Prevalence and factors associated with vitamin C deficiency in inflammatory bowel disease

BACKGROUND Patients with inflammatory bowel disease(IBD) are prone to several nutritional deficiencies. However, data are lacking on vitamin C deficiency in Crohn’s disease(CD) and ulcerative colitis(UC) patients, as well as the impact of clinical, biomarker and endoscopic disease severity on the development of vitamin C *** To determine proportions and factors associated with vitamin C deficiency in CD and UC *** In this retrospective study, we obtained clinical, laboratory and endoscopic data from CD and UC patients presenting to the IBD clinic at a single tertiary care center from 2014 to 2019. All patients had an available plasma vitamin C level. Of 353 subjects who met initial search criteria using a cohort discovery tool, 301ultimately met criteria for inclusion in the study. The primary aim described vitamin C deficiency(≤ 11.4 μmol/L) rates in IBD. Secondary analyses compared proportions with deficiency between active and inactive IBD. Multivariate logistic regression analysis evaluated factors associated with *** Of 301 IBD patients, 21.6% had deficiency, including 24.4% of CD patients and 16.0% of UC patients. Patients with elevated C-reactive protein(CRP)(39.1% vs 16.9%, P < 0.001) and fecal calprotectin(50.0% vs 20.0%, P = 0.009) had significantly higher proportions of deficiency compared to those without. Penetrating disease(P = 0.03),obesity(P = 0.02) and current biologic use(P = 0.006) were also associated with deficiency on univariate analysis. On multivariate analysis, the objective inflammatory marker utilized for analysis(elevated CRP) was the only factor associated with deficiency(odds ratio = 3.1, 95%confidence interval: 1.5-6.6, P = 0.003). The role of estradiol in male reproductive function

Traditionally, testosterone and estrogen have been considered to be male and female sex hormones, respectively. However, estradiol, the predominant form of estrogen, also plays a critical role in male sexual function. Estradiol in men is essential for modulating libido, erectile function, and spermatogenesis. Estrogen receptors, as well as aromatase, the enzyme that converts testosterone to estrogen, are abundant in brain, penis, and testis, organs important for sexual function. In the brain, estradiol synthesis is increased in areas related to sexual arousal. In addition, in the penis, estrogen receptors are found throughout the corpus cavernosum with high concentration around neurovascular bundles. Low testosterone and elevated estrogen increase the incidence of erectile dysfunction independently of one another. In the testes, spermatogenesis is modulated at every level by estrogen, starting with the hypothalamus-pituitary-gonadal axis, followed by the Leydig, Sertoli, and germ cells, and finishing with the ductal epithelium, epididymis, and mature sperm. Regulation of testicular cells by estradiol shows both an inhibitory and a stimulatory influence, indicating an intricate symphony of dose-dependent and temporally sensitive modulation. Our goal in this review is to elucidate the overall contribution of estradiol to male sexual function by looking at the hormone＇s effects on erectile function, spermatogenesis, and libido.

Cytomegalovirus(CMV)infection of the gastrointestinal (GI)tract has been reported in both immunocompetent and,more frequently,in immunocompromised *** describe a case of a 19-year-old male who developed CMV infection of the terminal ileum while receiving immunosuppression for lupus nephritis. This was a distinctly unusual site of infection which clinically mimicked Crohn's *** note that reports of terminal ileal CMV infection have been infrequent. Despite a complicated hospital course,ganciclovir therapy was effective in resolving his symptoms and normalizing his ileal *** report highlights the importance of accurate histological diagnosis and clinical follow-up of lupus patients with GI symptoms undergoing intense immunosuppression.

Microsurgical varicocelectomy： a review

Varicocelectomy is the most commonly performed surgical procedure for the treatment of male infertility. Although several different techniques for varicocele repair have been described in the literature, microsurgical varicocelectomy performed through a subinguinal or inguinal incision is recognized as the gold-standard approach for varicocelectomy, due to high success rates with minimal complications. Standard indications for varicocelectomy include palpable varicocele（s）, with one or more abnormal semen parameters, and, for the couple trying to conceive, in the setting of normal or correctable female infertility. However, varicocele repair is often recommended and undertaken for reasons other than infertility, including low serum testosterone, testicular pain, testicular hypotrophy and poor sperm DNA quality. This article reviews the technical aspects of microsurgical varicocelectomy, and its indications in adults and adolescents.

Can inhibition of telomerase increase pancreatic cancer cell's susceptibility to chemotherapeutic reagents?

Objectives: To clarify the inhibition of pancreatic can-cer cells by interference with the hTR component of thetelomerase reverse transcriptase enzymatic complexand evaluate susceptibility of antisense hTR pancreaticcancer cells to chemotherapeutic ***: A 593 bp of full length hTR cDNA was sub-cloned into a mammalian expression vector pcDNA3.1(-) in antisense orientation to construct an antisensehTR expression plasmid. The plasmids were introducedinto Pancl cells, a human pancreatic carcinoma cellline, by lipofectin, and G418-resistant stable trans-formants were expanded. Resulting stable clones werescreened for the presence of hTR insert by PCR withT7 and BGH reverse primers located on the flanks ofthe multiclonal site of pcDNA3.1 vector. Cell growth rate,hTR expression, telomerase activity, and anchorage-in-dependent growth property were analyzed. Finally, sus-ceptibility of antisense hTR cells to chemotherapeuticreagents was ***: Significant downregulation of endogenous hTRwas evident in the antisense-hTR transformed cells,and telomerase activity was markedly decreased com-pared to control cells in standard TRAP assays. Fur-thermore, the proliferation and the anchorage-inde-pendent growth ability in antisense-hTR expressingcells were significantly decreased compared with thecontrol parental cells. However, no crisis or senescencephenomena was observed. Antisense hTR appears toincrease Pancl cell's susceptibility to chemotherapeuticreagent cDDP, but not to differentiation reagent DM-SO, COX2 inhibitor sulinbac, NS-398, curcumin, andchemotherapeutic reagent adriamycin(ADM).Conclusions: These data indicate that hTR is probablya critical component of human telomerase activity andthat downregulation of the RNA component of humantelomerase is an effective target for anticancer strategyand antisense hTR can increase Pancl cell's susceptibilityto cDDP.

As the prevalence of diabetic retinopathy continues to be on the rise, the Chronic Care Model (CCM) offers a transformative, patient-focused approach for efficient diabetic retinopathy care, emphasizing the need for urgent and innovative strategies in the United States. The model integrates community resources, healthcare organizations, self-management support, delivery system design, decision support, and clinical information systems. Addressing challenges and solutions, the model emphasizes proactive and preventive measures, collaborative multidisciplinary care, technological integration, and overcoming resistance to change. This paper proposes the utilization of the Chronic Care Model (CCM) as a possible public health framework for comprehensive management of diabetic retinopathy in the United States. Implementing the CCM offers a comprehensive approach to diabetic retinopathy care, addressing both individual and systemic factors, essential for improving public health outcomes.
