Magnetic resonance imaging of the ligaments of the craniocervical region at 3Tesla magnetic resonance unit: Quantitative and qualitative assessment

Purpose: The assessment of the morphology and dimensions of the craniocervical ligaments using a 3 Tesla (T) Magnetic Resonance (MR) scanner, the correlatation of our results with those from cadaveric and other MR studies and the detection of the most appropriate sequence for the best imaging of the craniovertebral junction ligaments. Methods: 58 healthy volunteers (mean age 45 years) underwent a Magnetic Resonance Imaging (MRI) of the cervical spine at 3T MR unit. The MRI protocol included axial, coronal and sagittal Proton-Density (PD) sequences and sagittal T1 Fluid Attenuated Inversion Recovery (FLAIR) and T2 sequences. The images were evaluated by two radiologists and the posterior atlantoocipital ligament, the anterior atlantoocipital ligament, the transverse ligament and the apical ligament were anatomically detected, described and measured. Results: The transverse ligament was identified at 93.1%, the apical ligament was identified at 60.34%, the posterior at- lantooccipital membrane was identified at 94.8% and the anterior atlantooccipital membrane was identified at 96.5% of the cases. All ligaments appeared with low signal intensity, except the anterior atlantooc-cipital ligament which appeared with intermediate signal intensity. Their length, width and thickness were measured and, in general, correlated well with other anatomic and MR studies. Conclusion: Reliable assessment of the morphology and signal intensity of the craniocervical ligaments can be achieved with PD sequence at 3T MR imaging. The sagittal plane provides better delineation of the craniocervical (CC) ligaments but the axial and coronal planes are of paramount importance in the assessment of the transverse and apical ligaments.

Background There are few reports discussing the surgical pathological characteristics of superficial endobronchial lung cancer （SELC） defined as cancer growth limited to the bronchial wall. Its prognosis and corresponding TNM staging have not been fully clarified. Little is known as to whether T status is impacted by the existence of associated atelectasis or pneumonia （which might be controversial, indicating either T1 or T2）, and circumstantial invasion *** Between 1988 and 2007, 81 out of 8817 surgically treated patients met SELC criteria; there was no detectable invasion beyond the bronchial wall. A retrospective review was performed and follow-up information was *** The overall five-year survival rate of 81 patients was 85.6%; for NOM0 （n=67）, N1M0 （n=7） and N2M0 （n=7）patients, they were 89.3%, 75.0% and 60.0%, respectively. Intraluminal tumor size measured from 0.4 to 3.0 cm;obstructive atelectasis or pneumonia was noted in 14 patients. The presence of tumor-associated obstructive atelectasis or pneumonia did not have a significant impact upon prognosis （P=0.96）, nor did the greatest diameter of the tumor （P=0.70）. Histology showed carcinoma in situ （level one） in 13 cases; invasion of the submucosal layer （level two） in 12,involvement of the muscular layer （level three） in 20, invasion into the space between the muscular layer and cartilage （level four） in 21, and bronchial cartilage infiltration in 15 （level five）. We have synthesized water-stable polyaniline nanoparticles coated with tri- armed polyethylene glycol chains using a solvent-shift method and confirmed their colloidal size and aqueous solubility. Furthermore, we have demonstrated that the polyaniline nanoparticles can be doped with biological dopants to produce distinct color changes allowing the detection of live cancer cells.

目的用1H-MRS及通过扩散加权分段读取平面回波成像获得的表观扩散系数(ADC)区分良性和转移性脊柱压缩骨折的混杂效应,测定椎体骨折中残存的脂质含量。方法对52例因不同程度的外伤、骨质疏松或临床已知的转移性疾病引起后背痛和(或)椎体压缩性骨折的病人行1.5T单体素MRS检查来观测骨折椎体的骨髓中水和脂质的成分,以及DWI在b=0和650s/mm2时ADC值。结果 46例椎体骨折,脂质移位的总数各不相同。在轻度的冲击伤中,脂质部分移位[ADC值为(1.60±0.20)×10-3mm2/s]或全部移位,ADC值较高[(2.20±0.27)×10-3mm2/s]。在急性重度冲击伤中,脂质含量可以忽略不计,仍观测其中间ADC值。在肿瘤浸润的情况下,也忽略不计脂质含量并观测其中间ADC值[(1.22±0.14)×10-3mm2/s]。ROC曲线的诊断准确度为0.944。结论 ADC-MRS分析提供了椎体骨折残存脂质含量的信息,可帮助区分轻度冲击伤时良性和转移性椎体骨折。要点①创伤可引起正常椎体骨髓中脂质的移位。②残存脂质含量较高时可使ADC值降低和复杂。③仅应用ADC不能准确区分骨质疏松和转移性椎体骨折。④联合应用ADC和MRS有助于鉴别诊断。⑤扩散加权分段读取平面回波成像提高了脊柱影像的质量。

Background： Glehnia littoralis has been used for traditional Asian medicine, which has diverse therapeutic activities. However, studies regarding neurogenic effects of G. littoralis have not yet been considered. Therefore, in this study, we examined effects of G. littoralis extract on cell proliferation, neuroblast differentiation, and the maturation of newborn neurons in the hippocampus of adult mice. Methods： A total of 39 male ICR mice （12 weeks old） were randomly assigned to vehicle-treated and 100 and 200 mg/kg G. littoralis extract-treated groups （n = 13 in each group）. Vehicle and G. littoralis extract were orally administrated for 28 days. To examine neurogenic effects ofG. litmralis extract, we performed immunohistochemistry tbr 5-bromo-2-deoxyuridine （BrdU, an indicator for cell proliferation） and doublecortin （DCX, an immature neuronal marker） and double immunofluorescence staining for BrdU and neuronal nuclear antigen （NeuN, a mature neuronal marker）. In addition, we examined expressional changes of brain-derived neurotrophic factor （BDNF） and its major receptor tropomyosin-related kinase B （TrkB） using Western blotting analysis. Results： Treatment with 200 mg/kg, not 100 mg/kg, significantly increased number of BrdU-immunoreactive （＋） and DCX＋ cells （48.0 ±3.1and 72.0 ± 3.8 cells/section, respectively） in the subgranular zone （SGZ） of the dentate gyrus （DG） and BrdU＊/NeuN＋ cells （17.0 ±1.5 cells/section） in the granule cell layer as well as in the SGZ. In addition, protein levels of BDNF and YrkB （about 232% and 244% of the vehicle-treated group, respectively） were significantly increased in the DG of the mice treated with 200 mg/kg ofG. littoralis extract. Conclusion： G. littoralis extract promots cell proliferation, neuroblast differentiation, and neuronal maturation in the hippocampal DG, and neurogenic effects might be closely related to increases ofBDN F and TrkB proteins by G. littoralis extract treatment.

目的开发一种能够自动分辨前列腺多序列MR图像的人工智能(AI)工具。方法回顾性分析2017年5月至2018年12月华中科技大学同济医学院附属同济医院前列腺多序列MR图像。前列腺多序列MR图像的分类由ResNet18卷积神经网络(CNN)模型来实现。运用深度残差网络提升训练精度和测试精度。所使用的数据集包括19 146张7个前列腺MR序列图像(横断面T1WI、横断面T2WI、冠状面T2WI、矢状面T2WI、横断面DWI、横断面ADC、横断面PWI),选取其中2 800张图像作为训练集,选取剩余图像中的388张图像作为测试集。采用准确度评价ResNet18 CNN模型的效能。结果7个前列腺MR序列(横断面DWI、冠状面T2WI、横断面灌注成像、矢状面T2WI、横断面ADC、横断面T1WI和横断面T2WI)图像测试准确率分别为100.0%(44/44)、77.5%(31/40)、96.7%(116/120)、100.0%(44/44)、100.0%(44/44)、100.0%(52/52)和100.0%(44/44)。横断面PWI的分类0.8%(1/120)被错误地分到了横断面T2WI序列,仅2.5%(3/120)错误地分到矢状面T2WI序列;对于冠状面T2WI的分类15.0%(6/40)被错误地分到了横断面T2WI序列,7.5%(3/40)错误地分到矢状面T2WI序列。结论开发的能够自动分辨前列腺多序列MR图像的AI工具准确率高。
