Bacterial vaginosis:a synthesis of the literature on etiology,prevalence,risk factors,and relationship with chlamydia and gonorrhea infections

Bacterial vaginosis(BV) is a common vaginal disorder in women of reproductive age. Since the initial work of Leopoldo in 1953 and Gardner and Dukes in 1955, researchers have not been able to identify the causative etiologic agent of BV. There is increasing evidence, however, that BV occurs when Lactobacillus spp., the predominant species in healthy vaginal flora, are replaced by anaerobic bacteria, such as Gardenella vaginalis, Mobiluncus curtisii, M. mulieris, other anaerobic bacteria and/or Mycoplasma hominis. Worldwide, it estimated that 20%–30% of women of reproductive age attending sexually transmitted infection(STI) clinics suffer from BV, and that its prevalence can be as high as 50%–60% in high-risk populations(e.g., those who practice commercial sex work(CSW). Epidemiological data show that women are more likely to report BV if they: 1) have had a higher number of lifetime sexual partners; 2) are unmarried; 3) have engaged in their first intercourse at a younger age; 4) have engaged in CSW, and 5) practice regular douching. In the past decade, several studies have provided evidence on the contribution of sexual activity to BV. However, it is difficult to state that BV is a STI without being able to identify the etiologic agent. BV has also emerged as a public health problem due to its association with other STIs, including: human immunodeficiency virus(HIV), herpes simplex virus type 2(HSV-2), Chlamydia trachomatis(CT) and Neisseria gonorrhoeae(NG). The most recent evidence on the association between BV and CT/NG infection comes from two secondary analyses of cohort data conducted among women attending STI clinics. Based on these studies, women with BV had a 1.8 and 1.9-fold increased risk for NG and CT infection, respectively. Taken together, BV is likely a risk factor or at least an important contributor to subsequent NG or CT infection in high-risk women. Safety and efficacy outcomes of the Xen45 Gel Stent use for refractory glaucoma:a surgery series from surgeon trainees at a tertiary teaching hospital

Background:To study the effect of an ab interno gelatin stent(XEN45 Gel Stent,Allergan Inc.,Irvine,California,USA)on intraocular pressure(IOP)as placed by glaucoma fellowship trainees in eyes with refractory ***:A prospective noncomparative study at a tertiary training center on 28 unique eyes undergoing ab interno gelatin stent implantation by glaucoma fellowship *** was collected at baseline and postoperatively at day 1,week 1,and months 1,3,5,and *** outcome was mean IOP *** outcomes included change in number of glaucoma medication classes and visual *** outcomes included needling *** success was defined by achieving≥20%reduction in IOP with the same or fewer classes of antiglaucoma medications from baseline without the need for secondary surgical intervention and/or stent ***:At baseline,28.6%(8/28)of the subjects had prior failed incisional glaucoma surgery in a study population that was 54%African-American,with 78%with severe glaucoma(average mean deviation of−14.58 dB).Thirteen subjects terminated their clinic visits before their 12-month postoperative visit,leaving 15 subjects for end point *** IOP went from 21.6 mmHg(range 12.0–31.0,SD 6.6)at baseline to 12.5 mmHg(range 7.0–19.0,SD 3.6),a 42.1%reduction(p<0.007).All subjects decreased the number of medication classes they were taking with an average reduction of 3.8(range 2–5,SD 0.9)to 1.3(range 0–3,SD 1.0)classes,or a 65.8%decrease(p<0.006).Crude surgical success was 80.0%for the 15 subjects that followed up at 12 *** Kaplan-Meier cumulative probability of success for all 28 subjects at 12 months was 70.4%(95%CI:44.7–85.8%).Regardless of the length of follow-up,21.4%(6/28)met failure criteria:3 subjects failed because they required secondary surgical intervention,and the other 3 did not have adequate IOP *** bleb needling rate was 28.6%(8/28)and repeat was 17.9%(5/28).Conclusions:Compared to the re

Study of the ionospheric anomaly before the Wenchuan earthquake

The behavior of the ionosphere before the Wenchuan earthquake is analyzed with the global TEC and ionospheric foF2 observed at Xiamen. It can be found from TEC maps that in the afternoon (16―18LT) on May 9, 2008, 3 days before the earthquake, there is an enhancement of TEC with an amplitude of 10―15 TECU appearing in the east-south direction of Wenchuan, and another enhancement appears at the conjugate region of the Southern Hemisphere with an amplitude of 10 TECU, but no obvious in-crement or decrement can be seen in other regions on global scale. It can also be found that on May 6 (6 days before the great earthquake), there is a decrement of TEC with small amplitude of 4TECU and larger area with 80° along the longitude in southern China, and there is no synchronous decrement observed at the conjugate region of the Southern Hemisphere. There are also many anomalies ob-served by ionosondes across China. The result shows that foF2 and TEC abnormally increased, which is different from pervious results that ionospheric parameters decreased prior to earthquakes. Pre-liminary results suggest that the enhancement on May 9 maybe has a close relationship with the pos-sible enhancement of ionospheric electric fields, and it may be an ionospheric precursor of earth-quakes. Whereas the decrement on May 6 may be attributed to the geomagnetic disturbance appearing on May 5.

Manganese fluoride(MnF2)is a high-performance lithium-ion battery anode material with an excellent structural stability,low synthesis cost,and better manufacturing ***,its low theoretical capacity(577 mAh g^(-1)),weak conductivity of fluoride,and poor recyclability limit its practical ***,oxygen vacancies(Ov)and heteroatomic doping are among the most promising strategies to modulate the inherent reduced electronic conductivity and kinetic response of electrode materials in order to boost their lithium storage ***,self-templating,self-optimizing,and self-supporting metal-organic framework template approach with the introduction of oxygen vacancies by substitution of exogenous heteroatoms is proposed,where triple heteroatom-doped(N,O,and F)carbon-encapsulated MOF-derived manganese fluoride(Ov-ZMF@NOFs)microstructures are ***,the exogenously introduced triple heteroatomic carbon matrix forms a fluffy three-dimensional mechanical structure,interlaced conducting networks,efficient conducting pathways,and intense electrochemical dynamics at the periphery of the manganese fluoride *** from the above-mentioned features,the Ov-ZMF@NOFs exhibit expected electrochemical properties with ultra-long recyclability(high reversible capacity of 419 mAh g^(-1)at 6 A g^(-1))and good rate performance(capacity of 232 mAh g^(-1)at a current density of 16 A g^(-1)).Theoretical calculations underline the essential contribution of multiple heteroatoms doping in boosting the electrode conductivity and reducing the lithium-ion migration energy *** controllable vacancy engineering and heteroatom doping technology at the nanoscale provides a new philosophy and concept for the design and fabrication of next-generation high-energy lithium-ion battery materials.
