We were intrigued to read the retrospective study by Wang et al.“Propensity score matching study of 325 patients with spontaneous rupture of hepatocellular carcinoma”(1)published in the latest issue of Hepatobiliary Surgery *** authors elaborated the extremely significant conclusion that transarterial embolization(TAE)plus two-stage hepatectomy might be the optimal treatment for spontaneous rupture of hepatocellular carcinoma(RHCC)patients,whose overall survival(OS)and disease-free survival(DFS)aren’t significantly different compared with non-ruptured hepatocellular carcinoma(NHCC)patients undergoing hepatectomy,and had a better prognosis than other treatment,including TAE alone and one-stage *** highly appreciate the author’s research ***,when we read the document,there were some shortcomings.

Introduction:Inflammation plays a role in coronary artery disease(CAD).The neutrophil/lymphocyte ratio(NLR),monocyte/lymphocyte ratio(MLR)and platelet/lymphocyte ratio(PLR)are blood parameters associated with *** aimed to perform a systematic comparison and study the predictive values of these inflammatory parameters with respect to CAD ***:A total of 513 patients who had undergone coronary angiography(CAG)were retrospectively *** status,lipid profiles,CAG scans and hematological parameters were ***,MLR and PLR were *** patients were classified into a normal coronary group(n=133),coronary atherosclerosis(CA)group(n=149),chronic coronary syndrome(CCS)group(n=175)or acute myocardial infarction(AMI)group(n=56).NLR,MLR and PLR were compared among ***:NLR and MLR were higher in the CCS and AMI *** was higher in the AMI *** AMI group had higher NLR,PLR and MLR than the CCS *** regression analysis revealed that NLR(OR:1.227,95%CI:1.016–1.482.P<0.05)had a strong significant correlation with *** under the ROC curve of NLR was 0.580(95%CI=0.516–0.644)in predicting CCS and 0.727(95%CI=0.642–0.811)in predicting ***:NLR may be associated with the occurrence and progression of CAD,and may serve as a marker of inflammation.

Effect of Huangshukuihua(Flos Abelmoschi Manihot) on diabetic nephropathy:a Meta-analysis

OBJECTIVE: To assess the efficacy of Huangshukuihua(Flos Abelmoschi Manihot) on diabetic nephropathy(DN).METHODS: Articles were retrieved from Pub Med,EMBASE, the Cochrane Central Register of Controlled Trials, Web of Science, Chinese Biomedical Literature Database, and China National Knowledge Infrastructure Database, Chinese Evidence-Based Medicine Database, and Wanfang Database. Two reviewers independently reviewed the article. Only randomized controlled trials were included and 27 were identified involving 2239 patients(1143 in the treatment group and 1096 in the control group).RESULTS: Huangshukuihua(Flos Abelmoschi Manihot) had a significant effect on renal function by improving blood urea nitrogen and serum creatinine,reducing urine protein(24-h urine protein, and urinary albumin excretion rate), and improving serum albumin level, compared with the control ***: Our findings suggest that, although the bioactive ingredients and mechanism underlying renal protection are unknown, the role of Huangshukuihua(Flos Abelmoschi Manihot) in the treatment of DN deserves further investigation.

Predictors of liver failure after transarterial chemoembolization in patients with spontaneously ruptured hepatocellular carcinoma:A retrospective study

Background: Spontaneously ruptured hepatocellular carcinoma(rHCC) is a life-threatening condition. Transarterial chemoembolization(TACE) is a widely accepted treatment;however, it can lead to serious complications,especially liver failure. We sought to identify preoperative predictors of liver failure in patients with rHCC undergoing ***: Patients with rHCC who received TACE as the initial therapy were retrospectively studied at our institution between January 2016 and December 2021. Based on the occurrence of liver failure after TACE, the patients were divided into liver failure and no-liver failure groups. Predictors of liver failure after TACE were analyzed using univariate and multivariate regression analyses. The predictive performance was assessed using the area under the curve(AUC). Delong’s test was used to compare predictive ***: Sixty patients(19 and 41 in the liver failure and non-liver failure groups, respectively) were *** analysis showed that preoperative prothrombin activity(PTA) level(odds ratio [OR], 0.956;95%confidence interval [CI], 0.920–0.994;P = 0.024) and Child-Pugh grade B(OR, 6.419;95% CI, 1.123–36.677;P= 0.037) were independent predictors of liver failure after TACE in patients with rHCC. The AUCs of the preoperative PTA levels and Child-Pugh grade B for predicting liver failure after TACE in patients with rHCC were0.783 and 0.764, ***: Preoperative PTA level and Child-Pugh grade B were significant independent risk factors for liver failure after TACE in patients with rHCC. These can be used to predict liver failure after TACE in patients with rHCC for individual decision-making regarding treatment planning.

EFFECT OF LOSARTAN ON SLOWING PROGRESSION OF CHRONIC ALLOGRAFT NEPHROPATHY

Objective To investigate the effects of losartan, a specific angiotensin Ⅱ receptor blocker, on slowing progression of renal insufficiency in patients with biopsy-proven chronic allograft nephropathy （CAN） and the molecular mechanism of the therapy. Methods Twenty-two renal transplant recipients with biopsy-proven CAN （group A） were treated with losartan within two months after renal dysfunction for at least one year. Losartan was administered at a dose of 50 mg/d. Twenty-four recipients in the same fashion （group B） who never received angiotensin Ⅱ receptor antagonist were studied as control. The investigation time for each patient lasted one year. Renal functions and concentrations of plasma and urine transforming growth factor-betal （TGF-betal） were compared between the two groups at the initiation and end of the study. In group A, expressions of TGF-betal mRNA and immunofluorescence intensity of TGF-betal protein and pathological alterations in renal biopsy specimens were compared between before losartan therapy and after one year of the therapy. Results At the initiation of the investigation, no significant differences were found between group A and group B in clinical data such as donor age, cold-ischemia time, HLA mismatch, levels of creatinine clearance （Ccr）, plasma and urine TGF-betal concentrations. One year later, 14 of 22 （63.6%） patients showed stable or improved graft functions in group A, and 4 of 24 （16.7%） in group B. The difference was significant （P 〈 0.05）. At the end of the study, urine TGF-betal concentration was 273.8 ± 84.1 pg/*** in group A and 457.2 ± 78.9 pg/*** in group B. During one year study period, loss of Ccr was 6.6 ± 5.4 mL/min in group A and 16.2 ± 9.1 mL/min in group B. Both of the differences were significant between the two groups （P 〈 0.01）. No significant differences were found in plasma TGF-betal concentrations between the four values determined at the initiation and end of the study in the two groups （F= 2.56, P〉
