甲状腺机能亢进(以下简称“甲亢”)为儿童及青少年罕见疾病,其发病率随年龄增长而增高。其最常见的原因为自体免疫性甲状腺病,包括Basedow病及自体免疫性甲状腺炎。在Basedow病及自体免疫性甲状腺炎均可证实有不同的HLA-单型发生率增高,但其免疫机制各不相同,而缺乏抑制抗甲状腺组织抗体产生的能力则是共同的。精神、社会因素及环境影响对疾病的发生亦有重要作用。

目的　研究具有相同起源而转移能力不同的小鼠黑色素瘤细胞系 (B16、B16F10、B16BL6 )的某些生物学特性与其侵袭潜能的相关性。方法　用重组基质膜实验考察细胞的侵袭能力 ;用分光光度法测定黑色素瘤细胞的黑色素含量 ;利用细胞运动记录分析系统研究细胞在三维胶原基质中的运动状态 ;用明胶底物酶谱法分析细胞分泌Ⅳ型胶原酶的能力 ;用端粒重复扩增 (TRAP) PCR法测定细胞的端粒酶活性。结果　B16BL6和B16F10具有较高的侵袭能力 ,其运动能力及分泌Ⅳ型胶原酶的能力亦较强 ,但B16F10的黑色素含量却较低 ,端粒酶活性在三者之间无明显差异。结论在所研究的小鼠黑色素瘤不同亚系的某些生物学特性中 ,侵袭能力与细胞运动能力、分泌Ⅳ型胶原酶的能力之间有较好的相关性 ,而与黑色素含量及端粒酶活性无直接相关性。

The colon is an unusual site of intestinal atresia. Colonic atresia is subdivided into three phenotypes. Type III is the most common phenotype, where the proximal and distal blind sacs are not connected. Here, we report on the presence of colonic atresia with an “inverse apple-peel” appearance in a neonate with gastroschisis. The lack of mesenteric fixation of the entire small intestine, including the proximal colon, and the twisting around the vascular axis of the superior mesenteric artery led to intrauterine volvulus and hemorrhagic infarction of the ileocolic bowel at 34 weeks of gestation. According to the current nomenclature for small bowel atresia, we introduce type IIIB into the current colonic atresia classification. The occurrence of type IIIB has been mentioned in the literature, but no single cases have been reported until now. Patients with this type of atresia are predisposed to the loss of the ileocecal region.

Background: Blood pH and bicarbonate estimations are basal acid-base laboratory tests that are performed in infants with infantile hypertrophic pyloric stenosis (IHPS). This study aimed to define the clinical value of pCO2 and BE in infants suspected to have IHPS. Methods: We collected data from 80 “surgical” infants younger than 100 days with prolonged nonbilious vomiting who were suspected to have IHPS. In 65 infants, pyloric stenosis was confirmed, and 15 infants had nonsurgical conditions. Capillary blood was tested for standard acid-base parameters and lactate. The two groups were compared. Results: Eighty-eight percent of the IHPS infants had elevated standard bicarbonate levels (st bicarb) > 25 mmol/l, and 60% had BE > 3.5 mmol/l;12% of the infants showed hypercapnia (pCO2 ≥ 50 mmHg) associated with markedly increased standard bicarbonate and BE. Infants with nonsurgical vomiting were older at admission (p = 0.002), had a longer duration of vomiting (p 2 (p = 0.021), lower st bicarb (p < 0.001) and lower BE (p = 0.001). In addition, nonsurgical infants showed a trend to anemia (p = 0.002). Conclusions: In infants with IHPS/nonbilious vomiting, acid-base analysis (ABA) is equivocal or inconclusive. These findings may be misleading and could result in a false clinical decision. Nonsurgical vomiting is associated with a lower degree of alkalosis, normocapnia to slight hypercapnia and a base deficit. However, even infants with IHPS may present with a negative BE. In infants with IHPS and severe alkalosis, hypercapnia carries a risk for respiratory depression. Monitoring the infant’s respiration allows for the early detection of respiratory deterioration.

Background: Psychosocial factors attract interest in investigating the occurrence of cardiovascular disease. Design This study aimed to examine the impact of critical life events on the development of myocardial infarction in smokers and ex-smokers. We hypothesized that critical life events increase the risk of the disease. Methods: Data were taken from the Cologne Smoking Study (CoSmoS), a retrospective multicentre case-control study that examines which psychosocial factors may lead to a higher risk for smokers and ex-smokers of suffering from a myocardial infarction. Our sample consisted of n = 278 myocardial infarction participants and control participants. Both groups had a history of smoking. Logistic regression was used in the analysis. Results: The study results of the smoking and ex-smoking participants showed that sociodemographic data like gender and age have an effect on the development of myocardial infarction. Physical activity seems to offer protection aganist myocardial infarction. Final, the unexpected result that the experience of at least one critical life event seems to have a positive effect on health and so lowers the risk of myocardial infarction. Conclusions: Socio-demographic data and physical activity have an effect on the development of myocardial infarction. Participants with experience of critical life events appear to be strengthened after the events and possess adequate resources to protect their health.

Cutaneous leiomyosarcomas (LMS) originating from smooth muscle of the skin are rare malignant neoplasms. Preferential sites are hair-bearing areas, especially the legs. We report two patients in their 5th and 7th decade, in whom leiomyosarcomas were encountered in the lower leg and the capillitium, respectively. Histologically, the former was found to be a dermal leiomyosarcoma with subcutaneous infiltration, the latter an exclusively dermal tumor. The LMS of the lower leg had been excised three years previously in a different institution and had been diagnosed as a leiomyoma. Both lesions were excised with adequate safety margins in our institution and within a two-year postoperative observation period no recurrences were encountered. Dermal leiomyosarcomas have a 33% recurrence rate while that of subcutaneous leiomyosarcomas show is 50% . In contrast to exclusively dermal tumors, subcutaneous LMS show a tendency for hematogenic and lymphogenic metastases. Therapy of choice is excision with wide safety margins.

AIM To systematically review reports on deceased-donor-lobar lung transplantation(dd LLTx) and uniformly describe sizematching using the donor-to-recipient predicted-total lung-capacity(pT LC) ratio. METHODS We set out to systematically review reports on ddL LTx and uniformly describe size matching using the donorto-recipient pT LC ratio and to summarize reported oneyear survival data of ddL LTx and conventional-LTx. We searched in Pub Med, CINAHL via EBSCO, Cochrane Database of Systematic Reviews via Wiley(CDSR),Database of Abstracts of Reviews of Effects via Wiley(DARE), Cochrane Central Register of Controlled Trials via Wiley(CENTRAL), Scopus(which includes EMBASE abstracts), and Web of Science for original reports on ddL LTx. RESULTS Nine observational cohort studies reporting on 301 ddL LTx met our inclusion criteria for systematic review of size matching, and eight for describing one-year-survival. The dd LLTx-group was often characterized by high acuity;however there was heterogeneity in transplant indications and pre-operative characteristics between studies. Data to calculate the pT LC ratio was available for 242 ddL LTx(80%). The mean pT LCratio before lobar resection was1.25 ± 0.3 and the transplanted pT LCratio after lobar resection was 0.76 ± 0.2. One-year survival in the ddL LTxgroup ranged from 50%-100%, compared to 72%-88%in the conventional-LTx group. In the largest study ddL LTx(n = 138) was associated with a lower one-year-survival compared to conventional-LTx(n = 539)(65.1% vs84.1%, P < 0.001). CONCLUSION Further investigations of optimal donor-to-recipient size matching parameters for ddL LTx could improve outcomes of this important surgical option.

AIM: To investigate a new mother-baby system, consisting of a peroral cholangioscope and a duodenoscope in patients regarding its feasibility. METHODS: In the study period from January 2007 to February 2010, 76 consecutive patients(33 men, 43 women; mean age 63 years old) were included in this pilot series. Endoluminal images and biopsies were obtained from 55 patients with indeterminate strictures, while 21 patients had fixed filling defects. The diagnostic accuracy of peroral cholangioscopy(POCS) in the visualization of strictures and tissue sampling was evaluated, and therapeutic success was monitored. Followup was performed over at least 9 mo. RESULTS: A total of 55 patients had indeterminate strictures. Using the criteria "circular stenosis" and "irregular surface or margins", POCS correctly described 27 out of 28 malignant biliary strictures and 25 out of 27 benign lesions(sensitivity, 96.4%; specificity, 92.6%, diagnostic accuracy 94.5%). Visually targeted forceps biopsies were performed in 55 patients. Tissue sampling during POCS revealed malignancy in 18 of 28 cases(sensitivity: 64.3%). In 21 patients with fixed filling defects, 10 patients with bile duct stones were successfully treated with conventional stone removal. Nine patients with difficult stones(5 giant stones and 4 intrahepatic stones) were treated with visually guided laser lithotripsy. Two patients in the group with unclear fixed filling defects had bile duct adenoma or papillary tumors and were surgically treated. CONCLUSION: The new 95 cm POCS allows for accurate discrimination of strictures and fixed filling defects in the biliary tree, provides improved sensitivity of endoscopically guided biopsies and permits therapeutic approaches for difficult intrahepatic stones.
