Objective： To explore the interfering effect of acupuncture treatment in remodeling the swallowing functions for the patients with pseudobulbar palsy after cerebral infarction. Methods： Seventy-two cases with pseudobulbar palsy after cerebral infarction were randomly allocated into an observation group and a control group, 36 cases in each group. The treatment neurological therapies, once a day, with one day rest group was treated with acupuncture based upon routine after continuous treatment for six days, and therapeutic effects were assessed after six courses of the treatments. The control group was treated with routine neurological therapies, with the therapeutic effects observed after the treatment for 42 d. Results： The total effective rate was 80.6% in the observation group and 58.3% in the control group, with statistical significance in comparison of difference between the two groups （P〈0.05）, and with statistical significance in comparison of the score of deglutition disorder between the observation group and control group （P〈0.01）. Conclusion： Nape acupuncture can obviously improve the clinical symptoms and signs for the patients with pseudobulbar palsy, with affirmative therapeutic effects, and is worthy of popularization.

Rhizoma Drynariae Improves Endometrial Receptivity in a Mus Model of Dysfunctional Embryo Implantation

Background: Rhizoma drynariae is a traditional chinese medicine used in orthopedics and traumatology, but its effect on endometrial receptivity remains unknown. Aims and Objectives: To observe effect of Rhizoma drynariae and its main components on endometrial receptivity in a mus model of dysfunctional embryo implantation. Materials and Methods: Mus models were established by the Gn RHa+HMG+HCG method. Normal mus receiving saline were used as controls and the remaining six groups were: model receiving saline, progynova, aspirin, Rhizoma drynariae, osteopractic total flavone, and naringin. Pinopodes in uterine endometrium were examined by scanning electron microscopy. Stem cell factor(SCF) m RNA expressions was determined by real-time RT-PCR, and estrogen receptor α(ERα), progesterone receptor(PR) by immunohistochemistry. Results: In the model group, surface morphology of endometrium was heterogeneous, without obvious pinopodes. In the Rhizoma drynaria and progynova groups, pinopodes were abundant. Compared with the blank group, model group had lower levels of SCF(-47%), ERα(-63%) and PR(-50%)(all P<0.05). In comparison, Rhizoma drynariae group had higher levels of SCF(+73%), ERα(+118%) and PR(+101%)(all P<0.01). The individual main components of Rhizoma drynariae had variable efficacy. Conclusion: Rhizoma drynariae could improve endometrial receptivity of mouse models of dysfunctional embryo implantation as shown by increased numbers of pinopodes and higher levers of SCF, ERα, PR compared with the model group.

Background Older adults are prone to obesity and metabolic abnormalities and recommended to pursue a normal weight especially when obesity and metabolic abnormalities are co-existed. However, few studies have reported the possible differences in the effect of obesity on outcomes between older adults with metabolic abnormalities and those without metabolic abnormalities. Methods A total of 3485 older men were included from 2000 to 2014. All-cause mortality and cardiovascular mortality were obtained during a mean follow-up of five years Metabolic abnormalities were defined as having established hypertension, diabetes, or dyslipidemia and taking the disease-related medications. All participants were stratified by the presence or absence of metabolic abnormalities. Results In the non-metabolic abnormalities group, all-cause and cardiovascular deaths were lowest in overweight participants and highest in obese participants. In the metabolic abnormalities group, mortality was also lowest in overweight participants but highest in participants with normal weight. After adjustment for covariates, hazard ratios （95% CI） for all-cause death and cardiovascular death were 0.68 （0.51, 0.92） and 0.59 （0.37, 0.93）, respectively, in overweight participants with metabolic abnormalities. Furthermore, obesity was not associated with mortality risk in both groups. These findings were unchanged in stratified analyses. Conclusions Overweight was negatively associated with mortality risk in older men with metabolic abnormalities but not in those without metabolic abnormalities. Obesity did not increase death risk regardless of metabolic abnormalities. These findings suggest that the recommendation of pursuing a normal weight may be wrong in overweight/obese older men, especially for those with metabolic abnormalities.

Objective： To explore the clinical efficacy and safety of acupuncture in treating gastroesophageal reflux （GER）. Methods： Sixty patients with confirmed diagnosis of GER were randomly assigned to two groups. The 30 patients in the treatment group were treated with acupuncture at acupoints Zhongwan （CV 12）, bilateral Zusanli （ST36）, Sanyinjiao （SP6）, and Neiguan （PC6）, once a day, for 1 week as a therapeutic course, with interval of 2-3 days between courses; the 30 patients in the control group were administered orally with omeprazole 20 mg twice a day and 20 mg mosapride thrice a day. The treatment in both group lasted 6 weeks. Patients＇ symptoms and times of reflux attacking were recorded, the 24-h intraesophageal acid/bile reflux were monitored, and the endoscopic feature of esophageal mucous membrane was graded and scored at three time points, i.e., pro-treatment （T0）, immediately after ending the treatment course （T1） and 4 weeks after it （T2）. Besides, the adverse reactions were also observed. Results： Compared with those detected at TO, 24-h intraesophageal pH and bile reflux, endoscopic grading score and symptom score were all decreased significantly at T1 in both groups similarly （P〈0.01）, showing insignificant difference between groups （P〉0.05）. These indices were reversed at T2 to high level in the control group （P〈0.05）, but the reversion did not occur in the treatment group （P〉0.05）. No serious adverse reaction was found during the therapeutic period. Conclusion： Acupuncture can effectively inhibit the intraesophageal acid and bile reflux in GER patients to alleviate patients＇ symptoms with good safety and is well accepted by patients.

OBJECTIVE: To investigate the effect of electroacupuncture(EA) on irritable bowel syndrome(IBS) in mice through regulating nucleotide-binding oligomerization domain protein-like receptor family pyrin domain containing 6(NLRP6).METHODS: Water-avoidance stress(WAS) mice model was used to investigate the effects and the mechanism of EA. Abdominal withdrawal reflex test, open field test, and intestinal motility test were used to evaluate visceral sensitivity, anxiety,and intestinal motility in mice. The expressions of NLRP6, Mucin-2(MUC2) and E-cadherin were determined using immunofluorescence and Western blotting ***: EA significantly upregulated the expression of NLRP6 in the intestine of mice. Moreover, EA increased the expressions of MUC2 and E-cadherin in WAS ***: Our study found that the relief of IBS symptoms by EA may involve the increase in the expression of NLRP6 in WAS mice.

An evidence-based practice method according to literature retrieval through PICO （Patients,Intervention,Comparison,Outcome） questions and complementary and alternative medicine （CAM） topics,which can obtain helpful evidence for guiding clinical practice,was introduced with a practical example in this *** knowledge of diseases and Western medicine treatment can be acquired by literature retrieval through PICO question,while searching by CAM topics may provide evidence for chinese medicine （CM）.Thus the author held that literature retrieval through both PICO question and CAM topics was an ideal evidencebased practice method for integrative chinese and Western medicine （ICWM）.However,since the standard in CM evidence hierarchy is still under study,the value of the CAM thematic retrieval method remains very *** the future,studies on the definition and hierarchy of CM evidences and the herb-drug interaction between Western and chinese medicine during a combination therapy should be strengthened to improve the status of ICWM evidence-based practice.

Background Epicardial adipose tissue （EAT） is significantly associated with the formation and composition of coronary atherosclerotic plaque, cardiac events and the clinical prognosis of coronary heart disease. But, whether increased EAT deposition may affect the incidence of in-stent restenosis （ISR） is currently unclear. This study used coronary computed tomography angiography （CCTA） as a mean to investigate whether increased EAT volume was associated with ISR. Methods A total of 364 patients who underwent 64-slice CCTA examination for the evaluation of suspected coronary artery disease, and subsequently underwent percutaneous coronary intervention （PCI） for the first time, and then accepted coronary angiography （CA） follow-up for ISR examination in one year, were retrospectively included in this study. EAT volume was measured by CCTA examination. CA follow-up was obtained between 9 and 15 months. ISR was defined as 〉 50% kuninal diameter narrowing of the stent segment or peri-stent segment. EAT volume was compared between patients with and without ISR and additional well-known predictors of ISR were compared. Results EAT volume was significantly increased in patients with ISR compared with those without ISR （154.5 ± 74.6 mL vs. 131.0 ± 52.2 mL, P 〈 0.001）. The relation between ISR and EAT volume remained significant after adjustment for conventional cardiovascular risk factors and angiographic parameters. Conclusions EAT volume was related with ISR and may provide additional information for future ISR.

OBJECTIVE: To introduce a clinical case from Professor Huang Xi from Xiangya hospital in traditional chinese medical(TCM) treatment for a major depressive disorder(MDD) complicated by functional dyspepsia(FD).METHODS: A 26-year old unemployed male migrant worker(Han nationality) presented himself at the TCM outpatient department of our hospital on May 13 th 2013, with the chief complaint of persistent low mood and negative emotions lasting 2.5years. He was diagnosed as having MDD complicated by FD. The TCM differentiation made was "Ganqi-Yujie"(Liver-Qi stagnation). Zhiqiaochuanxiong(ZQCX) decoction, composed of Zhiqiao(Fructus Aurantii Submaturus) 40 g and Chuanxiong(Rhizoma Chuanxiong) 40 g, was given to be taken twice daily. The patient's condition was evaluated with a 128-channel electroencephalogram(EEG)and high-resolution manometry(HRM). During follow-up the patient was monitored with the Patient Health Questionnaire and Leeds Dyspepsia ***: After treatment, the symptoms of depression and the FD comorbidity disappeared. Furthermore, the main EEG and HRM were significantly improved after 1-7 weeks of ZQCX decoction ***: ZQCX decoction showed rapid prokinetic and anti-depressive effects in a MDD patient complicated by FD. Most commonly used antidepressants target only the brain, whereas ZQCX decoction targets the brain-gut axis according to TCM theory.
