Apolipoprotein E polymorphism influences orthotopic liver transplantation outcomes in patients with hepatitis C virus-induced liver cirrhosis

BACKGROUND Hepatitis C virus(HCV)infection is responsible for a chronic liver inflammation,which may cause end-stage liver disease and hepatocellular *** E(protein:ApoE,gene:APOE),a key player in cholesterol metabolism,is mainly synthesized in the liver and APOE polymorphisms may influence HCV-induced liver *** To determine whether APOE alleles affect outcomes in HCV-infected patients with liver cirrhosis following orthotopic liver transplantation(OLT).METHODS This was a cohort study in which 179 patients,both genders and aged 34-70 years,were included before or after(up to 10 years follow-up)*** injury severity was assessed using different criteria,including METAVIR and models for endstage liver *** polymorphisms were analyzed by quantitative real-time polymerase chain *** The APOE3 allele was the most common(67.3%).In inflammation severity of biopsies from 89 OLT explants and 2 patients in pre-transplant,the degree of severe inflammation(A3F4,0.0%)was significantly less frequent than in patients with minimal and moderate degree of inflammation(≤A2F4,16.2%)P=0.048,in patients carrying the APOE4 allele when compared to *** addition,a significant difference was also found(≤A2F4,64.4%vs A3F4,0.0%;P=0.043)and(A1F4,57.4%vs A3F4,0.0%;P=0.024)in APOE4 patients when compared to APOE3 *** fibrosis degree of the liver graft in 8 of 91 patients and the lack of the E4 allele was associated with more moderate fibrosis(F2)(P=0.006).CONCLUSION Our results suggest that the E4 allele protects against progression of liver fibrosis and degree of inflammation in HCV-infected patients.

World’s Fastest Supercomputer Picks COVID-19 Drug

We have demonstrated the application of the world’s fastest supercomputer Fugaku located in Japan to select the COVID-19 drugs and stopping the pandemic spread. Using computer simulation out of 2128 potential drug candidates, the world’s fastest supercomputer picked 30 most effective and potential drugs. Twelve of them are under clinical trials outside Japan;some are being tested in Japan. The computer reduced the computation time from one year to 10 days when compared to second superfast computer of the world. Fugaku supercomputer was employed to know the behavior of airborne aerosol COVID-19 virus. 3Cs were suggested: avoid closed and crowded spaces and contacts to stop the pandemic spread. The progress in vaccine development and proper use and type of mask has also been described in this article. The article will benefit greatly to stop spreading and treating the pandemic COVID-19.

Type 1 diabetes is a chronic illness with a high burden of *** effective interventions and recommendations for diabetes care exist,the intensive nature of diabetes management makes compliance *** isespecially true in children and adolescents as they have unique psychosocial and diabetes *** the development of effective in-person interventions targeting improving self-management and ameliorating psychosocial difficulties there are still a number of barriers to implementing these interventions,namely time,cost,and *** interventions allow for the dissemination of these interventions to a broader ***-management and psychosocial telehealth interventions are reviewed with a special emphasis on mobile phone and internet based technology *** efficacy has been demonstrated in a number of telehealth interventions with improved cost effectiveness over inperson interventions,many challenges remain including high participant attrition and difficulties with receiving reimbursement for services *** and other challenges are discussed with recommendations for researchers and telehealth providers provided.

Endoscopic full thickness resection for gastric tumors originating from muscularis propria

AIM: To do systematic review of current literature for endoscopic full thickness resection(EFTR) technique for gastric tumors originating from muscularis ***: An extensive English literature search was done till December 2015; using Pub Med and Google scholar to identify the peer reviewed original and review articles using keywords-EFTR, gastric tumor, muscularis propria. Human only studies were included. The references of pertinent studies were manually searched to identify additional relevant studies. The indications, procedural details, success rates, clinical outcomes, complications and limitations were considered. For the purpose of review, data from individual studies was combined to calculate mean. No other statistical test was ***: A total of 9 original articles were identified. Four articles were from same institute and the time frames of these studies were overlapping. To avoid duplication of data, only the study with patients over the longest time interval was included and other three were excluded. In total six studies were included in the final review. In our systematic review, the mean success rate for EFTR of gastric tumors originating from muscularis propria was 96.8%. The mean procedure time varied from a minimum of 37 min to a maximum of 105 min. There was no reported mortality from the technique itself. The most common histological diagnosis was gastrointestinal stromal tumors and leiomyoma. Gastric wall defect closure by either metallic clips or over the scope clip(OTSC) had similar outcomes although experience with OTSC was limited to smaller lesions(<3cm).CONCLUSION: EFTR is a minimally invasive technique to resect gastric submucosal tumors originating from muscularis propria with a high success rate and low complication rate.

Production of Biodiesel from Cottonseed Oil over Aminated Flax Fibres Catalyst: Kinetic and Thermodynamic Behaviour and Biodiesel Properties

The transesterification of cottonseed oil in the presence of methanol to fatty acid methyl ester (FAME) using flax-based fibres catalyst modified with an alkaline moiety was studied. The catalyst was prepared by radiation induced grafting (RIG) of glycidyl methacrylate (GMA) onto dignified flax fibres followed by amination with diethylamine (DEA) and treatment with NaOH solution. A maximum FAME conversion of 88.6% was obtained at 60°;C with a catalyst dosage of 2.5 wt%, an oil/methanol ratio of 1:33 and a time of 2 h. The biodiesel quality was verified by nuclear magnetic resonance (1H NMR). Kinetic analysis showed a reaction activation energy of 69.33 kJ·molˉ1 and a rate constant of 0.00349 minˉ1 indicating that the catalytic reaction was kinetically controlled. Thermodynamic analyses revealed that the reaction was reversible, non-spontaneous and endothermic with an enthalpy of 66.62 kJ·molˉ1. The obtained biodiesel showed physical and chemical characteristics complying with ASTM D6751. It can be concluded that the alkaline biopolymer catalyst prepared in the present study is a promising green candidate for biodiesel production.

BACKGROUND Esophageal cancer(EC)is associated with a poor prognosis,particularly so in Africa where an alarmingly high mortality to incidence ratio prevails for this *** To provide further understanding of EC in the context of the unique cultural and genetic diversity,and socio-economic challenges faced on the African *** We performed a systematic review of studies from Africa to obtain data on epidemiology,risk factors,management and outcomes of EC.A non-systematic review was used to obtain incidence data from the International Agency for Research on Cancer,and the Cancer in Sub-Saharan *** searched EMBASE,PubMed,Web of Science,and Cochrane Central from inception to March 2019 and reviewed the list of articles *** effects metaanalyses were used to assess heterogeneity between studies and to obtain odds ratio(OR)of the associations between EC and risk factors;and incidence rate ratios for EC between sexes with their respective 95%confidence intervals(CI).RESULTS The incidence of EC is higher in males than females,except in North Africa where it is similar for both *** highest age-standardized rate is from Malawi(30.3 and 19.4 cases/year/100000 population for males and females,respectively)followed by Kenya(28.7 cases/year/100000 population for both sexes).The incidence of EC rises sharply after the age of 40 years and reaches a peak at 75 years ***-analysis shows a strong association with tobacco(OR 3.15,95%CI:2.83-3.50).There was significant heterogeneity between studies on alcohol consumption(OR 2.28,95%CI:1.94-2.65)and on low socioeconomic status(OR 139,95%CI:1.25-1.54)as risk factors,but these could also contribute to increasing the incidence of *** best treatment outcomes were with esophagectomy with survival rates of 76.6%at 3 years,and chemo-radiotherapy with an overall combined survival time of 267.50 *** Africa has high incidence and mortality rates of EC,with preventable and nonmodifiable ris

Effects of USPSTF guidelines on patterns of screening and treatment outcomes for prostate cancer

The updated United States Preventive Services Task Force(USPSTF) for prostate cancer in 2012 recommends against prostate-specific antigen(PSA) based screening for men of all ages. Prostate cancer is the second most common and second most deadly cancer in American men. PSA screening for prostate cancer has been present since 1994 leading to an over diagnosis and over treatment of low volume disease. There is an overall agreement of men towards the guidelines but even with the understanding of the USPSTF, these men tend to follow more personal beliefs that have been influenced by their knowledge of the disease process and physician influence. Physicians also followed the directions of the patients and opted not to change their current practice of PSA screening despite the new guidelines. time, legal, and ethical issues were some of the barriers that physicians faced in tailoring their practice towards screening. The importance of informed consent is highlighted by both the patients and the physicians and clearly more effective when the patient was pre-informed of the disease processand prompted the physicians to initiate conversation of informed screening. Younger patients were inclined towards aggressive treatment and older patients opted towards watchful waiting both with emphasis on the importance of evidence-based information provided by the physician. Decision aids were useful in making informed decisions and could be used to educate patients on screening purposes and treatment options. However, even with well-created decision aids and physician influence, patients' own belief system played a major part in healthcare decision making in either screening or treatment for prostate cancer.

Association between Comorbidities and Selected Sociodemographic Factors with Complications of Diabetes: Results from the National Diabetic Registry Malaysia

Background: This study aims to determine the hazard ratio of having any complication from diabetes mellitus, and the associations between comorbidities and risk of having any complications from diabetes mellitus among diabetic patients who have attended government primary care clinics. Methods: Secondary data were retrieved from the Malaysian National Diabetic Registry which included all patients who received care. The data from the study on the socio-demographic, diabetes complications, clinical and treatment characteristics were analyzed using descriptive statistics. Cox regression was performed to estimate the hazard ratio for comorbidities, tobacco use, duration of diabetes and socio-demography characteristics upon time to diabetic complications. Results: Adjusted for other covariates, increase number of comorbidities contributed the highest hazard ratio risk: 1 comorbid (aHR: 2.47, 95% CI: 2.39, 2.55), 2 comorbidities (aHR: 4.34, 95% CI: 4.22, 4.47), 3 comorbidities (aHR: 6.56, 95% CI: 6.31, 6.81) and 4 comorbidities (aHR: 9.13, 95% CI: 8.20, 10.17). Other factors: age > 40 years (8%) Malays (27%) and smokers (10%) have hazard risks to develop diabetic complications. Conclusions: Increase in number of comorbidities will increase the risk of getting diabetes complications. Other factors such as age, gender, race, smoking status and duration of diabetes are also noted to contribute to increase risk for diabetes complications.

The purpose of this research is to develop the algorithms that control the behavior of heliostat in real time. Tracking the sun and calculating the position of the sun are possible by using illuminance sensor (CdS). As heat transfer from heliostat to receiver is delivered by solar radiation, configuration factor commonly utilized in radiation is applied to control heliostat. Algorithms for maximizing configuration factor between heliostat to receiver in real time are programmed by Simulink. By applying the optimized algorithms, the efficiency of the solar absorption in receiver can be maximized. Simulation was performed how to change the incidence angle during the daytime with respect to diverse distances.
