Common bacteria distribution and antibiotics choice in trauma patients with wounds

Objective The distribution and antibiotics resistance of common bacteria in the trauma patients with wounds were studied to direct the using antibiotics correctly. Methods Secretion bacteriology and bacterial sensitivity were detected for therapy in trauma patients with wounds from 1996 to 2000. The antibiotics resistance were examined. Results A total of 2 396 strains in 3 545 cases-times from wounds were isolated, the positive rate is 67.59%,the first five common strains were Staphylococ-cusaureus, S. epidermidis, Pseudomonasaeruginosa, Enterococcus,and Escherichia coli. The resistances rate to common antibiotics of *** was≥80 % , imipenem/ cilastatin (Tienam) and vancomycin was still effective to the G+ cocci. G- bacteria had higher resistance against the 3rd generation of cephalosporins. Conclusion The bacteriology and the surveillance of antibiotics resistance are important to using antibiotics reasonably. 6 refs,3 tabs.

Non-healing wounds of fracture patients: A clinical analysis

Objective To investigate the causes and regularities of Acinetobacter infection of the non-healing wounds of fracture patients. Methods To study retrospectively clinical factors in 33 hospitalized fracture patients with non-healing wounds and bacteriology culturing of their wound secretion positive to Acinetobacter in our hospital from Jan. 1999 to Mar. 2002, in order to provide a comprehensive intensive care. Results Many susceptible factors were revealed to be responsible for the Acinetobacter infection in the non-healing wounds of fracture patients, for example, wounds exposing surgical operation, combined and long-term using broad-specturm antibiotics. Many susceptible factors were irresponsible,for instance, age, chronic diseases, using adrenocortical hormones. Conclusion The steps of prevention were mainly the strict aseptic manipulation, short wounds exposing and surgical procedure. The key is reasonable using antibiotics. 8 refs,2 tabs.

Objective To explore the early diagnosis and surgical treatment of scapular fractures. Methods Sixteen cases with scapular fractures received operation that had been admitted to our hospital from January 1996 to January 2001 were retrospectively analyzed. AP Views of X-ray, two-dimensional CT scannings and three-dimensional CT reconstructions were used to confirm the injuries. According to Hardegger’ s classification for scapular fractures, There were 9 fractures of the scapular body,7 fractures of the scapular neck, 2 fractures of the glenoid rim, 1 fracture of the glenoid fossa,5 fractures of the scapular spine, 3 fractures of the acromion and 2 fractures of the coracoid process. Of the 16 patients, 11 (68.8%) cases had complex fractures of the scapula,and 14 cases(87.5%) had associated injuries. On the basis of the fracture patterns, anterior, posterior, posterosuperior approach, and anterior combined posterior approach were performed with plates or ( and) lag screws for surgical reconstruction.

Objective To investigate the relationship between the severity of acute pancreatitis and apoptosis of acinar cells. Methods The apoptotic ratio of acinar cells was measured in rat acute pancreatitis model by methods of in situ end labeling. Results There was almost no apoptotic acinar cells in the model of acute edematous pancreatitis. With the increase in the severity of the pancreatitis apoptotic cells were more and more common until to the kte stage of hemorrhageic and necrotic pancreatitis when the apoptotic ratio of acinar cells dwindled,meanwhile the necrotic acinar cell increased continuously. Conclusion In acute simple edematous pancreatitis apoptotic acinar cells is infrequently seen. In acute moderate hemorrhagic and necrotic pancreatitis the ratio is correlated positively with the severity of pancreatitis. In the late stage the ratio was correlated negatively with the severity of pancreatitis. 3 refs,3 figs,2 tabs.
