A prospective study evaluating emotional disturbance in subjects undergoing defecating proctography

AIM: To investigate the prevalence of psychiatric illness in association with functional gastrointestinal disorders using defecating proctography (DP) and validated questionnaires. METHODS: We prospectively evaluated 45 subjects referred for DP using hospital anxiety and depression scale (HADS), state trait anxiety inventory (STAI), patient health questionnaire 15-item somatic symptom severity scale (PHQ-15), validated questionnaires for sexual or physical abuse; post-traumatic stress disorder questionnaire (PTSD) and ROME-Ⅲ questionnaires for gastrointestinal complaints. DP results were considered negative if levator ani function was normal, rectoceles (if any) were < 4 cm and there was no evidence ofintussusception, rectal prolapse, or other anatomic abnormality demonstrated. Subjects were subsequently divided into those with structural defects seen on DP (DP positive group) and those with a normal defecography study (DP negative group). RESULTS: Forty five subjects were included in the study of which 20 subjects were classified as DP negative (44.4%). There was a striking prevalence of a history of sexual abuse in DP negative group compared to the DP positive group (n = 9, 5 respectively; P = 0.036). Further, subjects in the DP negative group scored significantly higher on the HADS anxiety (6.60 ± 1.00 vs 4.72 ± 0.40, P = 0.04) and depression scales (5.72 ± 1.00 vs 3.25 ± 0.46, P = 0.01). This correlated well with significantly higher scores on the STAI state anxiety scale (42.75 ± 3.16vs 35.6 ± 2.00,P = 0.027), PHQ-15 questionnaire (13.15 ± 0.82 vs 10.76 ± 0.97, P = 0.038) and prevalence of PTSD (20%vs 4%,P = 0.045) among DP negative subjects. There was no difference between the groups in terms of STAI trait anxiety. CONCLUSION: The findings of this prospective study demonstrate a significantly high degree of psychiatric ailments in patients with negative findings on DP who should be appropriately screened for a history of sexual abuse and symptoms of psychosoci

Plasma polymerized acetylene deposition using a return corona enhanced plasma reactor

A corona based weakly ionized plasma source was developed to deposit plasma polymerized acetylene coating at atmospheric pressure. The plasma source included a distinctive point-topoint geometry consisting of an array of high voltage needles and an array of protrusions placed over a grounded screen. The geometry facilitated various corona discharge modes that included return corona to contribute plasma polymerized acetylene deposition downstream from the corona section. Scanning probe techniques were used to investigate deposition on both the leading surface and the trailing surface of substrates. Deposition was initiated as distinct nodules that merged to form a thin plasma polymerized coating.

Androgens and prostate disease

A growing body of literature has established the anabolic benefits of testosterone （T） therapy in hypogonadal men. However, there remains a paucity of data regarding the risks of exogenous androgen use in older men and the potential for adverse effects on the prostate gland. Whether T therapy in older, hypogonadal men might worsen lower urinary tract symptoms or exacerbate, unmask, or even incite prostate cancer development has tempered enthusiasm for T therapy, while known prostatic disease has served as a relative contraindication to T therapy. Androgens are necessary for the development and maintenance of the prostate gland. However, epidemiologic studies do not consistently find a positive relationship between endogenous serum androgen concentrations and the risk of prostate disease. Recent data demonstrate that 5α-reductase inhibitors decrease the risk of low-grade prostate cancer, suggesting that modifying androgen metabolism may have beneficial effects on prostate health, yet similar reductions in high-grade disease have not been observed, thereby questioning the true clinical benefits of these agents for chemoprevention. Knowing how to best investigate the relationship between androgens and the development of prostate disease given the lack of large, randomized trials is difficult. Accumulating data challenges the assumption that alterations in serum androgens have parallel effects within the prostate hormonal environment or change androgen-regulated processes within the gland. Long-term intervention studies are needed to truly ascertain the effects of androgen manipulation on prostate tissue and disease risk. However, available data do not support the notion that restoring serum androgens to normal physiologic ranges drives prostate disease.

Updates on treatment of irritable bowel syndrome

Irritable bowel syndrome （IBS） is a highly prevalent gastrointestinal disorder characterized by abdominal pain and discomfort in association with altered bowel habits. It is estimated tD affect 10%-15% of the Western population, and has a large impact on quality of life and （in）direct healthcare costs. IBS is a multifactorial disorder involving dysregulation within the brain-gut axis, and it is frequently associated with gastrointestinal motor and sensory dysfunction, enteric and central nervous system irregularities, neuroimmune dysregulation, and postinfectious inflammation. As with other functional medical disorders, the treatment for IBS can be challenging. Conventional therapy for those with moderate to severe symptoms is largely unsatisfactory, and the development of new and effective drugs is made difficult by the complex pathogenesis, variety of symptoms, and lack of objective clinical findings that are the hallmark of this disorder. Fortunately, research advances over the past several decades have provided insight into potential mechanisms responsible for the pathogenesis of IBS, and have led to the development of several promising pharmaceutical agents. In recent years there has been much publicity over several of these new IBS medications （alosetron and tegaserod） because of their reported association with ischemic colitis and cardiovascular disease. While these agents remain available for use under restricted prescribing programs, this highlights the need for continued development of safe and effective medication for IBS. This article provides a physiologicallybased overview of recently developed and frequently employed pharmaceutical agents used to treat IBS, and discusses some non-pharmaceutical options that may be beneficial in this disorder.

Introduction:The central premise underlying international payments for Reducing Emissions from Deforestation and forest Degradation(REDD+)is that offering governments ex post payments for verified success in reducing emissions will motivate them to protect and restore ***,the extent to which performance-based payments motivate governments to protect and restore forests has yet to be evaluated *** have only quantitatively evaluated performance-based payments to non-governments for forest outcomes(*** for ecosystem services)and to governments for non-forest outcomes(***-based aid).Methods:We describe how researchers now have an opportunity to more easily evaluate performance-based payments to governments for forest outcomes thanks to India’s new ecological fiscal transfers(EFTs),which provide$6-12 billion per year to Indian states in proportion to their forest ***:India’s EFTs differ from REDD+programs in that they pay for states’stock of forest area in the recent past rather than reductions in the rate of forest carbon loss in the ***,India’s EFTs focus on a single outcome and have many recipient governments,significant financial scale,lack of contemporaneous confounding policy changes,universal participation,and long-term data ***:These features make India’s EFTs especially useful for testing the payment-forperformance premise of REDD+.

Siglec genes confer resistance to systemic lupus erythematosus in humans and mice

A recent meta-analysis revealed the contribution of the SIGLEC6 locus to the risk of developing systemic lupus erythematosus(SLE).However,no specific Siglec(sialic acid-binding immunoglobulin-like lectin)genes(Siglecs)have been implicated in the pathogenesis of ***,we performed in silico analysis of the function of three major protective alleles in the locus and found that these alleles were expression quantitative trait loci that enhanced expression of the adjacent SIGLEC12 *** data suggest that SIGLEC12 may protect against the development of SLE in Asian *** with human genetic data,we identified two missense mutations in lupus-prone ***1/Sle2/Sle3(Sle1–3)mice in Siglece,which is the murine Siglec with the greatest homology to human *** the mutations resulted in reduced binding of Siglec E to splenic cells,we evaluated whether Siglece−/−mice had SLE *** found that Siglece−/−mice showed increased autoantibody production,glomerular immune complex deposition and severe renal pathology reminiscent of human SLE *** data demonstrate that the Siglec genes confer resistance to SLE in mice and humans.

Natural killer(NK)cells are attractive effector cells of the innate immune system against human immunodeficiency virus(HIV)and ***,NK cell therapies are limited by the fact that target cells evade NK cells,for example,in latent reservoirs(in HIV)or through upregulation of inhibitory signals(in cancer).To address this limitation,we describe a biodegradable nanoparticlebased“priming”approach to enhance the cytotoxic efficacy of peripheral blood mononuclear cell-derived NK *** present poly(lactic-co-glycolic acid)(PLGA)nanodepots(NDs)that co-encapsulate prostratin,a latency-reversing agent,and anti-CD25(aCD25),a cell surface binding antibody,to enhance primary NK cell function against HIV and *** utilize a nanoemulsion synthesis scheme to encapsulate both prostratin and aCD25 within the PLGA NDs(termed Pro-aCD25-NDs).Physicochemical characterization studies of the NDs demonstrated that our synthesis scheme resulted in stable and monodisperse *** NDs successfully released both active prostratin and anti-CD25,and with controllable release *** Pro-aCD25-NDs were administered in an in vitro model of latent HIV and acute T cell leukemia using J-Lat 10.6 cells,the NDs were observed to prime J-Lat cells resulting in significantly increased NK cell-mediated cytotoxicity compared to free prostratin plus anti-CD25,and other *** findings demonstrate the feasibility of using our Pro-aCD25-NDs to prime target cells for enhancing the cytotoxicity of NK cells as antiviral or antitumor agents.
