Clinical outcomes of atezolizumab in combination with etoposide/platinum for treatment of extensive-stage small-cell lung cancer:A real-world,multicenter,retrospective,controlled study in China

Objective:Atezolizumab along with chemotherapy has prolonged the survival of patients with extensive-stage small-cell lung cancer(ES-SCLC)worldwide,although real-world(RW)data are lacking in *** study was designed to evaluate the efficacy and clinical outcomes of atezolizumab plus etoposide/platinum(EP).Methods:Data obtained in this retrospective study were captured from six oncology units of five medical facilities from January 2019 to April *** first-line treatments,atezolizumab combined with EP *** alone,we primarily evaluated progression-free survival(PFS);other efficacy indicators,including overall survival(OS),objective response rate(ORR),and patterns of SCLC progression and adverse events(AEs)were ***:The primary analysis included data from 225 patients,of whom 133 received EP along with atezolizumab(atezolizumab group)and 92 received EP alone(EP group).The PFS duration of the atezolizumab group[7.10 months;95%confidence interval(95%CI),6.53-9.00]exceeded that of the EP group(6.50 months;95%CI,4.83-7.53).Overall,the hazard ratio(HR)was 0.69(95%CI,0.49-0.97)(P=0.029);particularly,the HR was 0.54(95%CI,0.36-0.80)among patients undergoing≥4 chemotherapy cycles and 0.33(95%CI,0.20-0.56)among individuals with atezolizumab *** ORR and disease-control rate(DCR)were similar between the two *** of incomplete OS data,the median OS was not determined for either *** marrow suppression was the most common AE detected(58.6%)in the atezolizumab ***-related AEs occurred in 19 patients in the atezolizumab group(14.3%),with only one case of grade 3 ***:This RW study in China demonstrated improved clinical outcomes of atezolizumab along with EP for ES-SCLC,particularly in the chemosensitive *** results align with the results of the IMpower133 study,although the impact of this treatment modality on OS warrants additional follow-up studies.

OBJECTIVE: To observe the curative effect of Jinye Baidu granule in the treatment of fever and swollen and sore throat caused by wind-warmth lung-heat disease(heat in the lung-wei) to further identify the ***: This randomized, double-blind, parallel, controlled trial will include patients with acute upper respiratory infection and wind-warmth lung-heat disease(heat in the lung-wei). Patientswith serious bacterial infection(white blood cell count > 12 × 10~9, neutrophils > 80%) will be excluded. Patients will be divided into three categories(blocks) according to their condition: fever only, a swollen and sore throat, and combined fever plus a swollen and sore throat. Patients within each of the three blocks will be further divided into a treatment group and a control group via stratified blocked randomization. The treatment group will be treated with Jinye Baidu granule, and the control group will be treated with Fufang Shuanghua granule. The primary outcome measure will be body temperature recovery time for patients with fever, throat symptom score for patients with a swollen and sore throat, and body temperature recovery time and throat symptom score for patients with combined fever plus a swollen and sore throat. Routine blood testing, urine testing, liver function, kidney function and ECG data of all patients will be collected as safety indices before and after enrollment, and adverse events will be recorded during the whole trial ***: This study protocol will include stratified block analysis according to patients' symptoms, and identify the accurate clinical indications of Jinye Baidu granule. It will also enable safety evaluation from laboratory indices and adverse events, which will provide reliable evidence for clinical treatment.

Spirometric Reference Equations for Semi-Urban and Urban Bantu Cameroonians

Background: Spirometric reference values vary substantially across ethnic groups, and remain largely poorly characterized among Africans. We derived spirometric reference equations for adult Cameroonians and compared their performance with those derived from other ethnic groups. Methods: Spirometric variables according to the American Thoracic Society/European Respiratory Society 2005 guidelines were acquired in voluntary healthy non-smoker subjects in Yaounde (Capital City) and Foumbot (semi-urban area in West Region), in Cameroon during November 2011 to January 2012 (Yaounde) and August 2012 (Foumbot). Reference equations were derived separately for men and women from multiple linear regressions. Results: A total of 411 subjects (206 men) met the inclusion criteria. The mean age was 39.5 ± 16.1 years (min - max: 18 - 85 years) for men and 39.2 ± 14.1 years (18 - 90 years) for women. Age and height were the only variables significantly associated with spirometric values in the final linear regression models. Derived reference values were lower than those derived from Global Lung Initiative 2012 equations for different ethnic groups, except for the forced expiratory volume in 1 second/forced vital capacity ratio (FEV1/FVC ratio). The mean FEV1/FVC ratio was 0.88 ± 0.07 for Cameroonian men and 0.89 ± 0.07 for Cameroonian women. Variations in the performance of derived models in bootstrap internal validation were marginal. Conclusion: This study highlights the importance of deriving specific predictive equations for each ethnic group. The use of adjustment factors applied to Caucasian equations when compared with the values derived in our study leads to an overestimation of the values for FEV1 and FVC.

AIM: To investigate the role of hepatic peroxisome proliferator-activated receptor-γ (PPAR-γ) in increased susceptibility to endotoxin-induced toxicity in rats with bile duct ligation during ***: Male Sprague-Dawley rats were subjected to bile duct ligation (BDL). Sham-operated animals served as controls. DNA binding were determined by polymerase chain reaction, Western blotting analysis,and electrophoretic mobility shift assay, *** and sham-operated rats received a non-lethal dose of intraperitoneal lipopolysaccharide (LPS) injection (3 mg/kg, i.p.). Additionally, the potential beneficial effects of the PPAR-γ agonist rosiglitazone were determined in BDL and sham-operated rats treated with a non-lethal dose of LPS. Survival was assessed in BDL rats treated with a non-lethal dose of LPS and in sham-operatedrats treated at a lethal dose of LPS (6 mg/kg, i.p.).RESULTS: PPAR-γ activity in rats undergoing BDL wassignificantly lower than in the sham-controls. Hepatic PPAR-γ gene expression was downregulated at both them RNA and protein levels. In a parallel group, serumlevels of pro-inflammatory cytokines were nearly unde-tectable in the sham-operated rats. When challenged with a non-lethal dose of LPS (3 mg/kg), the BDL ratshad approximately a 2.4-fold increase in serum IL-6,a 2.7 fold increase in serum TNF-α, 2.2-fold increasein serum IL-1 and 4.2-fold increase in serum ALT. Thesurvival rate was significantly lower as compared with that in sham-operated group. Additionally, rosiglitazone significantly reduced the concentration of TNF-α, IL-1β, IL-6 and ALT in sham-operated rats, but not in BDL rats, in response to LPS (3 mg/kg). Also, the survival was improved by rosiglita zone in sham-operated rats challenged with a lethal dose of LPS, but not in BDL rats, even with a non-lethal dose of LPS (3 mg/kg).CONCLUSION: Obstructive jaundice downregulates hepatic PPAR-γ expression, which in turn may contributeto hypersensitivity towards endotoxin.

Non-Invasive Pressure Support Ventilation in Major Lung Resection for High Risk Patients: Does It Matter?

Background and purpose: Patients with severely impaired pulmonary function have an increased operative risk for major lung resection. The clinical benefits of pre- and perioperative, non-invasive pressure support ventilation (NIPSV) have up to now not been extensively evaluated. Patients with severely reduced pulmonary function were investigated in this prospective and randomised single centre clinical trial. Methods: Standard pulmonary evaluation was performed in all patients before major lung resection. To predict postoperative pulmonary function, a lung perfusion-ventilation scan was carried out. All patients enrolled in the study were instructed preoperatively on how to use a NIPSV respirator. Af-ter lung resection patients were randomised either for continuation of NIPSV or for standard treatment. Results: Of the 52 patients assessed, 21 patients met the inclusion criteria for the study protocol. Predicted mean postoperative FEV1 was 1.10 L (range 0.92 - 1.27 L). Lobectomy was performed in 14 patients, pneumonectomy in 6 patients and a segmentectomy in 1 patient. No inhospital deaths occurred. Pulmonary complications (reintubation, pneumonia) were more frequent in the NIPSV group than in the control group (3 patients versus 1 patient), without statistical significance (p = 0.31). Conclusions: We observed no mortality and a low morbidity in this high risk group. Postoperative continuation of NIPSV had no beneficial effect on the clinical outcome. Preoperative conditioning with NIPSV, however, seems to be a suitable tool for patients with severely impaired pulmonary function. This study may serve therefore as basis for further investigations for the potential clinical benefits of prophylactic NIPSV in major lung surgery.

OBJECTIVE:To investigate the effects of ribavirin administration combined with Reduning in a mouse model of influenza A(H1N1)-induced severe ***:Influenza A/Beijing/501/2009(H1N1)-infected C57BL/6 mice were randomly divided into four experimental groups treated with either a mock injection of phosphate-buffered saline(PBS),ribavirin(66.6 mg/kg daily)or Reduning(86.6 mg/kg daily),or a combination of both,for 7 *** were monitored for clinical signs and survival,and body weight was measured daily for 14 *** titer,lung wet-to-dry ratios,pathology and cytokines including interleukin(IL)-6,IL-10,and interferon(IFN)-γwere assayed on different ***:In the untreated group injected with phosphate buffer saline,all the mice died of the *** survival rate of mice treated with Reduning was only 10%,whereas 100%of the ribavirin-and the combination-treated mice *** lung viral loads indicated that ribavirin significantly inhibited virus replication,whereas Reduning did *** wet-to-dry ratios demonstrated that both ribavirin and Reduning,administered together or separately,reduced acute lung edema compared with results in the untreated *** analyses also showed that treatment with a combination of both drugs relieved pathological lesions,whereas the single drug treatment did *** of IL-6,IL-10 and IFN-γin mice treated with ribavirin or the combination of both ribavirin and Reduning were all significantly lower than in the untreated group,especially in the combination-treated *** addition,Reduning administration significantly decreased both IL-6 and IL-10production but had no effect on IFN-γ.CONCLUSION:Due to the synergistic effect of antiviral and antiinflammation,the combination of ribavirin and Reduning could be an effective treatment for severe H1N1 which was considered to be significant to delayed antiviral and drug resistant.

目的探讨动态肺量测定技术在支气管哮喘中的应用。方法建立动态肺量测定技术,对20例支气管哮喘患者每天清晨、下午和入睡前连续14d动态监测气道功能、症状和情绪。结果白天标准肺功能检查结果正常的支气管哮喘患者,第一秒用力呼气量(FEV1)和最大呼气峰流量(PEF)在夜间和凌晨可出现明显下降。肺功能指标与支气管哮喘症状的相关分析发现,只有7例患者(35%)支气管哮喘症状与FEV1和PEF存在相关关系。4例患者(20%)肺功能基本正常,但有很多症状,症状与FEV1和PEF无相关关系,但与负面情绪相关。另外2例患者(10%)肺功能损害严重,但几乎没有症状。结论动态肺量测定技术能客观反映支气管哮喘患者气道功能的昼夜自然变化,因此可以更准确地反映支气管哮喘的严重程度。动态肺量测定技术结合症状和情绪的同步监测,可以帮助识别出特殊症状感觉类型的支气管哮喘患者。

Objective:To analyze and study the efficacy and safety of Endu combined with pemetrexed and cisplatin in the clinical treatment of lung ***:From August 2016 to September 2020,32 patients with lung adenocarcinoma who were treated in our hospital were selected for group *** to their specific treatment plan,the patients were divided into control group and experimental group,with 16 cases in each *** control group was treated with pemetrexed and cisplatin,and the experimental group was treated with Endostar combined with the treatment received by the control *** clinical efficacy and safety of the two regimens were assessed by comparing the changes in symptoms and the incidence of adverse reactions between the two groups of patients after ***:The disease control rate of the experimental group was significantly higher than that of the control group,and there was no significant difference in the incidence of adverse reactions between the two ***:From the experimental results,we found that the treatment of patients with lung adenocarcinoma by Endostar combined with pemetrexed and cisplatin can effectively improve the treatment efficacy without increasing adverse reactions and therefore relevant chemotherapy regimens can be considered for wider clinical applications.
