, P<0.05) and to the third grade (r=0.52, P<0.05) embryo. Objectives: To evaluate antifouling property of extracts from Red Sea soft corals against primary biofilm and biofouling.Methods: Seven species of soft corals Sarcophyton glaucum(a), Sinularia compressa,Sinularia cruciata(a), Heteroxenia fuscescens(a), Sarcophyton glaucum(b), Heteroxenia fuscescens(b) and Sinularia cruciata(b) were chosen to test their extracts as antibacterial and antifouling agents in Eastern Harbour of Alexandria, Mediterranean Sea.Bioactive compounds of soft corals were extracted by using methanol and concentrated under vacuum. The residues of extracts were mixed in formulation of inert paint which consisted of rosin, chlorinated rubber and ferrous oxide against micro and macro fouling organisms. The formulated paints were then applied on PVC panels twice by brush,hanged in a steel frame and immersed in Eastern Harbour of Alexandria Mediterranean Sea followed by visual inspection and photographic recordings.Results: After 185 days of immersion in seawater, the antifouling results agreed with the antibacterial results where extracts of Sinularia compressa and Heteroxenia fuscescens(b) gave the best activity against marine fouling tubeworms and barnacles. The inhibition activity was correlated with the major functional groups(hydroxyl, amino, carbonyl,aliphatic(fatty acids), C]C of alkene or aromatic rings and CeC l of aryl halides) of the extracts.Conclusions: The strong antifouling activity makes them promising candidates for new antifouling additives. After the screening and application of natural organic compounds from soft corals, marine organisms show activity against micro and macro fouling organisms.

This paper deals with the effects of electromagnetic forces on the orbital motion of a spacecraft. The electrostatic charge which a spacecraft generates on its surface in the Earth＇s magnetic field will be subject to a perturbative Lorentz force. A model incorporating all Lorentz forces as a function of orbital elements has been developed on the basis of magnetic and electric fields. This Lorentz force can be used to modify or perturb the spacecraft＇s orbits. Lagrange＇s planetary equations in the Gauss variational form are derived using the Lorentz force as a perturbation to a Keplerian orbit. Our approach incorporates orbital inclination and the true anomaly. The numer- ical results of Lagrange＇s planetary equations for some operational satellites show that the perturbation in the orbital elements of the spacecraft is a second order perturba- tion for a certain value of charge. The effect of the Lorentz force due to its magnetic component is three times that of the Lorentz force due to its electric component. In addition, the numerical results confirm that the strong effects are due to the Lorentz force in a polar orbit, which is consistent with realistic physical phenomena that occur in polar orbits. The results confirm that the magnitude of the Lorentz force depends on the amount of charge. This means that we can use artificial charging to create a force to control the attitude and orbital motion of a spacecraft.

Some pelvic pain syndromes are very resistant to medical treatment. Several studies have demonstrated that sacral neuromodulation, which has been successfully used for the treatment of bladder dysfunction, incontinence, urinary retention and urinary frequency [1]-[3], can be successfully used for the treatment of chronic pelvic pain [4]-[7]. Several studies have also demonstrated significant involvement of dorsal column pathways in the transmission of visceral pelvic pain [8] and the successful use of spinal cord stimulation for the treatment of chronic pelvic pain [9]. We report three cases of severe chronic pelvic pain that failed conservative treatment modalities. Placement of a combined sacral nerve roots stimulator and a low thoracic spinal cord stimulator resulted in a significant pain relief and improvement in daily life activities. We believe that this combination may help patients suffering from chronic pelvic pain resistant to medical management.

In this article we discuss the effect of posture on impedance during the placement of a spinal cord stimulator. Although, according to several studies, there appears not to be a specific and consistent change in impedance with change in posture, we noticed on multiple occasions that if the impedance remains very high during spinal cord stimulator (SCS) lead placement, asking the patient to sit up from the prone position may drop the impedance down. This could be explained by having air around the lead, from using loss of resistance to air technique in finding the epidural space, which moves up when sitting the patient up, or having the lead immersed in epidural fat. Sitting the patient up from the prone position should be tried if the impedance remains very high before aborting the procedure and taking the lead out.

A 40-years-old female patient with severe right facial pain with a throbbing component along the mandibular division of the trigeminal nerve resistant to medical management. Continuous inferior alveolar nerve block with local anesthetics using an indwelling catheter provided a complete pain resolution for the patient for 2 weeks, after which the catheter got infected and was removed. A trial of balloon occlusion of the right internal maxillary artery provided complete resolution of the throbbing component of the patient’s pain. This was followed by permanent embolization with multiple coils.

Temperature Effects on the Electrical Performance of Large Area Multicrystalline Silicon Solar Cells Using the Current Shunt Measuring Technique

The temperature effects on the electrical performance of a large area multicrystalline silicon solar cell with back-contact technology have been studied in a desert area under ambient conditions using the current shunt measuring technique. Therefore, most of the problems encountered with traditional measuring techniques are avoided. The temperature dependency of the current shunt from 5oC up to 50oC has been investigated. Its temperature coefficient proves to be negligible which means that the temperature dependency of the solar cell is completely independent of the current shunt. The solar module installed in a tilted position at the optimum angle of the location, has been tested in two different seasons (winter and summer). The obtained solar cell short circuit current, open circuit voltage and output power are correlated with the measured incident radiation in both seasons and all results are discussed.
