Background Blastocystis hominis(Bh)is zoonotic parasitic pathogen with a high prevalent globally,causing opportunistic infections and diarrhea *** immunodeficiency virus(HIV)infection disrupts the immune system by depleting CD4^(+)T lymphocyte(CD4^(+)T)cell counts,thereby increasing Bh infection risk among persons living with HIV(PLWH).However,the precise association between Bh infection risk and HIV-related biological markers and treatment processes remains poorly ***,the purpose of the study was to explore the association between Bh infection risk and CD4^(+)T cell counts,HIV viral load(VL),and duration of interruption in antiviral therapy among *** A large-scale multi-center cross-sectional study was conducted in China from June 2020 to December *** genetic presence of Bh in fecal samples was detected by real-time fluorescence quantitative polymerase chain reaction,the CD4^(+)T cell counts in venous blood was measured using flowcytometry,and the HIV VL in serum was quantified using fluorescence-based *** cubic spline(RCs)was applied to assess the non-linear association between Bh infection risk and CD4^(+)T cell counts,HIV VL,and duration of interruption in highly active antiretroviral therapy(HARRT).Results A total of 1245 PLWH were enrolled in the study,the average age of PLWH was 43 years[interquartile range(IQR):33,52],with 452(36.3%)being female,50.4%(n=628)had no immunosuppression(CD4^(+)T cell counts>500 cells/μl),and 78.1%(n=972)achieved full virological suppression(HIV VLs/ml).Approximately 10.5%(n=131)of PLWH had *** prevalence of Bh was found to be 4.9%[95%confidence interval(CI):3.8-6.4%]among *** nonlinear associations were observed between the Bh infection risk and CD4^(+)T cell counts(Pfor nonlinearityshaped),HIV VL(Pfor nonlinearityu.s.aped),and duration of interruption in HARRT(Pfor nonlinearityu.s.aped).Conclusions

Warming but not thawing of the cold permafrost in northern Alaska during the past 50 years

Climate warming has not resulted in measurable thawing of the cold (–5°C to –10°C) permafrost in northern Alaska during the last half century. The maximum depths of summer thaw at five locations near Barrow, Alaska, in 2005 were within the ranges of the depths obtained at those same locations during the early 1950s. However, there has been a net warming of about 2°C, after a cooling of 0.4°C during 1953–1960, at the upper depths of the permafrost column at two of the locations. Thawing of permafrost from above (increase in active layer thickness) is determined by the summer thawing index for the specific year; any warming, or cooling, of the upper permafrost column results from the cumulative effect of changes in the average annual air temperatures over a period of years, assuming no change in surface conditions. Theoretically, thawing from the base of permafrost should be negligible even in areas of thin (about 100–200 m) permafrost in northern Alaska. The reported shoreline erosion along the northern Alaska coast is a secondary result from changes in the adjacent ocean ice coverage during the fall stormy period, and is not directly because of any 'thawing' of the permafrost.

Objective:Glioma is a highly invasive tumor,frequently disposed in essential areas of the brain,which makes its surgical excision extremely difficult;meanwhile adjuvant therapy remains quite ***:In the current report,a new therapeutic approach in curing malignant neoplasms has been performed on the u87 human glioblastoma *** approach,termed"Karanahan",is aimed at the eradication of cancer stem cells(CsCs),which were recently shown to be capable of internalizing fragments of extracellular double-stranded *** being internalized,these fragments interfere in the process of repairing interstrand cross-links caused by exposure to appropriate cytostatics,and such an interference results either in elimination of CsCs or in the loss of their tumorigenic *** of the approach requires a scheduled administration of cytostatic and complex composite double-stranded DNA ***:u87 cells treated in vitro in accordance with the Karanahan approach completely lost their tumorigenicity and produced no grafts upon intracerebral transplantation into immunodeficient *** sCID mice with developed subcutaneous grafts,the treatment resulted in reliable slowing down of tumor growth rate(Psplantation of u87 cells followed by surgical excision of the developed graft and su.s.quent therapeutic treatment,the Karanahan approach was shown to reliably slow down the tumor growth rate and increase the median survival of the mice twofold relative to the ***:The effectiveness of the Karanahan approach has been demonstrated both in vitro and in vivo in treating developed subcutaneous grafts as well as orthotopic grafts after surgical excision of the tumor.

Background:Retreatments are sometimes necessary to correct residual or induced refractive errors following refractive *** different combinations of primary treatment methods and retreatment techniqu.s.have been studied,however,few studies have investigated wavefront-optimized(WFO)technology for retreatment following primary refractive *** study aimed to report the outcomes of WFO photorefractive keratectomy(PRK)retreatments of refractive error following previous laser refractive surgery with PRK,laser in situ keratomileusis(LAsIK),or laser-assisted subepithelial keratectomy(LAsEK).Methods:We reviewed records of patients who underwent WFO PRK retreatments using the Allegretto Wave Eye-Q 400 Hz Excimer Laser system(Alcon surgical)between January 2008 and April 2011 at Walter Reed army Medical Center and Madigan army Medical *** were recorded in terms of uncorrected distance visual acuity(uDVA),manifest refraction spherical equivalent(MRsE),corrected distance visual acuity(CDVA),and complications at 1 month(M),3 M,and 6 M ***:seventy-eight patients(120 eyes)underwent WFO PRK retreatment during the study *** surgery was surface ablation in 87 eyes(78 PRK,9 LAsEK)and LAsIK in 33 *** mean spherical equivalent before retreatment was−0.79±0.94 D(−3.00 to 1.88 D).uDVA was≥20/20 in 69 eyes(60.0%)at 1 M,54 eyes(71.1%)at 3 M,and 27 eyes(73.0%)at 6 M *** was within±0.50 D of emmetropia in 78 eyes(67.8%)at 1 M,59 eyes(77.6%)at 3 M,and 25 eyes(67.6%)at 6 M *** was maintained within±1 line of pre-op in 113 of 115 eyes(98.3%)at 1 M,74 of 76 eyes(97.4%)at 3 M,and 37 eyes(100%)at 6 M ***:Although follow-up was limited beyond 3 M,WFO PRK retreatments in patients with residual refractive error may be a safe and effective *** studies are necessary to determine the long-term safety and stability of outcomes.

Background： Pancreatectomies have been identified as procedures with an increased risk of *** surgical patients, readmissions within 30 days of discharge are usually procedure-related. We sought to determine predictors of 30-day readmission following pancreatic resections in a large healthcare ***： We retrospectively collected information from the records of 383 patients who underwent pancreaticresections from 2004–2013. To find the predictors of readmission in the 30 days after discharge,we performed a univariate screen of possible variables using the Fisher’s exact test for categorical variables and the Mann–Whitney u test for continuous variables. Multivariate analysis was used to determinethe independent factors.

Off-line analysis of single-unit neuronal discharge in the globus pallidus region in Parkinson＇s disease

Background The accuracy of microelectrode-guided localization can make the operation safe and effective, but only experienced neurosurgeons are capable of performing this operation. A good index to identify neuronal discharges between globus pallidus interna and globus pallidus externa is needed. The aim of this research was to establish a good and practical electrophysiologic index to distingu.s. neuronal discharge in the interior globus pallidus from neuronal discharge in the exterior globus pallidus region of the brain in Parkinson＇s disease. The effect of neurons having an atypical discharge on successful surgical localization was also quantitatively evaluated Methods The study included 30 patients with primary Parkinson＇s disease who underwent pallidotomy between september 2000 and October 2002. During each pallidotomy, the neuronal discharges in the pallidum and its vicinity were recorded. The recorded spikes were used to calculate the frequency, bu.s. index, pause index, and pause ratio of the single-unit discharge. The interior and exterior globus pallidus regions were compared in terms of frequency, bu.s. index, pause index, and pause ratio. The sensitivity, specificity, false-negative ratio, false-positive ratio, and accuracy of those indices were then evaluated. Results The valu.s.of frequency, bu.s. index, pause index, and pause ratio in the interior globus pallidus were （96±43） Hz, 2.31±1.81, 0.05±0.05, and 0.27±0.28, respectively, and in the exterior globus pallidus were （59±27） Hz, 0.88±0.63, 0.20±0.14, and 1.54±1.17, respectively. use of the four indices to distingu.s. the two neuron types produced a sensitivity of 0.84, 0.78, 0.77, and 0.93 with a specificity of 0.64, 0.79, 0.88, and 0.87, respectively. The false-positive ratio was 0.36, 0.21, 0.12, and 0.13 and the false-negative ratio was 0.16, 0.22, 0.23, and 0.07 while the accuracy was 0.72, 0.79, 0.80, and 0.90, respectively. Conclusions Pause ratio is a relatively reliable index to distingu.s.

Femoral nerve regeneration and its accuracy under different injury mechanisms

surgical accuracy has greatly improved with the advent of microsurgical techniqu.s. However, complete functional recovery after peripheral nerve injury has not been achieved to date. The mechanisms hindering accurate regeneration of damaged axons after peripheral nerve injury are in urgent need of exploration. The present study was designed to explore the mechanisms of peripheral nerve regeneration after different types of injury. Femoral nerves of rats were injured by crushing or freezing. At 2, 3, 6, and 12 weeks after injury, axons were retrogradely labeled using 1,1′-dioctadecyl-3,3,3′,3′-tetramethylindocarbocyanine perchlorate（Dil） and True Blue, and motor and sensory axons that had regenerated at the site of injury were counted. The number and percentage of Dil-labeled neurons in the anterior horn of the spinal cord increased over time. No significant differences were found in the number of labeled neurons between the freeze and crush injury grou.s.at any time point. Our results confirmed that the accuracy of peripheral nerve regeneration increased with time, after both crush and freeze injury, and indicated that axonal regeneration accuracy was still satisfactory after freezing, despite the prolonged damage.
