To the Editor:Diabetic kidney disease (DKD)is the most common cause of end-stage renal disease (EsRD);however,the onset of DKD is difficult to detect.[1]When persistent microalbuminuria becomes detectable,DKD has already progressed to the third disease stage,and finding biomarkers that are more sensitive than microalbuminuria is therefore necessary to indicate kidney damage at an earlier stage of DKD.[2]Both glomerular and tubulointerstitial damages have been repeatedly demonstrated to be important factors in the pathophysiology of DKD.[3] Therefore,we investigated the expression levels of six markers closely related to the glomerulus and renal tubule.

Modulation of alloimmune responses is critical to improving transplant outcome and promoting long-term graft *** determine mechanisms by which a nonhematopoietic erythropoietin(EPO)derivative,carbamylated EPO(CEPO),regulates innate and adaptive immune cells and affects renal allograft survival,we utilized a rat model of fully MHC-mismatched kidney *** administration markedly extended the survival time of kidney allografts compared with the transplant alone control *** therapeutic effect was inhibited when the recipients were given LY294002,a selective inhibitor of the phosphoinositide 3-kinase(PI3K)/protein kinase B(AKT)signaling pathway or anti-EPO receptor(EPOR)antibody,in addition to *** vitro,CEPO inhibited the differentiation and function of dendritic cells and modulated their production of proinflammatory and anti-inflammatory cytokines,along with activating the PI3K/AKT signaling pathway and increasing EPOR mRNA and protein expression by these innate immune ***,after CD4^(+)T cells were exposed to CEPO the Th1/Th2 ratio decreased and the regulatory T cell(Treg)/Th17 ratio *** effects were abolished by LY294002 or anti-EPOR antibody,suggesting that CEPO regulates immune responses and promotes kidney allograft survival by activating the PI3K/AKT signaling pathway in an EPOR-dependent *** immunomodulatory and specific signaling pathway effects of CEPO identified in this study suggest a potential therapeutic approach to promoting kidney transplant survival.

Objective:To describe the spanning 25 years data for the occurrence, magnitude, and trends regarding antimicrobial resistance of non-typhoidalsalmonella (NTs) isolated from non-immune travelers to Thailand participating in joint military ***:A total of 355 NTs isolates, obtained from 2052 fecal samples from us soldiers deployed for military maneuvers in Thailand during 1988-2013, were examined for NTs serogroup/serotypes and tested for antimicrobial susceptibility by disk diffusion to these 10 antibiotics:ampicillin, azithromycin (AZM), ciprofloxacin, colistin, gentamicin, kanamycin, nalidixic acid, streptomycin (sTR), tetracycline (TET), and trimethoprim/sulfamethoxazole. Identified AZM-resistant NTs isolates were further evaluated for their minimal inhibitory concentration by the E-test ***:NTs infections accounted for 17.3％ (355/2052), including 11 serogrou.s.and 50 different serotypes. The most prevalent serogroup wassalmonella group C2-C3 (35.8％, 127/355) followed by grou.s.B (21.1％, 75/355) and C1 (18.6％, 66/355). Identified serotypes includedsalmonellahadar (n=60),salmonellarissen (n=45), andsalmonella blockley (n=34). Among the predominate serogrou.s. antimicrobial resistance was consistently high against TET (76.9％, 273/355) followed by sTR (40.8％, 145/355). Onesalmonella senftenberg isolate demonstrated decreased ciprofloxacin susceptibility. Most isolates (94.6％) were resistant to one or more antimicrobials, and the most common multidrug resistance was TET-sTR-nalidixic acid (11.5％, 41/355).Conclusions:The prevalence of NTs serotypes and the growing magnitude of antibiotic resistant bacteria isolated from deployed us military in Thailand are documented from 1988-2013. This study demonstrates the antibiotic resistance profiles, highlighting the effectiveness of AZM that is a first-line treatment for travelers to southeast Asia. 目的研究外科脓毒症病人胰岛素抵抗的临床意义。方法40例外科脓毒症病人入实验组,20例同期相似APACHEⅡ评分但无脓毒症的外科病人入实验对照组。实验组再分别根据病情严重度分为sepsis组和septicshock&MODs(sM)组,根据原发病分为sAP组、肠瘘组和其他组。检测病人入组后不同时间点的空腹血糖、空腹胰岛素、临床代谢指标和血浆TNF-α浓度,并评价APACHEⅡ、sOFA和ss评分。胰岛素抵抗指数通过稳态模式评估法计算。结果脓毒症状态下胰岛素抵抗存在占84%,所有脓毒症病人lgHOMA-IR检测峰值均显著大于正常值。实验组lgHOMA-IR从24h(PsAP组、肠瘘组和其他组之间lgHOMA-IR在各时间点无显著差异(P>0.05)。sM组lgHOMA-IR在各时间点显著高于sepsis组,相关分析显示lgHOMA-IR与APACHEⅡ(r=0.591,PsOFA(r=0.496,Pss评分(r=0.553,P<0.01)和血浆TNF-α(r=0.658,P<0.01)呈显著正相关。lgHOMA-IR独立相关于LDL、前白蛋白、胆固醇和甘油三酯。R2为0.188。结论外科危重病人普遍存在胰岛素抵抗,与导致感染的原发病无关,可以作为代谢紊乱和病情严重度的预警指标。

Aiming at the problem that a large number of array elements are needed for uniform arrays to meet the requirements of direction map,a sparse array pattern synthesis method is proposed in this paper based on the sparse sensing ***,the Orthogonal Matching Pu.s.it(OMP)algorithm and the Exact Augmented Lagrange Multiplier(EALM)algorithm were improved in the sparse sensing theory to obtain a more efficient Orthogonal Multi⁃Matching Pu.s.it(OMMP)algorithm and the semi⁃Exact Augmented Lagrange Multiplier(sEALM)***,the two improved algorithms were applied to linear array and planar array pattern syntheses *** showed that the improved algorithms could achieve the required pattern with very few *** simulations verified the effectiveness and superiority of the two synthetic *** addition,compared with the existing sparse array synthesis method,the proposed method was more robust and accurate,and could maintain the advantage of easy implementation.
