Childhood vitiligo is a common disorder of pigmentation in India. Considering the lack of uniformly effective and safe treatment modalities for children with vitiligo, search for newer therapeutic agents continues. This study was designed to evaluate the role of topical tacrolimus in the treatment of childhood vitili go. Twenty-five children with vitiligo were treated with topical 0.03%tacrolim us ointment applied twice daily for 12 weeks. Response was noted as marked to co mplete ( > 75%repigmentation), moderate (50-75%repigmentation) and mild ( olimus is an effec tive and well-tolerated treatment modality in Asian children with vitiligo.

The longitudinal profile of a river channel can be described in function of mathematical expressions. The logarithmic fit is the most used method to describe the relative equilibrium of a channel elevation profile. Rivers showing zones of high channel gradient and convex-upward profiles can be evaluated in function of the offset distance with respect to the logarithmic curve. The Jama River profile has been constructed using differential GPS data for the downstream reach and 4-m-grid DEM data base for the remaining headward profile. The resulting longitudinal profile shows a prominent knickpoint of about 80 m in elevation characterized by sharp local convexity. The offset distance in vertical direction from the logarithmic plot is interpreted as the successive accumulation of multiple uplift episodes associated with the Jama Fault System activity. The horizontal offset is suggested to represent the remaining retreated distance for the upstream propagation of the Jama knickpoint. The highly resistant Cretaceous rocks outcropping along the Jama knickpoint reach acts as an inflection point between two subprofiles that show well-fitted logarithmic curves, thereby representing a temporary partitioned equilibrium along the Jama River profile.

Intraperitoneal drains during open appendicectomy for gangrenous and perforated appendicitis

Intra-abdominal drains are still routinely used in the surgical management of gangrenous and perforated appendicitis. A systematic review was performed with the aim of establishing their influence on postoperative complications in such cases. A literature search was conducted using the search engines Pub Med and Cochrance Central Register of Controlled Trials. Included were retrospective case-controlled and prospective randomized controlled trials on the use of drain for open appendicectomy in gangrenous and perforated appendicitis. Twelve articles were found that met the inclusion criteria. Intrabdominal abscesses, postoperative ileus, surgical site infections, fecal fistulas and burst abdomen had significant higher incidences in the drain vs non drain group(10.3%, 20.3%, 32.5%, 3.4% and 5.7% vs 4.7%, 8.5%, 16.2%, 0% and 0%, respectively). In most cases the risk was more than doubled in the drain group compared to the non-drain one. There were no significant differences among groups in termsof mortality while the results were underpowered to effectively evaluate wound dehiscence and adhesions. The use of intra-abdominal drains in the management of gangrenous and perforated appendicitis by open appendicectomy is associated with an increased rate of common postoperative complications.

Evaluation on Resource Utilization Potential of Typical Agricultural Waste in Southwest China

To impel resource utilization of agricultural waste,four provinces and cities of southwest China were taken as research objects,to analyze yield change of typical agricultural waste in southwest China during 2010-2016,and resource amounts of typical agricultural crops straw and livestock and poultry waste and their nutrient contents in four provinces and cities of southwest China in 2016 were calculated. The results showed that resource amount of typical agricultural waste in southwest China overall showed a continuously increasing trend with the improvement of agricultural comprehensive productivity. Straw resource amount of agricultural crops was dominant by rice and corn,and they accounted for 61. 61% of total amount in southwest China. Livestock and poultry manure was dominant by cattle and pig excrement,and they accounted for 89. 59% of total in southwest China. Seen from distribution region,the resource amount sequence of typical agricultural waste in southwest China was Sichuan > Yunnan > Guizhou > Chongqing. N,POand KO in straw resource of typical agricultural crops in southwest China of 2016 were respectively 76. 11 ×10~4,23.36 ×10~4 and 125. 78 × 10~4 t,and total nutrient amount was 22. 53 × 10~5 t. N,POand KO of livestock and poultry manure resource were respectively 27. 28 × 10~4,12. 87 ×10~4 and 62. 45 × 10~4 t,and total nutrient amount was 10. 26 × 10~5 t. Therefore,resource amount of agricultural waste in southwest China is rich in a long term,and nutrient storage is huge. Its reasonable recycling is favorable for sustainable development of agriculture in southwest China,and has important actual significance for guaranteeing crop yield and decreasing chemical fertilizer use.

Context： Balance is an important aspect of sports performance, which can be impaired following MHI （minor head injury） The BESS （balance error scoring system） is a tool commonly used for assessment of post-MHI recovery. Objective： This study aimed to ascertain test-retest reliability and minimal detectable change of the BESS in order to ensurem that it had appropriate clinical levels of reliability and sensitivity. Design： Test-retest reliability. Setting： University indoor sport facility. Participants： N = 36 sub-elite athletes. Interventions： The BESS was undertaken by the participants on two separate testing sessions, 1 week apart. Main Outcome Measures： Test-retest reliability and minimal detectable change. Results： Strong test-retest reliability of the total BESS score was identified （1CC （intraclass correlation coefficients） = 0.784） with a MDC （minimal detectable change） of 6-10 error points, dependent on confidence interval used. Conclusions： The strong reliability score and the relatively low MDC when using conservative measures would recommend the use of BESS in post-MHI assessment of young academy-level athletes. However, more research is required to further determine BESS MDC in both sub-elite and elite groups of athletic performers so as to provide clinicians with an accurate guideline regarding whether an athlete is presenting with post-MHI balance impairment.

Objective:To investigate the clinical symptoms of coronavirus disease 2019(COVID-19)patients with and without B.1.1.7 ***:This retrospective observational study included COVID-19 patients who were divided into two groups,the mutation and the non-mutation *** characteristics,clinical characteristics,laboratory parameters,and mortality rates were recorded and compared between the two ***:A total of 196 patients were included in the *** relationship between the mutant virus status and sex,age,comorbidity,survival status,and disease severity was not significant(P>0.05).No significant differences were found in duration of hospitalization between the mutation and the non-mutation group(P>0.05).However,there was a statistically significant difference between patients with and without mutant viruses in hemoglobin,mean platelet volume,procalcitonin,low density lipoprotein,iron-binding capacity,potassium,calcium,C-reactive protein,folate,creatine kinase myocardial band,D-dimer,and international normalized ratio(P<0.05).Conclusions:No significant difference is found in mortality rate,disease severity or duration of hospitalization between the patients with and without variant *** monitoring of COVID-19 patients is required for all variants.

Background:Few data are available on cutaneous leishmaniasis caused by dermot ropic species in human immunodeficiency virus (HIV)-infected patients. Objectiv es:To describe nine cases of cutaneous leishmaniasis in HIV+patients and to co mpare their clinical features and their response to treatment with those of HIV -patients with the forms of leishmaniasis commonly found in French Guiana. Meth ods:A case-control study was carried out between July 1994 and December 2000 i n French Guiana. We compared the following variables in nine HIV-infected patients with leishmaniasis and 27 matched controls:clinical type of leishmaniasis, number of lesions, presence of lymphangitis and adenopathy, the rate of recovery after treatment, and recur rence or reinfection. Results:Eight of the HIV-infected patientshad localized cutaneous leishmaniasis and one had mucocutaneous leishmaniasis. All of the cont rols had localized cutaneous leishmaniasis. Leishmania guyanensis was the only s pecies isolated from HIV-infected subjects. HIV-Leishmania coinfected patients had a higher rate of recurrence or reinfection (P 200 CD4 +T cells mm-3) is characterized by a higher rate of recurrence or reinfection and is more difficult to treat than that in HIV-subjects.

Diaphragmatic hernias may be acquired or congenital. Among congenital hernias, the most common is Bochdalek hernia and eight out of ten Bochdalek hernias occur on the left side. They are usually diagnosed in the paediatric age group, and it is exceedingly rare for the diagnosis to be established in adulthood. Renal ectopy associated with a Bochdalek hernia is extremely rare, and very few cases are reported worldwide. We are reporting a case of a 73-year-old male patient with a right-sided Bochdalek hernia and renal ectopy. In this case, promp diagnosis and treatment could help to reduce the high risk of death in this kind of patients.
