Background:The serum vitamin D level varies widely by population,and studies have linked vitamin D levels with the risk of type 2 diabetes mellitus(T2DM).However,the relationship is inconsistent and the impact of vitamin D on T2DM among East Chinese adults is *** study aimed to investigate the association between serum 25-hydroxyvitamin D(25[OH]D)levels and the risk of T2DM and evaluated whether the association is modified by genetic ***:In the Survey on Prevalence in East China for Metabolic Diseases and Risk Factors(SPECT-China)cohort,1862 participants free of T2DM at baseline were included.A weighted genetic risk score was calculated with 28 variants associated with *** logistic models were used to examine the association of serum 25(OH)D and genetic risk with ***:After a 5-year follow-up,132 cases of T2DM were *** observed no significant association between quartiles of serum 25(OH)D and T2DM risk after multivariable adjustment(χ^(2)=0.571,P_(for trend)=0.426).Compared to those in the lowest quartile of 25(OH)D,the odds ratios(ORs)(95%confidence interval[CI])for participants with increased quartiles were 1.29(0.74-2.25),1.35(0.77-2.36),and 1.27(0.72-2.24),*** observed a positive association of glycated hemoglobin(HbA1c)with 25(OH)D at baseline(β=1.752,P=0.001)and after follow-up(β=1.385,P=0.003),and a negative association of ln conversion homeostasis model assessment(HOMA)-βwith 25(OH)D at baseline(β=-0.982,P=0.021).There was no significant interaction between 25(OH)D and diabetes genetic predisposition on the risk of T2DM(χ^(2)=2.710,P_(for interaction)=0.100).The lowest OR(95%CI)of T2DM was among participants with low genetic risk and the highest quartile of 25(OH)D(0.17[0.05-0.62]).Conclusion:Serum 25(OH)D may be irrelevant to the risk of incident T2DM among East Chinese adults regardless of genetic predisposition.

BACKGROUND Whether operation is superior to non-operation for humeral shaft fracture remains *** hypothesized that operation could decrease the nonunion and reintervention rates and increase the functional *** To compare the clinical efficacy between operative and nonoperative approaches for humeral shaft *** We searched the PubMed,Web of Science,ScienceDirect,and Cochrane databases from 1990 to December 2023 for clinical trials and cohort studies comparing the effects of operative and conservative methods on humeral shaft *** investigators independently extracted data from the eligible studies,and the other two assessed the methodological quality of each *** quality of the included studies was assessed using the Cochrane risk bias or Newcastle-Ottawa *** nonunion,reintervention and the overall complications and functional scores were pooled and analyzed using Review Manager software(version 5.3).RESULTS A total of four randomized control trials and 13 cohort studies were included,with 1285 and 1346 patients in the operative and nonoperative groups,*** in the operative group were treated with a plate or nail,whereas those in the conservative group were managed with splint or functional *** studies were assessed as having a high risk of bias,and the other 13 were of a low risk of bias according to the Newcastle-Ottawa Scale or Cochrane risk bias *** operative group had a significantly decreased rate of nonunion[odds ratio(OR)0.30;95%CI:0.23 to 0.40,reintervention(OR:0.33;95%CI:0.24 to 0.47),and overall complications(OR:0.62;95%CI:0.49 to 0.78)].The pooled effect of the Disabilities of Arm,Shoulder,and Hand score showed a significant difference at 3[mean difference(MD)-8.26;95%CI:-13.60 to-2.92],6(MD:-6.72;95%CI:-11.34 to-2.10),and 12 months(MD:-2.55;95%CI:-4.36 to-0.74).The pooled effect of Visual Analog Scale scores and the Constant-Murley score did not significantly differ between

[Objectives]To explore the effect of continuous nursing model based on the Omaha system on stroke patients.[Methods]A total of 40 stroke patients in the inpatient department of the Rehabilitation Department of Affiliated hospital of Jinggangshan university were selected and divided into two groups by the random number table *** in the control group were given routine nursing,and those of the observation group were given continuous nursing based on the Omaha system *** score of neurological impairment,daily living ability scale and the quality of life,as well as patients satisfaction rate before and after treatment were compared between the two groups.[Results]The score of neurological impairment,daily living activity ability and the quality of life,as well as patients nursing satisfaction rate in the observation group were significantly better than those of the control group,with statistical significance(P<0.05).[Conclusions]The continuous nursing model based on the Omaha system can significantly improve the neurological function of stroke patients,improve their quality of life and life ability,and promote their rehabilitation.

目的旨在评估儿科重症监护护士对腹内压监测的知识、态度和实践现状及培训需求,为临床制订相关培训方案和操作流程提供参考依据。方法采用横断面调查法。2023年4月—6月选取浙江省8所三级甲等医院212名儿科重症监护护士为研究对象。采用自行编制的腹内压监测知信行评估问卷进行调查,包括知识(14个条目)、态度(6个条目)和实践(8个条目)3个维度,共28个条目。结果护士问卷总分为(60.73±8.35)分;知识维度得分为(7.84±2.35)分,得分率为56.0%;态度维度得分为(25.16±3.23)分,得分率为83.9%;实践维度得分为(28.44±6.46)分,得分率为69.3%。接受过腹内压监测培训的护士问卷总分、知识和行为维度得分均高于未接受培训的护士;年龄≤30岁的护士知识维度得分高于年龄>30岁的护士(P<0.05)。55.7%的护士认为目前的知识不足以有效地进行腹内压监测。难以识别腹内高压高危人群(64.6%)、不熟悉腹内压测量的操作流程(55.6%)和护患比分配不合理(52.8%)是护士实施腹内压监测的主要障碍因素。结论儿科重症监护护士对腹内压监测持积极的态度,但其知识水平和实践行为仍有待加强,特别是对已发表的共识定义、腹内压测量方法和监测频率方面的知识是不充分的。护理管理人员要根据护士的培训需求和实施腹内压监测的潜在障碍因素,实施针对性的腹内压监测培训方案,以促进腹内压监测的标准化和科学性。
