Surgery in (pre)malignant celiac disease

AIM: To report the outcome of surgery in patients with(pre)malignant conditions of celiac disease(CD) and the impact on ***: A total of 40 patients with(pre)malignant conditions of CD,ulcerative jejunitis(n = 5) and enteropathy associated T-cell lymphoma(EATL)(n = 35),who underwent surgery between 2002 and 2013 were retrospectively evaluated. Data on indications,operative procedure,post-operative morbidity and mortali ty,adjuvant therapy and overal l survival(OS) were collected. Eleven patients with EATL who underwent chemotherapy without resection were included as a control group for survival analysis. Patients were followed-up every three months during the first year and at 6-mo intervals ***: Mean age at resection was 62 years. The majority of patients(63%) underwent elective laparotomy. Functional stenosis(n = 1 3) and perforation(n = 12) were the major indications for surgery. In 70% of patients radical resection wasperformed. Early postoperative complications,mainly due to leakage or sepsis,occurred in 14/40(35%) of patients. Eight patients required reoperation. More patients who underwent resection in the acute setting(n = 3,20%) died compared to patients treated in the elective setting. With a median follow-up of 20 mo,seven patients(18%) required reoperation due to long-term complications. Significantly more patients who underwent acute surgery could not be treated with adjuvant chemotherapy. Patients who first underwent surgical resection showed significantly better OS than patients who received chemotherapy without ***: Although the complication rate is high,the preferred first step of treatment in(pre)malignant CD consists of local resection as early as possible to improve survival.

We are defined by sexual dimorphism. Male and female, the sexes we are （and are perceived to be）, affect how we feel, how we behave and how we interact with others. We look around and see all complex creatures, including ourselves, as either male or female. This is so much a part of our daily existence that it reinforces the idea of man- hood and womanhood as expressions of a natural order. We regard this division into two sexes as one of the great eternal verities and tend to experience the difference between the sexes as rather absolute. Subjectively, man and woman are seen as opposites, poles apart and mutually exclusive forms of human existence.

A Synoptic Overview and Moisture Tra jectory Analysis of the“7.21” Heavy Rainfall Event in Beijing

The heavy rainfall in Beijing on 21 July 2012 was a high impact event. First, a synoptic overview of this event is presented based on the ECMWF reanalysis and forecast data, together with observations of hourly precipitation. It is found that an east to northeastward moving ＂low-level northwest vortex＂ caused this event. The vortex was formed under favorable circulation patterns and intensified by low-level dry intrusions. The source regions of the large volume of moisture necessary to sustain the intense rainfall are diagnosed by back-trajectory analysis. Approximately 77% of the moisture was transported from the Bay of Bengal （BOB）. The transport processes, including vertical profiles, mean humidity variations, and relative importance of the rain-paths, are further quantified by the back-trajectory analysis and cluster analysis. The results highlight the importance of the southwestern path in transporting moisture to northern China, which accounts for 88.4% of the moisture from the BoB, and 68% of the total of the ＂7.21＂ heavy rainfall event.

Low dose oral haloperidol does not prolong QTc interval in older acutely hospitalised adults： a subanalysis of a randomised double-blind placebo-controlled study

Background Haloperidol is the most frequently prescribed antipsycbotic for delirium symptoms. The risk of QTc prolongation often raises concerns, although the effect of haloperidol on QTc interval has not yet been investigated in a randomised placebo-controlled fixed-dose study. Methods A subanalysis of a randomised double-blind placebo-controlled study was conducted to evaluate the effect of prophylactic haloperidol 1 mg or placebo 1 mg orally twice-daily （maximum of 14 doses） on QTc interval in patients aged 70 years and over. Bedside, 12-lead ECGs were recorded before, during and after the one-week intervention period. Automatic QTc measurements were ob- tained in addition to manual measurements of QT and RR intervals, blinded for treatment status. Manual measurements were corrected （QTc） using Bazett （QTc-B）, Framingham （QTc-Fa）, Fridericia （QTc-Fi） and Hodges （QTc-H） methods. Mixed model analyses were used to test for differences in longitudinal course of QTc between patients receiving haloperidol and placebo. Results ECG recordings of 72 patients （haloperidol n = 38） were analysed, 45.8% male. Median （range） haloperidol serum concentration on day 4 was 0.71 （0.32-1.82） μg/L （n = 23）. Longitudinal course of mean QTc did not significantly differ between treatment arms for any of the automatic or manually derived QTc values. Conclusions Low dose oral haloperidol did not result in QTc prolongation in older acutely hospitalised patients. Results may not be generalizable to patients with existing ECG abnormalities such as atrial fibrillation.

Molecular mechanisms of heavy metal tolerance and evolution in invertebrates

Following the genomics revolution, our knowledge of the molecular mechanisms underlying defenses against stress has been greatly expanded. Under strong selective pressure many animals may evolve an enhanced stress tolerance. This can be achieved by altering the structure of proteins （through mutations in the coding regions of genes） or by altering the amount of protein （through changes in transcriptional regulation）. The latter type of evolution can be achieved by substitutions in the promoter of the gene of interest （cisregulatory change） or by altering the structure or amount of transcriptional regulator proteins （trans-regulatory change）. The metallothionein system is one of the best studied stress response systems in the context of heavy metals. Metallothionein expression is assumed to be regulated by metal transcription factor 1 （MTF-1）; however, up to now the involvement of MTF- 1 has only been proven for some vertebrates and Drosophila. Data on invertebrates such as nematodes and earthworms suggest that other mechanisms of metallothionein induction may be present. A detailed study of Cd tolerance was done for a species of soilliving springtail, Orchesella cincta. The metallothionein gene of this species is overexpressed in metal-exposed field populations. Analysis of the metallothionein promoter has demon- strated extensive polymorphisms that have a fimctional significance, as shown in bioreporter assays. In a study comparing 20 different populations, the frequency of a high-expresser promoter allele was positively correlated with the concentration of metals in soil, especially Cd. The springtail study shows that cis-regulatory change of genes involved in the cellular stress response may contribute to evolution of metal tolerance.

The thylakoid membrane inside chloroplasts hosts the light-dependent reactions of *** embedded protein complexes are responsible for light harvesting,excitation energy transfer,charge separation,and *** higher plants,when the illumination conditions vary,the membrane adapts its composition and nanoscale morphology,which is characterized by appressed and non-appressed regions known as grana and stroma lamellae,*** we investigate the nanophotonic regime of light propagation in chloroplasts of higher plants and identify novel mechanisms in the optical response of the thylakoid *** results indicate that the relative contributions of light scattering and absorption to the overall optical response of grana strongly depend on the concentration of the light-harvesting *** the pigment concentrations typically found in chloroplasts,the two mechanisms have comparable strengths,and their relative value can be tuned by variations in the protein composition or in the granal ***,we find that collective modes in ensembles of grana significantly increase light absorption at selected wavelengths,even in the presence of moderate biological *** variations in the granal separation or a large disorder can dismantle this collective *** propose that chloroplasts use this mechanism as a strategy against dangerously high illumination conditions,triggering a transition to low-absorbing *** conclude that the morphological separation of the thylakoid membrane in higher plants supports strong nanophotonic effects,which may be used by chloroplasts to regulate light *** adaptive self-organization capability is of interest as a model for novel bioinspired optical materials for artificial photosynthesis,imaging,and sensing.

Should cross-sex hormone treatment of transsexual subjects vary with ethnic group？

Guidelines for cross-sex hormone treatment of transsexual people have been developed, but no attention has been paid to the specifics of ethnic groups. South East （SE） Asian male-to-female （MtoF） transsexual people may be able to transition to the female sex with lower doses of estrogens/progestins than Caucasians thus reducing health risks. Female-to-male （FtoM） may virilize less profoundly with standard doses of androgens, but this is probably sufficient to pass acceptably as men in view of the less pronounced sex differences in physique in Asians compared with Caucasians. It is timely that studies in Asians are conducted to get a better insight into their specific needs and risks of cross-sex hormone treatment.

Scheduling Reclaimer Operations in the Stockyard to Minimize Makespan

This paper considers a reclaimer scheduling problem in which one has to collect bulk material from stockpiles in the quay in such a way that the time used is minimized. When reclaimers are allowed to work on the same stockpile simultaneously, a fully polynomial time approximation scheme（FPTAS） is designed. Further,we present a 2-approximation algorithm in the case that any stockpile can be handled by only one reclaimer at a time. When the number of reclaimers is two, we give a 3/2-approximation algorithm. Numerical experiments show that the algorithms perform much better than our worst case analysis guarantees.

Lensless diffractive imaging with ultra-broadbandtable-top sources: from infrared to extreme-ultravioletwavelengths

Lensless imaging is an approach to microscopy in which a high-resolution image of an object is reconstructed from one or more measured diffraction patterns,providing a solution in situations where the use of imaging optics is not ***,current lensless imaging methods are typically limited by the need for a light source with a narrow,stable and accurately known *** have developed a general approach to lensless imaging without spectral bandwidth limitations or sample *** use two time-delayed coherent light pulses and show that scanning the pulse-to-pulse time delay allows the reconstruction of diffraction-limited images for all the spectral components in the *** addition,we introduce an iterative phase retrieval algorithm that uses these spectrally resolved Fresnel diffraction patterns to obtain high-resolution images of complex extended *** demonstrate this two-pulse imaging method with octave-spanning visible light sources,in both transmission and reflection geometries,and with broadband extreme-ultraviolet radiation from a high-harmonic generation *** approach enables effective use of low-flux ultra-broadband sources,such as table-top high-harmonic generation systems,for high-resolution imaging.
