Awareness during emergence from anesthesia: Features and future research directions

The anesthesia awareness with recall(AAWR) phenomenon represents a complication of general anesthesia consisting of memorization of intraoperative events reported by the patient immediately after the end of surgery or at a variable distance from it. Approximately 20% of AAWR cases occur during emergence from anesthesia. Clinically, these unexpected experiences are often associated with distress especially due to a sense of paralysis. Indeed, although AAWR at the emergence has multiple causes, in the majority of cases the complication develops when the anesthesia plan is too early lightened at the end of anesthesia and there is a lack of use, or misuse, of neuromuscular monitoring with improper management of the neuromuscular block. Because the distress caused by the sense of paralysis represents an important predictor for the development of severe psychological complications, the knowledge of the phenomenon, and the possible strategies for its prophylaxis are aspects of considerable importance. Nevertheless, a limited percentage of episodes of AAWR cannot be prevented. This paradox holds also during the emergence phase of anesthesia which represents a very complex neurophysiological process with many aspects yet to be clarified.

The treatment of advective fluxes in high-order finite volume models is well established, but this is not the case for diffusive fluxes, due to the conflict between the discontinuous representation of the solution and the continuous structure of analytic solutions. In this paper, a derivative reconstruction approach is proposed in the context of spectral volume methods, for the approximation of diffusive fluxes, aiming at the reconciliation of this conflict. Two different reconstructions are used for advective and diffusive fluxes： the advective reconstruction makes use of the information contained in a spectral cell, and allows the formation of discontinuities at the spectral cells boundaries; the diffusive reconstruction makes use of the information contained in contiguous spectral cells, imposing the continuity of the reconstruction at the spectral cells boundaries. The method is demonstrated by a number of numerical experiments, including the solution of shallow-water equations, complemented with the advective-diffusive transport equation of a conservative substance, showing the promising abilities of the numerical scheme proposed.

Forex(foreign exchange)is a special financial market that entails both high risks and high profit opportunities for *** is also a very simple market since traders can profit by just predicting the direction of the exchange rate between two ***,incorrect predictions in Forex may cause much higher losses than in other typical financial *** direction prediction requirement makes the problem quite different from other typical time-series forecasting *** this work,we used a popular deep learning tool called“long short-term memory”(LSTM),which has been shown to be very effective in many time-series forecasting problems,to make direction predictions in *** utilized two different data sets—namely,macroeconomic data and technical indicator data—since in the financial world,fundamental and technical analysis are two main techniques,and they use those two data sets,*** proposed hybrid model,which combines two separate LSTMs corresponding to these two data sets,was found to be quite successful in experiments using real data.

Objective:To assess the role of debulking surgery in extensive hepatic alveolar echinococcosis(HAE).Methods:Five cases of extensive HAE undergoing debulking surgery with their long-term follow-ups were ***,a systematic review was carried ***:The average age of patients in the study was(50.8±17.1)years(range 36 to 77 years).Three cases were *** median follow-up period was 61 months(range 12 to 84 months).Left liver resections were performed in 2 cases and right hemihepatectomies were undertaken in 2 *** patients(cases 2 and 4)developed early surgical complications,while one of them(case 4)who suffered from the recurrent sub-hepatic abscess died at 12 months ***:This study highlights the importance of debulking surgery for the management of complex symptomatic HAE lesions in patients with no curative option available.

The wide diffusion of healthcare monitoring systems allows continuous patient to be remotely monitored and diagnosed by doctors. The problem of congestion, namely due to the uncontrolled increase of traffic with respect to the network capacity, is one of the most common phenomena affecting the reliability of transmission of information in any network. The aim of the paper is to build a realistic simulation environment for healthcare system including some of the main vital signs model, wireless sensor and mesh network protocols implementation. The simulator environment is an efficient mean to analyze and evaluate in a realistic scenario the healthcare system performance in terms of reliability and efficiency.

This paper investigates the Italian firms＇ disclosure in response to the new guidance on the management commentary （MC） contained in the International Financial Reporting Standard （IFRS） practice statement issued by International Accounting Standards Board （IASB） in 2010. The study is organized as follows. After reviewing the relevant literature on financial disclosure and MC, this paper examines the content of the Italian management＇s report-----known as ＂Relazione sulla Gestione＂ and that of the recent IASB＇s IFRS practice statement. Hence, it applies a self-constructed disclosure index and a multiple correspondence analysis （MCA） on MC of a sample of 66 Italian non-financial listed firms. The results show that the level of disclosures provided by the Italian listed firms does not seem to be affected by the IASB＇s guidelines. However, the survey highlights large differences in the level and type of disclosure provided in MC among the sectors and firms. This paper aims to contribute to the financial reporting debate by understanding the IASB＇s guidelines and their impacts on the voluntary disclosure practices of Italian listed firms.

Non-alcoholic fatty liver disease (NAFLD) comprehends a wide range of conditions, encompassing from fatty liver or steatohepatitis with or without fibrosis, to cirrhosis and its complications. NAFLD has become the most common form of liver disease in childhood as its prevalence has more than doubled over the past 20 years, paralleling the increased prevalence of childhood obesity. It currently affects between 3% and 11% of the pediatric population reaching the rate of 46% among overweight and obese children and adolescents. The prevalence of hepatic steatosis varies among different ethnic groups. The ethnic group with the highest prevalence is the Hispanic one followed by the Caucasian and the African-American. This evidence suggests that there is a strong genetic background in the predisposition to fatty liver. In fact, since 2008 several common gene variants have been implicated in the pathogenesis of fatty liver disease. The most important is probably the patatin like phospholipase containing domain 3 gene (PNPLA3) discovered by the Hobbs’ group in 2008. This article reviews the current knowledge regarding the role of ethnicity and genetics in pathogenesis of pediatric fatty liver.

The present work aims to conduct a critical analysis of control systems and tools of performance assessment in local public services companies, with a particular reference to the public transport sector. The interest in this issue arises for various reasons, from the growing economic significance of the sector at a national level, measurable in terms of production value and the number of operators involved, to the spread of outsourcing policies, liberalization and privatization, and the use of the standard cost to establish the level of funding. In the field of public utilities, until a few years ago, the concept of control was intended only in a ＂bureaucratic＂ sense as the capacity to fulfill the formal obligations linked to the strong regulatory context, ignoring aspects that have become fundamental today, such as economy, efficiency, and effectiveness of management. The approach used is deductive and the study ends with the presentation of the possible implications of the use of standard cost and the presentation of an application hypothesis of a balanced scorecard （BSC） for local public transport （LPT）.
