AIM: To compare the risk of developing advanced colorectal neoplasm(ACRN) according to age in ***: A total of 70428 Koreans from an occupational cohort who underwent a colonoscopy between 2003 and 2012 at Kangbuk Samsung Hospital were retrospectively selected. We evaluated and compared odds ratios(OR) for ACRN between the young-adults(YA family history of colorectal cancer(OR = 1.46, 95%CI: 1.01-2.10), diabetes mellitus related factors(OR = 1.27, 95%CI: 1.06-1.54), obesity(OR = 1.23, 95%CI: 1.03-1.47), CEA(OR = 1.04, 95%CI: 1.01-1.09) and low-density lipoprotein-cholesterol(OR = 1.01, 95%CI: 1.01-1.02) were related with an increased risk of ACRN. However, age(OR = 1.08, 95%CI: 1.06-1.09), male sex(OR = 2.12, 95%CI: 1.68-2.68), current smoking(OR = 1.38, 95%CI: 1.12-1.71), obesity(OR = 1.34, 95%CI: 1.09-1.65) and CEA(OR = 1.05, 95%CI: 1.01-1.09) also increased the risk of ACRN in the OA ***: The risks of ACRN differed based on age group. Different colonoscopic screening strategies are appropriate for particular subjects with risk factors for ACRN, even in subjects younger than 50 years.

This study was designed to evaluate the effects of testosterone supplementation(TS)on body composition in patients with HIV and the side effects of TS.A comprehensive literature search strategy was used to retrieve relevant randomized controlled trials(RCTs)examining the effects of TS on body composition.A total of 14 eligible studies were included,enrolling 388 and 349 randomized patients in TS and control groups,*** quality of studies included was assessed,and data on total body weight(BW),lean body mass(LBM),fat mass(FM),serum total testosterone(TT),free testosterone(FT)levels,and adverse events were extracted and analyzed using Review Manager software ***-analysis results showed that TS was associated with a small but significant modification in total BW,serum TT,and FT levels in HIV-infected patients and in patients given various drug *** also significantly increased LBM in male patients,but no significant difference in LBM was observed between female counterparts treated with TS or ***,TS relative to placebo did not lead to a significant reduction in *** significant difference was observed between the two groups in terms of adverse *** findings suggested that TS may be recommended to improve body composition in patients with HIV-related weight ***,owing to the high heterogeneity across included trials,further evaluations using large-scale,multi-center,blinded RCTs are needed.

Prostate cancer(PCa)growth and progression rely on the interaction between the androgen receptor(AR)and the testicular ligands,testosterone and dihydrotestosterone(DHT).Almost all men with advanced PCa receive androgen deprivation therapy(ADT).ADT lowers circulating testosterone levels,which impairs AR activation and leads to PCa ***,ADT is palliative and PCa recurs as castration-recurrent/resistant PCa(CRPC).One mechanism for PCa recurrence relies on intratumoral synthesis of DHT,which can be synthesized using the frontdoor or primary or secondary backdoor *** metabolism inhibitors,such as those targeting 5a-reductase,aldo-keto-reductase family member 3(AKR1C3),or cytochrome P45017A1(CYP17A1)have either failed or produced only modest clinical *** goal of this review is to describe the therapeutic potential of combined inhibition of 5a-reductase and 3a-oxidoreductase enzymes that facilitate the terminal steps of the frontdoor and primary and secondary backdoor pathways for DHT *** of the terminal steps of the androgen metabolism pathways may be a way to overcome the shortcomings of existing androgen metabolism inhibitors and thereby delay PCa recurrence during ADT or enhance the response of CRPC to androgen axis manipulation.

Objective:Recent studies have reported the underuse of active surveillance or watchful wait-ing for low-risk prostate cancer in the United *** study examined prostate cancer-specific and all-cause death in elderly patients older than 75 years with low-risk tumors managed with active treatment versus watchful waiting with active surveillance(WWAS).Methods:We performed survival analysis in a cohort of 18,599 men with low-risk tumors(early and localized tumors)who were 75 years or older at the time of prostate cancer diagnosis in the linked Surveillance,Epidemiology,and End Results(SEER)-Medicare database(from 1992 to 1998)and who were followed up through December *** was defined as having an-nual screening for prostate-specific antigen and/or digital rectal examination during the follow-up *** risks of prostate cancer-specific and all-cause death were compared by Cox regression *** propensity score matching technique was used to address potential selection ***:In patients with well-differentiated(Gleason score 2-4)and localized disease,those managed with WWAS without delayed treatment had higher risk of all-cause death(hazard ratio 1.20,95%confidence interval 1.13-1.28)but a substantially lower risk of prostate cancer-specific death(hazard ratio 0.62,confidence interval 0.51-0.75)than patients undergoing active *** managed with WWAS with delayed treatment had comparable mortality ***-tivity analyses based on propensity score matching yielded similar ***:In men older than 75 years with well-differentiated and localized prostate cancer,WWAS without delayed treatment had a lower risk of prostate cancer-specific death and compa-rable all-cause death as compared with active *** patients in whom treatment was delayed had comparable mortality *** results support WWAS as an initial management option for older men with well-differentiated and localized prostate cancer.

Plexins and semaphorins are a large family of proteins that are involved in cell movement and *** importance of plexins and semaphorins has been emphasized by their discovery in many organ systems including the nervous(Nkyimbeng-Takwi and Chapoval,2011;McCormick and Leipzig,2012;Yaron and Sprinzak,2012),epithelial(Miao et al.,1999;Fujii et al.,2002),and immune systems(Takamatsu and Kumanogoh,2012)as well as diverse cell processes including angiogenesis(Serini et al.,2009;Sakurai et al.,2012),embryogenesis(Perala et al.,2012),and cancer(Potiron et al.,2009;Micucci et al.,2010).Plexins and semaphorins are transmembrane proteins that share a conserved extracellular semaphorin do-main(Hota and Buck,2012).The plexins and sema-phorins are divided into four and eight subfamilies respectively based on their structural *** are relatively small proteins containing the extracellular semaphorin domain and short intra-cellular *** contain the semaphorin domain and long intracellular tails(Hota and Buck,2012).The majority of plexin and semaphorin research has fo-cused on the nervous system,particularly the devel-oping nervous system,where these proteins are found to mediate many common neuronal cell processes including cell movement,cytoskeletal rearrangement,and signal transduction(Choi et al.,2008;Takamatsu et al.,2010).Their roles in the immune system are the focus of this review.

Motivations and Training Needs of General Practitioner Preceptors

Background：General practitioner （GP） preceptors play an important role in the cultivation of *** problems exist in the training of GP *** study aimed to explore the willingness and training needs of GP preceptors and compare the differences between preceptors from general practice and other ***：A total of 375 questionnaire forms were sent to 375 GP preceptors from 11 different provinces,and 344 completed forms were *** main outcome included general information,teaching motivations,and training needs of GP ***：The study showed that about 89.2% of GP preceptors were willing to be *** majority of respondents strongly agreed that the motivation for becoming a GP supervisor was to learn from *** most important capability they should master was clinical teaching （92.2%）,followed by lecture （83.1%） and doctor-patient communication （83.1%）.The top three preferred methods of GP preceptors training were case discussion （78.8%）,workshop （57.6%）,and classroom teaching （56.4%）.The domains in which most GP preceptors wanted to acquire knowledge and skill were mental health （59.3%）,rehabilitation （47.1%）,pediatrics （41.0%）,and obstetrics （37.5%）.No significant differences were found in the willingness to train GPs （x2 =3.34,P 〉 0.05） and whether they would become or continue to become a GP supervisor after the training （x2 =l.106,P 〉 0.05）.Conclusions：Although most preceptors were under on-the-job training,they were glad to train *** be qualified,preceptors should be trained according to the actual needs of GP preceptors.
