Health technology assessment in traditional Chinese medicine in China: current status, opportunities, and challenges

Objectives:The widespread usage of traditional Chinese medicine(TCM)and the government's emphasis on TCM in China have created a positive macro-environment and great opportunities to develop and strengthen health technology assessment(HTA)in TCM.Over the past ten years,a series of policies have been issued by the government to promote the application of HTA and the development of TCM in China.This study reviews the concepts and characteristics of TCM,summarizes the current status of HTA in TCM,and analyzes the opportunities and challenges faced by HTA development in TCM with respect to deepening of healthcare reforms.Methods:Literature review and website searches were used to acquire information regarding the basic theories of TCM and the current status,opportunities,and challenges for HTA in TCM.Results:The number of articles on HTA in TCM rose substantially,especially those published in China National Knowledge Infrastructure(CNKI).With the optimistic policy environment and huge market demand for TCM,the development of HTA in TCM is expected to flourish in China.However,HTA in TCM also faces numerous challenges such as the absence of a clinical efficacy evaluation system for TCM and insufficient basic research evidence and qualified personnel.Conclusions:TCM has unique characteristic that distinguishes it from Western medicine.Currently,it is important to take all measures to address the existing main challenges and thereby take advantage of the present opportunities to develop and improve HTA in TCM.

Bacterial communities living inside the midgut of insects have been attracting increasing interest.Previous studies have shown that both the midgut and midgut contents harbor bacterial communities.However,whether the bacterial communities of the insect midgut are similar to those of the insect midgut contents（including the peritrophic membrane,food particles,and digestive fuids secreted by the midgut in this study） remains unknown.In the present study,we analyzed two economically important silkworms,the Chinese oak silkworm Antheraea pernyi（Lepidoptera:Saturniidae） and the mulberry silkworm Bombyx mori（Lepidoptera:Bombycidae）,through Illumina Mi Seq technology to address this issue.In A.pernyi larvae,17 phyla and 162 genera were found in the midgut,while 7 phyla and 36 genera were found in the midgut contents.For B.mori larvae,30 phyla and 465 genera were found in the midgut,but 22 phyla and 344 genera were found in the midgut contents.This evidence from the two silkworms suggests that the bacterial composition and diversity in the midgut are more diverse than those in the midgut contents.Principal component analysis revealed a signifcant difference in the bacterial community structure between the midgut and midgut contents of B.mori.To our knowledge,this is the frst study to compare the bacterial communities between the midgut and midgut contents in insects,and the results will provide useful information for probing the functional differentiation within the midgut in the future.

＜正＞The European Cosmetics regulation prevents the use of animal tests for evaluating cosmetics. To address the need for properly evaluating cosmetic ingredients,some "alternative" methods have been validated（skin irritation for example） and others are being developed.Most of the non animal assays are used not as standalone but in integrated testing strategies.In the area of genetic toxicity, several in vitro assays are available.However,it has been shown that these assays used in isolation or in combination show poor predictive performances compared to in vivo results.In the current regulatory context,it is important to find approaches that are predictive of in vivo situations.To address specifically the cutaneous route,genetic toxicity assays using reconstructed skin were developed.These models have several interesting features:- The biological barrier（also we now know that it is not as much efficient as that of normal human skin）.-The metabolic capability.:it has been shown that most of the enzyme present in human skin are also retrieved in these tissue and the also display activities more or less similar to that of normal skin.-The ability to test different types of molecules:liquids,solids,gels.In the current work,the tissues were used in a co-culture system.Human lymphoblasto’id cells（TK6） were cultured beneath the tissues.The tissues are made of keratinocytes sitting on a collagen matrix.It was previously checked that the medium used was compatible with both biological models.Different incubation procedures were investigated for the comet assay as well as for the micronucleus assay in order to find the more suitable ones.For the comet assay,optimal conditions were tested for dissociating the tissues. This is important because the tissue dissociation procedure when not performed appropriately may yield DNA damage that can bias the issue of the assay.The purpose of this project was to investigate the added value of this co-culture system compared to the existing ones.Combining the comet assay and the micronuc

This paper is to address a theoretical result on a global stabilization technique for a class of nonlinear interconnected systems or namely large-scale dynamic networks.The problem is of interest and has strong impacts in resolving complex nonlinear output regulation problems.To handle the problem,we first present a new tractable condition,based on an i ISS（integral input-to-state stable） network stability criterion proposed in literature,for nonlinear networks with cascade interconnection and i ISS dynamic uncertainties.Then,we develop a recursive procedure for achieving i ISS based global robust stabilization design.Distinguished from relevant studies in recent literature,our study is strictly extended and also leads to constructive i ISS-Lyapunov functions for the closed-loop system.

Neoadjuvant chemotherapy and cytoreductive surgery in epithelial ovarian cancer

Ovarian cancer is one of the leading causes of death among gynecological cancers. This is because the majority of patients present with advanced stage disease. Primary debulking surgery(PDS) followed by adjuvant chemotherapy is still a mainstay of treatment. An optimal surgery, which is currently defined by leaving no gross residual tumor, is the goal of PDS. The extent of disease as well as the operative setting, including the surgeon's skill, infl uences the likelihood of successful debulking. With extensive disease and a poor chance of optimal surgery or high morbidity anticipated, neoadjuvant chemotherapy(NACT) prior to primary surgery is an option. Secondary surgery after induction chemotherapy is termed interval debulking surgery(IDS). Delayed PDS or IDS is offered to patients who show some clinical response and are without progressive disease. NACT or IDS has become more established in clinical practice and there are numerous publications regarding its advantages and disadvantages. However, data on survival are limited and inconsistent. Only one large randomized trial could demonstrate that NACT was not inferior to PDS while the few randomized trials on IDS had inconsistent results. Without a defi nite benefi t of NACT prior to surgery over PDS, one must carefully weigh the chances of safe and successful PDS against the morbidity and risks of suboptimal surgery. Appropriate selection of a patient to undergo PDS followed by chemotherapy or, preferably, to have NACT prior to surgery is very important. Some clinical characteristics from physical examination, serum tumor markers and/or fi ndings from imaging studies may be predictive of resectability. However, no specific features have been consistently identifi ed in the literature. This article will address the clinical data on prediction of surgical outcomes, the role of NACT, and the role of IDS.

Association between Polymorphisms of Glutathione S-Transferase and Progression to Cervical Cancer in Women from Burkina Faso and Mali

Although persistence of high-risk human papillomavirus infection is the main risk factor, Glutathione S-Transferase highly polymorphic enzyme involved in the metabolism of xenobiotics, is a good candidate gene. The objective of this study was to compare the polymorphisms of Glutathione S-Transferase M1-null in women with cancerous lesions and without lesions. This study consisted of 322 uterine cervix samples of women from Mali and Burkina Faso with Cervical Intra-epithelial Neoplasia 2 and 3, adenocarcinoma and squamous cell carcinoma and 100 women with no lesions. Human Papillomavirus genotyping was performed by Real-time multiplex Polymerase Chain Reaction. Glutathione S-Transferase gene polymorphisms were determined using conventional Polymerase Chain Reaction followed by migration on agarose gel. A statistically significant association with high relative risks of 10.77 for the development of High grade Superficial or Squamous Intra-epithelial Lesion (95% CI = 5.59 - 20.72;p presented relative risks of 9 and 11.05 for high-grade lesions, 15 and 11.40 for cancer. Similarly, significant results had been observed in women with human papillomavirus positive and human papillomavirus negative. The results of the present study support the idea that the deletion of Glutathione S-Transferase M1 plays a crucial role in the progression of high-grade lesions and cervical cancer.

Among mammalian phylogenies,those characterized by rapid radiations are particularly problematic.The New World monkeys(NWMs,Platyrrhini)comprise 3 families and 7 subfamilies,which radiated within a relatively short time period.Accordingly,their phylogenetic relationships are still largely disputed.In the present study,56 nuclear non-coding loci,including 33 introns(INs)and 23 intergenic regions(IGs),from 20 NWM individuals representing 18 species were used to investigate phylogenetic relationships among families and subfamilies.Of the 56 loci,43 have not been used in previous NWM phylogenetics.We applied concatenation and coalescence tree-inference methods,and a recently proposed question-specific approach to address NWM phylogeny.Our results indicate incongruence between concatenation and coalescence methods for the IN and IG datasets.However,a consensus was reached with a single tree topology from all analyses of combined INs and IGs as well as all analyses of question-specific loci using both concatenation and coalescence methods,albeit with varying degrees of statistical support.In detail,our results indicated the sister-group relationships between the families Atelidae and Pitheciidae,and between the subfamilies Aotinae and Callithrichinae among Cebidae.Our study provides insights into the disputed phylogenetic relationships among NWM families and subfamilies from the perspective of multiple non-coding loci and various tree-inference approaches.However,the present phylogenetic framework needs further evaluation by adding more independent sequence data and a deeper taxonomic sampling.Overall,our work has important implications for phylogenetic studies dealing with rapid radiations.

The 22 q11.2 deletion is a common micro-deletion that causes an array of developmental defects including Di George syndrome and velocardiofacial syndrome. About 30% of patients with 22 q11.2 deletion develops schizophrenia. DGCR2（Di George Syndrome Critical Region Gene 2） is one of the 20-40 genes in the 22 q11.2 deletion. Reduced expression of DGCR2 has been found in schizophrenia patients. It encodes DGCR2 is a 550 – amino acid protein with a single trans-membrane domain in human and mouse. Reduced expression of DGCR2 has been found in schizophrenia patients;h. However, the physiological function of DGCR2 remains unclear. To address this question, we have generated DGCR2 deficient mice（DGCR2-Lac Z） where the DGCR2 gene is replaced by a cassette containing Lac Z. We found that the β-galactosidase activity in heterozygous mice was detectable in a number of brain regions in the mouse brain, with abundant expression in the hippocampus, hypothalamus, cortex and habenular nucleus. Immunostaining indicated that Using immunostaining with anti-DGCR2 antibody, we found that DGCR2 was expressed present in the neurons, and as well as it was mainly localized to the dendrites and DGCR2 puncta were co-localized with those of to PSD-95, indicating that it is expressed in synapses puncta. Remarkably, Furthermore, the spine density and the frequency of m EPSC were both reduced in hippocampal CA1 region of DGCR2 deficient mice. DGCR2 could interact with some presynaptic cell organizers like adhesion molecules, like Neurexins that are known to which regulate synapse formation and transmission. Experiments are under way to determine whether and how we will further determine whether DGCR2 regulates-Neurexins interaction is responsible for the synapse formation. Our study may reveal pathophysiological mechanisms of DGCR2-involved mental disorders.

Severe traumatic brain injury remains a major health-care problem worldwide. Although major progress has been made in understanding of the pathophysiology of this injury,this has not yet led to substantial improvements in outcome. In this report,we address present knowledge and its limitations,research innovations,and clinical implications. Improved outcomes for patients with severe traumat- ic brain injury could result from progress in pharmacological and other treatments,neural repair and regeneration,optimisation of surgical indications and techniques,and combination and individually targeted treatments. Expanded classification of traumatic brain injury and in- novations in research design will underpin these advances. We are optimistic that further gains in outcome for patients with severe traumat- ic brain injury will be achieved in the next decade.
