目的探讨胆红素和C反应蛋白(C-r.active p.otein,Cr.0}预测急性心肌梗死(acute myocar.ial infar.tion,AMI)患者支架术后长期预后的意义。方法选择2.16年9月至2.18年3月中山大学附属江门医院行经皮冠状动脉介入治疗的12.例AMI患者为AMI组,12.例健康体检者为对照组,测定两组患者血清中总胆红素(total bilir.bin,TBIL)、直接胆红素(dir.ct bilir.bin,DBIL)、间接胆红素(indir.ct bilir.bin,IBIL)、Cr.0}浓度。6个月内对AMI组患者进行心血管事件评定,发生心血管事件为事件组,其余患者为非事件组,对6个月内发生心血管事件患者的Cr.0}TBIL浓度进行直线回归分析。结果与对照组比较,AMI组患者TBIL、DBIL、IBIL浓度更低,Cr.0}度更高,差异有统计学意义(p.0.05)。与非事件组比较,事件组患者TBIL浓度较低,Cr.0}度更高,差异有统计学意义(p.0.05)。对6个月内发生主要不良心血管事件患者的Cr.0}TBIL浓度进行直线回归分析,其r.Squar.(确定系数)为0.851(p.0.01)。结论 AMI患者TBIL、DBIL、IBIL浓度较健康体检者低,而Cr.0}度更高;TBIL浓度越低,Cr.0}度越高,AMI患者支架术后长期(6个月)预后越差。联合检测TBIL、Cr.0}度可预测AMI患者支架术后长期主要不良心血管事件的发生率,能更准确预测其长期预后。

目的:探索精索静脉曲张(VC)大鼠模型中氧化应激介导附睾上皮紧密连接蛋白ZO-1损伤及对附睾功能影响。方法:将45只体重2.0～2.5 g的SD大鼠随机分为假手术组(单纯暴露并分离左肾静脉)、实验组(部分缩窄左肾静脉并充分结扎精索静脉侧支)和治疗组[造模术后给予150 mg/(kg·d)维生素E灌胃60 d],每组各15只。模型构建后60d分别观察左侧附睾组织学改变,采用qp.r.免疫组化染色、免疫荧光染色及Wester.印迹等检测左侧附睾上皮连接蛋白ZO-1及连接相关蛋白表达水平,同时检测附睾组织超氧化物歧化酶(SOD)、总抗氧化能力(T-AOC)活性,丙二醛(MDA)、α葡糖苷酶含量以及精子活力指标。结果:与假手术组相比,实验组左侧附睾上皮结构紊乱,上皮细胞数量减少,可见部分上皮细胞脱落至附睾管腔。qp.r.测claudin11、β-catenin、JAM-A、cadher.n12.ZO-1等多个连接蛋白表达,结果显示除实验组较假手术组ZO-1表达明显下调外(p.0.05),其余蛋白均无显著差异。免疫组化染色、免疫荧光染色Wester.印迹结果显示,与假手术组相比,实验组ZO-1表达明显降低(p.0.05);经维生素E治疗后,治疗组ZO-1表达较实验组明显升高(p.0.05)。实验组较假手术组大鼠附睾组织中MDA明显升高[(1.2.±0.18)nmol/mg p.otein vs(0.41±0.05)nmol/mg p.otein,p.0.05)],SOD[(2.8.62.67.84)U/mg p.otein vs(814.65±73.64)U/mg p.otein,p.0.05)]、T-AOC[(0.2.±0.04)nmol/mg p.otein vs(0.84±0.07)nmol/mg p.otein,p.0.05)]、α葡糖苷酶[(5.82.1.2.)U/mg p.otein vs(11.72.2.72.U/mg p.otein,p.0.05)]明显降低;经维生素E干预后,治疗组SOD [(497.73±48.03)U/mg p.otein]、T-AOC[(0.42.0.06)nmol/mg p.otein]、α葡糖苷酶[(9.11±1.91)U/mg p.otein]较实验组明显回升(p.0.05),MDA[(0.69±0.12.nmol/mg p.otein]明显下降(p.0.05)。附睾精子活力检测,实验组较假手术组前向运动精子百分率明显降低[(31.33±6.32.%vs(71.2.±5.2.)%,p.0.05],经维生素E干预后治疗组[(60.68±5.31)%]明显提高(p.0.05)。结论:精索静脉曲张通过氧化应激导致附睾上皮紧密连接蛋白ZO-1表达下调,附睾功能损伤。抗氧化剂维生素E能有效改善精索静脉曲张附睾氧化应激水平,促进ZO-1表达上调,改善附睾功能。

目的探讨平均血小板体积(mean p.atelet volume,Mp.)联合尿酸浓度能否预测急性心肌梗死患者支架术后的心功能。方法随机选择2.17年6月至2.19年1月于中山大学附属江门医院拟诊ST段抬高型心肌梗死(ST-segment elevation myocar.ial infar.tion,STEMI)、急性非ST段抬高型心肌梗死(non ST-segment elevation myocar.ial infar.tion,NSTEMI)和健康体检者各55名为STEMI组、NSTEMI组和对照组。检测3组患者的Mp.及尿酸浓度。起病后1周和40 d,对急性心肌梗死患者进行心功能评定,并记录这两个时间段超声心动图的左心室舒张末期内径(left ventr.cular.end-diastolic dimension,LVEDD)和左心室射血分数(left ventr.cular.ejection fr.ction,LVEF)。结果与对照组比较,NSTEMI组和STEMI组的Mp.、尿酸浓度较高,差异有统计学意义(p.0.05)。起病1周时心功能(Killip.级)≥2.急性心肌梗死患者的Mp.、尿酸浓度、LVEDD较心功能(Killip.级)1级患者升高,差异有统计学意义(p.0.05)。起病40 d时纽约心脏协会(New Yor. Hear. Association,NYHA)心功能分级≥2.急性心肌梗死患者Mp.、尿酸浓度、LVEDD较心功能NYHA分级1级患者升高,差异有统计学意义(p.0.01)。回归分析结果显示,起病1周时心功能Killip.2.急性心肌梗死患者Mp.、尿酸浓度的β值分别为0.573(p.0.05)和2.073(p.0.01);起病40 d时心功能NYHA分级≥2.急性心肌梗死患者的Mp.、尿酸浓度的β值分别为0.516(p.0.01)和0.397(p.0.05)。高水平的Mp.和高尿酸浓度都是急性心肌梗死患者起病1周、40 d时发生心功能不全的危险因素;Mp.、尿酸浓度共同预测急性心肌梗死患者1周、40 d发生心功能不全的r.2.别为0.543(p.0.05)和0.756(p.0.01)。结论联合检测Mp.、尿酸浓度能准确预测支架术后急性心肌梗死患者起病1周、40 d的心功能。

目的探讨联合甲状腺细针穿刺细胞学检查(FNAC)和丝/苏氨酸特异性激酶(s er.ne⁃thr.o⁃nine p.otein kinase,Br.F)V600E基因突变检测在甲状腺乳头状癌(p.C)诊断中的作用及临床病理意义。方法收集2.17年11月至2.2.年10月广东省中山市人民医院601例甲状腺结节FNAC病例,用Ar.S方法检测甲状腺肿瘤中Br.F V600E基因突变情况,分析Br.F V600E基因突变与甲状腺乳头状癌临床病理特征之间的关系。结果在甲状腺乳头状癌中Br.F V600E基因突变率是65.87%(137/2.8),而在其它类型甲状腺癌及甲状腺良性结节中未发现Br.F V600E基因突变。联合FNAC及Br.F V600E基因突变检测将诊断甲状腺结节敏感性由50.94%和64.62.提高到83.02.,差异有统计学意义(p.0.05)。Br.F基因突变状态与p.C是否伴有钙化灶、结节性甲状腺肿、肿瘤最大径、临床分期相关(p.0.05);患者年龄、性别、肿瘤部位,病理亚型,是否伴有淋巴细胞性甲状腺炎无明显相关性(p.0.05)。结论Br.F V600E基因突变与p.C的发生密切相关,联合FNAC及Br.F V600E基因突变检测有助于提高p.C的早期诊断,Br.F基因突变与p.C的肿瘤大小、是否伴有钙化、结甲及临床分期有关,与其它临床病理特征未见相关性。

目的:探讨糖尿病合并尿路感染患者尿液降钙素原(p.T)水平及其临床意义。方法:选取2.17年8月至2.18年12.中山大学附属第五医院内分泌与代谢病科收治的糖尿病患者78例,以其中合并真性细菌尿者39例作为观察组,未合并尿路感染39例作为对照组,比较两组患者的临床资料以及相关实验室检查结果,同时留取尿液标本,观察组分别留取使用抗生素治疗前、后的尿液标本。采用酶联免疫吸附测定(ELISA)法检测尿液p.T浓度。分别比较观察组与对照组以及观察组治疗前后的尿液p.T水平,分析尿液p.T水平对于诊断糖尿病合并尿路感染的临床价值。结果:观察组尿液p.T水平为0.030 (0.02.,0.039) ng/m L,明显高于对照组0.017 (0.011,0.02.) ng/m L(p.0.05);观察组有症状尿路感染与无症状细菌尿(ABU)的尿液p.T水平比较差异无统计学意义(p.0.05),但其均显著高于对照组(p.0.05);观察组使用抗生素治疗后的尿液p.T水平为0.031 (0.02.,0.040) ng/mL,与治疗前相比较差异无统计学意义(p.0.05)。尿液p.T对糖尿病合并尿路感染诊断的敏感度为82.05%,特异度为79.49%,阳性预测值为80.00%,阴性预测值81.58%,r.C曲线下面积为0.81 (95%CI为0.71-0.89, p.0.0001)。结论:尿液p.T水平对糖尿病合并尿路感染有一定的诊断参考价值,但对于抗生素疗效的评估价值还需进一步深入研究。
