The aging of the periocular region has always aroused great interest.A fresh,young,and attractive sight determined an ever-greater attention to surgical and non-surgical techniques to obtain this *** particular,the change in the concept of a young look,considered then“full”,led to the increasing use of surgical(fat grafting)or medical(hyaluronic acid)filling *** rejuvenation became increasingly popular in the field of cosmetic treatments,with a focus on achieving a youthful and refreshed *** the various techniques available,the choice between using fat grafting or fillers presented a clinical *** particular,what surgery considered of fundamental importance was a long-lasting result over *** the other hand,aesthetic medicine considered it fundamental not to have to resort to invasive *** what was the reality?Was there one path better than the other,and above all,was there a better path for patients?The minireview aims to explore the physiopathology,diagnosis,treatment options,prognosis,and future studies regarding this *** analyzed the literature produced in the last 20 years comparing the two *** literature reveals advancements in biomaterials,stem cell research and tissue engineering held promise for further enhancing the field of eyelid *** choice between fat grafting and fillers in eyelid cosmetic treatments presented a clinical *** physiopathology,accurately diagnosing eyelid aging,exploring treatment options,assessing prognosis,and conducting future studies were essential for providing optimal care to patients seeking eyelid rejuvenation.

Design of a Novel Macro-Micro Integrated Brain Surgery Robot Based on Modular Parallel Mechanisms

The advancement and development of medical surgical robots have provided new technological support for brain surgery and neurosurgical procedures, improving the reliability of highly complex and precise surgeries. In turn,this urges the design and development of novel surgical robots to possess higher precision, stability, and enhanced motion capabilities. In response to this practical demand, this paper introduces a macro-micro integrated medical brain surgery robot system based on the concept of modular PMs(parallel mechanisms), which have a total of 13 active DOFs(degrees of freedom). This system divides the motion process of brain surgery into a large-scale macro-motion space and a small-scale high-precision motion space for design and planning control. The introduction of the design concept that combines multiple modular parallel sub-mechanisms has brought a significant level of decoupling characteristics to the mechanism itself. A comprehensive introduction and analysis of the surgical robot are provided, covering aspects such as design, kinematics, motion planning, and performance indicators. To address the pose allocation and coordination of motion between the macro platform and the micro platform, a pose allocation algorithm based on the decoupling and non-decoupling characteristics in various dimensions of the macromicro platform is proposed. The designed measurement experiments have demonstrated that the repeatability in positioning accuracy of the macro and micro platform reaches the level of micron and submicron *** experiments of motion control and simulated brain electrode implantation validate the excellent performance and stability of the entire surgical robot system. This research contributes innovative insights to the development of medical surgical robot systems, particularly in the domain of mechanism design.

Objective:Primary tumor treatment through surgical resection and adjuvant therapy has been extensively studied,but there is a lack of effective strategies and drugs for the treatment of tumor ***,we describe a functional product based on a combination of compounds,which can be used as an adjuvant therapy and has well-known mechanisms for inhibiting cancer metastases,improving anti-cancer treatment,and enhancing immunity and antioxidant *** designed combination,named MVBL,consists of four inexpensive compounds:L-selenium-methylselenocysteine(MSC),D-α-tocopheryl succinic acid(VES),β-carotene(β-Ca),and L-lysine(Lys).Methods:The effects of MVBL on cell viability,cell cycle,cell apoptosis,cell migration,cell invasion,reactive oxygen species(ROS),and paclitaxel(PTX)-combined treatment were studied in *** inhibition of tumor metastasis,antioxidation,and immune enhancement capacity of MVBL were determined in ***:MVBL exhibited higher toxicity to tumor cells than to normal *** did not significantly affect the cell cycle of cancer cells,but increased their *** healing,adhesion,and transwell assays showed that MVBL significantly inhibited tumor cell migration,adhesion,and *** sensitized MDA-MB-231 breast cancer cells to PTX,indicating that it can be used as an adjuvant to enhance the therapeutic effect of chemotherapy *** mice,experimental data showed that MVBL inhibited tumor metastasis,prolonged their survival time,and enhanced their antioxidant capacity and immune ***:This study revealed the roles of MVBL in improving immunity and antioxidation,preventing tumor growth,and inhibiting metastasis in vitro and in *** may be used as an adjuvant drug in cancer therapy for improving the survival and quality of life of cancer patients.

AIM To determine the prevalence of work disability in inflammatory bowel disease(IBD), and to assess risk factors associated with work *** For this retrospective cohort study, we retrieved clinical data from the Dutch IBD Biobank on July 2014, containing electronic patient records of 3388 IBD patients treated in the eight University Medical Centers in the Netherlands. Prevalence of work disability was assessed in 2794 IBD patients and compared with the general Dutch population. Multivariate analyses were performed for work disability(sick leave, partial and full disability) and long-term full work disability(> 80% work disability for > 2 years).RESULTS Prevalence of work disability was higher in Crohn's disease(CD)(29%) and ulcerative colitis(UC)(19%) patients compared to the general Dutch population(7%). In all IBD patients, female sex, a lower education level, and extra-intestinal manifestations, were associated with work disability. In CD patients, an age > 40 years at diagnosis, disease duration > 15 years,smoking, surgical interventions, and anti-TNFα use were associated with work disability. In UC patients, an age > 55 years, and immunomodulator use were associated with work disability. In CD patients, a lower education level(OR = 1.62, 95%CI: 1.02-2.58), and in UC patients, disease complications(OR = 3.39, 95%CI: 1.09-10.58) were associated with long-term full work *** The prevalence of work disability in IBD patients is higher than in the general Dutch population. Early assessment of risk factors for work disability is necessary, as work disability is substantial among IBD patients.

Background: Hereditary transthyretin(ATTRv) amyloidosis is an autosomal dominant disease linked to transthyretin gene mutations which cause instability of the transthyretin tetramer. After dissociation and misfolding they reassemble as insoluble fibrils(i.e. amyloid). Apart from the common Val30 Met mutation there is a very heterogeneous group of non-Val30 Met mutations. In some cases, the clinical picture is dominated by a rapidly evolving restrictive and hypertrophic cardiomyopathy. Methods: A case series of four liver recipients with the highly clinically relevant, rare and particularly aggressive Val122 del mutation is presented. Medical and surgical therapeutic options, waiting list policy for ATTRv-amyloidosis, including the need for heart transplantation, and status of heart-liver transplantation are discussed. Results: Three patients needed a staged(1 patient) or simultaneous(2 patients) heart-liver transplant due to rapidly progressing cardiac failure and/or neurologic disability. Domino liver transplantation was impossible in two due to fibrotic hepatic transformation caused by cardiomyopathy. After a follow-up ranging from 3.5 to 9.5 years, cardiac(allograft) function was maintained in all patients, but neuropathy progressed in three patients, one of whom died after 80 months. Conclusions: This is the first report in(liver) transplant literature about the rare Val122 del ATTRv mutation. Due to its aggressiveness, symptomatic patients should be prioritized on the liver and, in cases with cardiomyopathy, heart waiting lists in order to avoid the irreversible neurological and cardiac damage that leads to a rapid lethal outcome.

Accurate localization of blood vessels with image navigation is a key element in vascular-related medical research and vascular ***,current vascular navigation techniques cannot provide naked-eye visualization of deep vascular information noninvasively and with high resolution,resulting in inaccurate vascular anatomy and diminished surgical success ***,we introduce a photoacousticenabled automatic vascular navigation method combining photoacoustic computed tomography with augmented and mixed reality,for the first time,to our knowledge,enabling accurate and noninvasive visualization of the deep microvascular network within the tissues in real time on a real surgical *** approach achieves precise vascular localization accuracy(us vessels of minimum diameter(∼0.15 mm)in rabbit thigh and the maximum depth(∼7 mm)in human arm can be vividly projected on the skin surface with a computer visionbased projection tracking system to simulate preoperative and intraoperative vascular ***,this strategy provides a way to visualize deep vessels without damage on the surgical surface and with precise image navigation,opening an avenue for the application of photoacoustic imaging in surgical operations.
