The study modifies an adhesive system with hydroxyapatite nanoparticles as load, characterizing it and evaluate the effectiveness of its bond to dental structure. The middle thirds of healthy premolar tooth crowns were obtained, and each crown was sectioned vertically, resulting in two sections. The sections were divided into ten groups (n = 15), in which resin composite restorations were simulated: (G1E and G1D) conventional adhesive system (SAC);(G2E and G2D) SAC modified with HAP;(G3E and G3D) Primer modified with HAP;(G4E and G4D) monocomponent adhesive system;(G5E and G5D) self-etching adhesive system. The specimens were submitted to the microshear test and characterization technique. There was statistically significant difference (Kruskal-Wallis) between the groups (p 0.01). G3 presented the highest bond strength to enamel (64.40 MPa, ±7.36) and dentin (39.59 MPa, ±21.46). The majority of specimens were found adhesive fractures. Bond strength to enamel and dentin of the primer modified with HAP of SAC showed higher values.

BACKGROUND： Fibromyalgia （FMS） is a syndrome characterized by chronic widespread musculoskeletal pain, whose etiology is not completely understood. Different therapeutic approaches have been used with inconsistent results. This observation does not invalidate the continued search for alternative treatments aimed at improving quality of life （QoL） in FMS. OBJECTIVE： This study compared three classica acupuncture （AC）, electroacupuncture （EAC） and promotion of QoL in FMS patients. traditional Chinese medicine （TCM） therapies： moxibustion （MX） in the management of pain and DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： A preliminary, group-assigned, comparative study enrolled 30 women, mean age （46.90_＋9.24） years （range 20-60 years）, who met the 1990 American College of Rheumatology criteria for FMS diagnosis and a pain-pressure threshold （PPT）〈 4 kg/cm2. The study was conducted in a teaching tertiary-care medical institution from May 2010 through April 2012. AC, EAC and MX were delivered for 30 min, once a week, for 8 weeks, bilaterally at Neiguan （PC6）, Hegu （LI4）, Yanglingquan （GB34）, Sanyinjiao （SP6） and Taichong （LR3） acupoints. MAIN OUTCOME MEASURES： Each week, immediately before treatment and after treatment, subjects were tested for PPTs, Wong-Baker Faces Pain Scale （WBFPS; for pain intensity） and Medical Outcomes Study 36-item Short Form Health Survey （SF-36： for QoL）. RESULTS： There was no significant improvement in pain or reduction of tender points in any of the groups studied, at the end of the 8th session. Significant improvement of QoL was perceived in vitality （after AC treatment） and in mental health （after EAC and MX treatments）. CONCLUSION： TCM therapies （AC, EAC and MX） promoted an improvement in the QoL in two areas （vitality and mental health） in FMS women. Further large-scale clinical trials are required to confirm this effect.

AIM: To investigate the microleakage of two different root canal obturation systems, using the nuclear medicine ***: Twenty-six single-rooted extracted teeth were selected. The crowns were sectioned to obtain 15-mm long root segments and each tooth was prepared using rotary Pro File instruments. The roots were divided into 2 experimental groups using Real Seal 1 and Real Seal sealer or Thermafil and Top Seal sealer as well as two control groups. On the 7th and the 28 th day the apices were submersed in a solution of 99 m TcPertechnetate during 3 h. The radioactivity was counted using a γ camera. RESULTS: The present study showed that none of the root canal-filled teeth was leakage free. The statistical analyses were made using Kruskal-Wallis and statistical significance was assessed using α = 0.05. Although apical leakage measured in counts per minute(cpm) in the Thermafil/Top Seal group was lower than in the Real Seal/Real Seal group(363 916 ± 180 707.7 cpm vs533 427 ± 414 020.6 cpm) on 7th day and(1 678 200 ± 567 217.4 cpm vs 2 240 518 ± 383 356.7 cpm) on 28 th day, there was no statistical difference(P > 0.05). In the Thermafil/Top Seal group and Real Seal 1/Real Seal group it was found that over time, the number of counts increased between 7 d and 28 d(363 916 ± 180 707.7 cpm vs 1 678 200 ± 567 217.4 cpm) and(533 427 ± 414 020.6 cpm vs 2 240 518 ± 383 356.7 cpm), respectively, with statistically significant differences(Thermafil/Top Seal group, P = 0.015 and Real Seal 1/Real Seal group, P = 0.036).CONCLUSION:Both carrier-based Realseal 1 and Thermafil techniques showed a similar sealing effect,but none of the materials was leakage free.

This study investigated the microleakage of two different root canal obturation systems, using the nuclear medicine approach, with sodium pertechnetate 99mTc. Twenty six single-rooted extracted teeth were selected. The crowns were sectioned to obtain15 mmlong root segments and each tooth was prepared using rotary ProFile? instruments. The roots were divided into 2 experimental groups and two control groups. Twenty root canals were filled, using Thermafil? and Topseal? or MTA Fillapex? as a sealer. On the 7th and the 28th day the apices were submersed in a solution of 99mTc-Pertechnetate during 3 hours. The radioactivity was counted using a gamma camera. Although apical leakage on the 7th day in the Topseal group was reduced compared with RealSeal1, with a statistical significant difference (p = 0.057), on the 28th day, the MTA Fillapex increased the sealing properties (p = 0.017).

High levels of distress and disturbance amongst those experiencing acute mental illness can be a major problem for mental health nurses. The feelings experienced by these nurses when caring for and supporting disturbed and/or distressed patients along with their concurrent thoughts are not well described in the literature. To date, this complex issue has not been explored within a comparative European context. The objective of this qualitative study was to explore the feelings and thoughts of mental health nurses when supporting and caring for distressed and/or disturbed patients in 6 European countries. Methods: Focus groups were used to collect data from 130 mental health nurses working in acute inpatient psychiatric settings. Results: Data were analysed using content analysis. Findings highlighted 6 broad themes: 1) Mixed emotions: expressive and responsive, 2) Procedure for caring for and supporting disturbed and/or distressed patients, 3) Use of guidelines for caring and supporting disturbed and/or distressed patients, 4) Team and organisational support, 5) Ethical concerns: Cognitive dissonance and 6) Education and training. Commonalities and differences were?found across all themes. Approaches to care, nurses’ role and education, clinical guidelines and/or standards vary from country to country, therefore the care, treatment and management of distressed and/or disturbed patients are various. As a result, mental health nurses have different experiences, various emotional quandaries concurrent with cognitive dissonance and different coping strategies when caring for and supporting distressed and disturbed patients. Conclusions: More emphasis needs to be given to the emotional quandaries and concurrent cognitive dissonance experienced by mental health nurses caring for distressed and/or disturbed inpatients in acute psychiatric settings. Increased access to education and training with particular attention to interpersonal communication and relationship building

Several authors have shown the beneficial role to incorporate silicon into hydroxyapatite lattice, although, the mechanism behind the enhanced bioactivity of this Si-hydroxyapatite (Si-HA) is poorly understood. The incorporation of Si into the HA lattice alters the surface charge of HA, leading to more negative values. Due to the importance of the surface properties on the interaction between biomaterials, physiological fluids, and the host tissue, it is important to further characterize the surface of Si-HA by determining its surface energy and wettability. Our results showed that the incorporation of Si increased the hydrophilicity of HA, leading to a higher interfacial tension. Another important property for osteointegration is the formation of an apatite layer. The dissolution of Si-HA in the presence of serum-free simulated body fluid (SBF) started at early time points and using atomic force microscopy (AFM) it was possible to observe the dissolution at the grain boundaries and grains, therefore an apatite layer was formed in a short period of time. As the dissolution-precipitation process is much more complex in vivo, we tried to mimic the initial stages of the in vivo reaction by incubating the Si-HA in serum-SBF. It was shown that the dissolution kinetics in serum-SBF was slower when compared to the dissolution in serum free-SBF. At the same time point, no significant dissolution features were observed or apatite layer was visualized. The phase imaging AFM indicated the presence of a layer on top of these materials that could be a proteinaceous layer, as XPS analysis detected an increase on the concentration of nitrogen on the surface of the samples incubated in the presence of proteins.

Introduction: Multidrug resistance (MDR) is one of the major problems of chemotherapy. Overexpression of efflux pumps, such as P-glycoprotein (Pgp), multiple resistance-related protein 1 (MRP-1) and lung resistance protein (LRP) can lead to MDR. Verapamil and L-buthionine-sulfoximine (BSO) are two modulators of these proteins. This study aims to compare 99mTc-Sestamibi transport kinetics in human colorectal adenocarcinoma cell lines, in the presence and absence of the MDR modulators verapamil and BSO. Material and Methods: MDR proteins expression was evaluated in sensitive (WiDr) and resistant (LS1034) human colorectal adenocarcinoma cell lines. Intracellular and plasma membrane Pgp and MRP1, and LRP expression was analyzed by flow-cytometry and western blot. Cellular transport kinetics was assessed using 99mTc-Sestamibi. MDR modulation was evaluated though retention studies in resistant cells after incubation with the modulators. Results: Pgp expression was significantly higher (p≤0.001) in resistant cells. These results were confirmed by western blot analysis. 99mTc-Sestamibi uptake and retention percentage were significantly higher (p99mTc-Sestamibi, there were differences among the MDR modulators used (p99mTc-Sestamibi could indicate MDR phenotype in colorectal adenocarcinoma cells. As the modulators used showed a reversion of the retention profile only for the first minutes, their administration should occur immediately before the administration of cytotoxic drugs.

Anticancer drug nephrotoxicity is an important and increasing adverse drug event that limits the efficacy of cancer *** kidney is an important elimination pathway for many antineoplastic drugs and their metabolites,which occurs by glomerular filtration and tubular *** agents,both conventional cytotoxic agents and molecularly targeted agents,can affect any segment of the nephron including its microvasculature,leading to many clinical manifestations such as proteinuria,hypertension,electrolyte disturbances,glomerulopathy,acute and chronic interstitial nephritis,acute kidney injury and at times chronic kidney *** clinician should be alert to recognize several factors that may maximize renal dysfunction and contribute to the increased incidence of nephrotoxicity associated with these drugs,such as intravascular volume depletion,the associated use of nonchemotherapeutic nephrotoxic drugs(analgesics,antibiotics,proton pump inhibitors,and bonetargeted therapies),radiographic ionic contrast media or radiation therapy,urinary tract obstruction,and intrinsic renal *** of patients at higher risk for nephrotoxicity may allow the prevention or at least reduction in the development and severity of this adverse ***,the aim of this brief review is to provide currently available evidences on oncologic drug-related nephrotoxicity.

Objective In this study,we assess the delivery of congenital pediatric surgical care under Brazil’s system of universal health coverage and evaluate differences in delivery between public and private *** A cross-sectional national survey of pediatric surgeons in Brazil was *** were asked which of 23 interventions identified through the Disease Control Priorities 3(Surgical Interventions for Congenital Anomalies)they perform and to report barriers faced while providing surgical *** were weighted by state and stratified by sector(public vs private).Results A sample of 352 responses was obtained and weighted to represent 1378 practicing pediatric surgeons registered in Brazil during the survey time.73%spend the majority of their time working in the public sector(‘SistemaÚnico de Saúde’and Foundation hospitals),and most of them also work in the private ***,Brazilian pediatric surgeons have the expertise to provide thoracic,abdominal,and urologic *** working mostly in the public sector were more likely to report a lack of access to essential medications(25%vs 9%,p<0.01)and a lack of access to hospital beds for surgical patients(52%vs 32%,p<0.01).Conclusions Brazilian pediatric surgeons routinely perform thoracic,abdominal,and urologic *** working in government-financed hospitals face barriers related to infrastructure,which may impact Brazilians who rely on Brazil’s universal health coverage *** that support pediatric surgeons working in the public sector may promote the workforce available to provide congenital pediatric surgical care.
