AIM:To elucidate etiologic associations between Helicobacter pylori(H pylori),lifestyle,environmental factors and gastric cardiac adenocarcinoma(GCA)among men. METHODS:A hospital-based case-control study was conducted in Taiwan from 2000 to *** cases were newly confirmed as primary *** controls were selected matching with age,sex,and admission date to each *** were informedof potential risk factors with a structured question- naire by trained interviewers during hospitalization and provided a blood sample for the determination of H pylori *** ratio(OR)and 95%confidence interval(95%CI)were used to evaluate risk,and a multivariate conditional logistic regression model was performed. RESULTS:All participants recruited for this study were men,consisting of 41 cases and 205 *** of the univariate analysis showed that significant factors associated with the etiology of GCA included H pylori(OR =2.69,95%CI=1.30-5.53),cigarette smoking(OR= 2.28,95%CI=1.05-4.96),working or exercising after meals(OR=3.26,95%CI=1.31-8.11),salted food (OR=2.51,95%CI=1.08-6.11),fresh vegetables(OR =0.28,95%CI=0.09-0.80),fruits(OR=0.19,95% CI=0.04-0.89),and rice as principal food(OR=0.53, 95%CI=0.30-0.85).Multivariate conditional logistic regression models indicated that a significantly elevated risk of contracting GCA was associated with working or exercising after meals(OR=3.18,95%CI=1.23-9.36) and H pylori infection(OR=2.93,95%CI=1.42-6.01). In contrast,the consumption of fresh vegetables(OR =0.22,95%CI=0.06-0.83),fruits(OR=0.28,95% CI=0.09-0.79)and rice as principal food(OR=0.48, 95%CI=0.24-0.93)remained as significant beneficial factor associated with GCA. CONCLUSION:Working or exercising after meals and H pylori infection increase the risk of GCA,but higher intakes of rice,fresh vegetables and fruits reduce the risk.

中亚干旱区是丝绸之路的核心区,也是史前人类扩散和东西方文化交流的重要通道.该区气候干旱,植被和生态群落受制于水文因素.由于缺乏精确年代控制的高分辨率古气候记录,中亚地区全新世水文变化及极端干旱事件在史前人口迁移和跨欧亚大陆文化交流中扮演的角色还不清楚.本文基于来自吉尔吉斯斯坦的石笋多指标记录,重建了中亚干旱区目前年代最精确(测年误差~6‰)、分辨率最高(~3年)、涵盖中晚全新世的降水变化序列.结果显示在5820~5180 a BP期间,该区存在一次持续时间为640年的超级干旱事件,主要受西风带北移的影响.这次干旱事件对中亚史前文化的发展产生了重要影响,很可能阻碍了史前东西方文化沿中亚绿洲路线上的相向扩散,使得最早的跨欧亚大陆文化交流在5000~4000 a BP沿着北方草原路线开展.随着大干旱后降水的逐渐增加和生态环境的恢复,中亚绿洲和河谷中增加的定居人口促进了不同绿洲之间的相互联系,进一步在4000~3000 a BP开辟了跨大陆文化交流的史前丝绸之路,为历史时期丝绸之路的最终开通奠定了基础.

自2008年至今,有大量关于慢性HBV感染的自然史和治疗的最新数据不断涌现。其中包括慢性HBV感染的无症状感染者,以社区为基础的队列研究,HBV基因型的作用,非药物诱导的自然HBV变异型毒株,无创性肝纤维化评估方法的应用,HBsAg定量在临床中的应用,更有效的新治疗药物和新治疗方案等等。来自亚太地区的专家审查和评估了相关数据,并共同商讨了近年来报道的最有意义的发现,基于此,对2008年版的亚太地区慢性乙型肝炎治疗共识进行修订,同时对2008年版治疗指南定义的关键词组进行了修订。修订后的指南包括以下几方面内容:一般治疗,肝纤维化评价适应证,何时开始治疗或停药,初始抗病毒治疗药物的选择,如何监测治疗中和治疗后的患者。关于特殊人群的治疗建议中包括了对妊娠妇女,已发生耐药,合并其他病毒感染,肝功能失代偿,接受免疫抑制治疗、化疗,肝移植或肝细胞癌患者的具体治疗建议。

T-CELL SUBSETS OF PERIPHERAL BLOOD AND SPLEEN IN HEPATOCARCINOMA-BEARING RATS

Peripheral blood OX-19+ cells (pan T-cells) and W3/25+ cells (Th cells) decreased with tumor development in tumor-bearing rats giving significant differences when compared with healthy rats. No significant changes were found in OX-8+ cells (Ts cells). Hence Th Ts ratio in tumor-bearing rats was lower than that in control group. Meanwhile on day 17 following tumor implantation in tumor-bearing rats with intrahepatic or peritoneal metastasis there were less W3/25+ cells and more OX-8+ cells than those of hepatoma-bearing rats without metastasis (p<0.01). In the spleen the phenotypes of lymphocyte markers showed less OX-8+ cells and more OX-12+ cells (B cells) tumor-bearing rats. It was suggested that in tumor-bearing host not only was T-cell immune function inhibited but also there was abnormal distribution of T-cell subsets. It seems that there is a close relationship between tumor growth and its spread and a lower Th/Ts ratio.

The robotic surgical system has been applied to various types of pancreatic surgery. However, controversies exist regarding a variety of factors including the safety, feasibility, efficacy, and cost-effectiveness of robotic surgery. This study aimed to evaluate the current status of robotic pancreatic surgery and put forth experts' consensus and recommendations to promote its development. Based on the WHO Handbook for Guideline Development, a Consensus Steering Group* and a Consensus Development Group were established to determine the topics, prepare evidence-based documents, and generate recommendations. The GRADE Grid method and Delphi vote were used to formulate the recommendations. A total of 19 topics were analyzed. The first 16 recommendations were generated by GRADE using an evidence-based method (EBM) and focused on the safety, feasibility, indication, techniques, certification of the robotic surgeon, and cost-effectiveness of robotic pancreatic surgery. The remaining three recommendations were based on literature review and expert panel opinion due to insufficient EBM results. Since the current amount of;evidence was low/meager as evaluated by the GRADE method, further randomized controlled trials (RCTs) are needed in the future to validate these recommendations.

Objective: The aim of this study is to investigate the current status of pancreatic cancer patients undoing surgical treatment in China and to find ways to improve the survival of these patients in the future. Methods: This study is a national, multicenter, cross-sectional study in China. Information regarding pancreatic cancer patients undergoing surgical treatment from 34 high-volume tertiary IIIA level hospitals was collected and analyzed from the March 1, 2016 to the February 28, 2017. Results: In total, 2200 pancreatic cancer patients were enrolled from 34 tertiary IIIA level hospitals in 16 provinces across China. The male-to-female ratio was 1.5. More than 80% of the patients were between 50 and 70 years old. The top 4 symptoms were epigastric discomfort, abdominal pain, jaundice, and weight loss. Carbohydrate antigen 19-9 and carcinoembryonic antigen were elevated in 70.9% and 27.1% of patients, respectively. A multidisciplinary team (MDT) discussion was carried out for 35.0% of patients before surgery. The proportion of minimally invasive pancreatic surgeries was approximately 20%. A total of 83.4% of the operations achieved R0 resection, and the incidence of grade 3/4 postoperative complications was 7.7%. Only 13.4% of the patients received postoperative adjuvant chemotherapy. The percentage of pathological stage I tumors was only 24.5%. Conclusion: The majority of pancreatic cancer patients undergoing surgical resection in China are in an advanced stage. The MDT consultations for pancreatic cancer have not been widely carried out. R0 resection has been achieved in most cases, with relatively low incidence of serious complications, but minimally invasive pancreatic surgery should be further promoted. The application of postoperative chemotherapy remains low. OBJECTIVE:To investigate the safety profiles of Motherwort injection(MI).METHODS:A multi-center,prospective and drugderived hospital intensive monitoring method was conducted to assess the safety of MI in real world *** study was based on a very large population after the injection was approved and marketed in *** patients using the injection in participating hospitals were monitored to determine the incidence,pattern,severity and outcome of associated adverse ***:The post-marketing surveillance was performed in 10 094 female patients from April to December,*** incidence of adverse drug reactions(ADRs) was 0.79‰(8/10 094).Among the 8 patients,the reported adverse events mainly included systemic abnormalities,such as fever,chills and eyelid edema;skin and appendages disorders,such as pruritus and rash;gastrointestinal disorders,such as nausea,abdominal distension and pain;heart rate and rhythm disorders,such as palpitation and increased heart *** of these ADRs were mild in ***:In this study the ADRs incidence rate of MI is very low,which supports that it is generally safe for use in obstetric and gynecological ***,the total number of 8 ADRs recorded over a relatively short time span seems limited,and the low number of reports could not represent an absolute guarantee of safety.
