Predictive Factors of Renal Damage during Sickle Cell Disease at the hematology-Oncology Department of Donka University Hospital

Introduction: Sickle cell disease, which is the most common hereditary hemoglobinopathy in the world, attacks all body systems, particularly the kidneys. The view of this study was to investigate the predictive factors of kidney damage during sickle cell disease. Materials and methods: It was a retrospective, descriptive and analytical study on files of sickle cell patients hospitalized in the hematology-Oncology Department of Donka University Hospital during a period from January 1, 2016 to December 31, 2019. Records of sickle cell patients with one or more renal abnormalities were retained. Sickle cell patients without kidney damage were also selected for a comparative study. Only patients without sickle cell disease were excluded. Results: Seventy-five (75) medical records were collected during the study period. From these cases, thirteen (13) records with kidney disease were observed, a frequency of 17%. The mean age of patients was 24.2 years for extremes of 10 and 65 years. The sex ratio was 1.6 in favor of men. The SSFA 2 form was the most represented with 92%. 24-hour proteinuria was measured in 13 patients between whom 6 patients (46.2%) had a proteinuria level ≤ 1 g. Eight (8) patients (61.5%) were in stage 1 of chronic kidney disease. The most common type of renal involvement was tubulo-interstitial nephropathy with 8 patients (61.5%). Bivariate analysis showed that elevated serum creatinine (P < 0.001), elevated serum uremia (P < 0.001) and the SSFA 2 form of the sickness (P < 0.003) were the main factors linked with renal damage in sickle cell patients. Conclusio n: After the observation of an increased serum creatinine and urea, a predominance observation of the SSFA 2 form, it should be possible to target patients for whom screening for kidney damage should henceforth be systematic.

Background： Despite substantial progress toward measles control are making in China, measles outbreaks in immunocompromised population still pose a challenge to interrupt endemic transmission. This study aimed to investigate the features of measles in pediatric hematology and oncology patients and explore the reasons behind the outbreak. Methods： We collected demographic, epidemiological, and clinical data ofimmunocompromised measles children. All suspected measles cases were laboratory-confirmed based on the presence of measles lgM and/or identification of measles RNA. The clinical data were statistically analyzed by t-test for continuous variables and Fisher＇s exact test for categorical variables. Results： From March 9 to July 25 in 2015, a total of 23 children with malignancies and post hematopoietic stem cell transplantation （post- HSCT） were notified to develop measles in Shanghai. Of these 23 patients with the median age of 5.5 years （range： 11 months 14 years）, 20 （87.0%） had received 1-3 doses of measles vaccine previously; all patients had fever with the median lever duration of 8 days; 21 （91.3%） had cough; 18 （78.3%） had rash; 13 （56.5%） had Koplik＇s spot; 13 （56.5%） had complications including pneumonia and acute liver failure： and five （21.7%） vaccinated patients died from severe pneumonia or acute liver failure. Except the first patient, all patients had hospital visits within 7-21 days beibre measles onset and 20 patients were likely to be exposed to each other. Conclusions： The outcome of measles outbreak in previously vaccinated oncology and post-HSCT pediatric patients during chemotherapy and imnaunosuppressant medication was severe. Complete loss of protective immunity induced by measles vaccine during chemotherapy was the potential reason. Improved infection control practice was critical tbr the prevention of measles in rnalignancy patients and transplant recipients.

Objective： To evaluate the effect of moxa-stick suffumigation in the hematology and hematopoieticstem cell transplantation （HSCT） wards with luminar flow. Methods： The plate exposure method was usedto measure the effect of air-disinfection of moxa-stick suffumigation in hematology and HSCT wards. Theyearly average qualified rates of air sampling in HSCT wards were evaluated from 2007 to 2010. To furtherinvestigate the disinfecting effect of moxa-stick suffumigation, the colony counts of common pathogens（including Staphylcoccus aureus and Pseudomonas aeruginosa） before and after moxa-stick suffumigation werecompared. Results： The mean air quality rates of the HSCT wards with class 100 laminar flow were all above90.0% （91.2%-96.2%） from 2007 to 2010. Moxa-stick suffumigation effectively decreased the presence ofbacteria in the hematology ward＇s air （P〈0.01）. The most notable effect was the drastic reduction in the colonycounts of Staphylococcus aureus and Pseudomonas aeruginosa on the blood plates exposed to air treatedwith moxa-stick suffumigation （77.1 ± 52.9 cfu/m2 vs 196.1 ± 87.5 cfu/m2, P〈0.01; and 100.2±35.3 cfu/m2 vs371.5± 35.3 cfu/m2, P〈0.01）. Conclusion： Moxa-stick suffumigation proved to be a reliable and effective air-disinfection method for hematology and HSCT wards, and hence, it should be employed extensively.
